B12 PERMANENT STABILIZATION METHODS, INCLUDING SEQUENCING INFORMATION, APPLICATION RATES FOR SOIL AMENDMENTS AND SEED MIXTURES, AND THE TYPE AND APPLICATION RATES FOR SOIL AMENDMENT AND ANCHORED MULCH ARE INCLUDED IN THE SPECIFICATIONS. B13 MATERIAL HANDLING AND SPILL PREVENTION PLAN: PURPOSE: THE INTENTION OF THIS SPILL PREVENTION, CONTROL AND COUNTERMEASURES PLAN (SPCCP) IS TO ESTABLISH THE PROCEDURES AND EQUIPMENT REQUIRED TO PREVENT THEIR DISCHARGE TO STORMWATER. THE PLAN ALSO ESTABLISHES THE ACTIVITIES REQUIRED SHOULD SUCH OCCUR. DEFINITIONS: POLLUTANT MEANS POLLUTANT OF ANY KIND OR IN ANY FORM, INCLUDING BUT NOT LIMITED TO: SEDIMENT, PAINT, CLEANING AGENTS, CONCRETE WASHOUT, PESTICIDES, NUTRIENTS, TRASH WITH WASTE OTHER THAN DREDGED SOIL. DISCHARGE: INCLUDES BUT IS NOT LIMITED TO: ANY SPILLING, LEAKING, PUMPING, POURING, EMITTING, EMPTYING, OR DUMPING. PLAN REVIEW AND AMENDMENTS: THIS PLAN SHALL BE REVIEWED AND/OR AMENDED, IF NECESSARY, WHENEVER THERE IS A CHANGE IN THE DESIGN OF THE SITE, CONSTRUCTION, OPERATION, OR MAINTENANCE WHICH MATERIALLY AFFECTS THE SITE'S POTENTIAL FOR THE DISCHARGE OF REGULATED MATERIAL. THEREFORE, IT IS FELT THERE IS LITTLE OR NO POSSIBILITY OF POST CONSTRUCTION GROUNDWATER CONTAMINATION. ALERT PROCEDURES FOR SPILLS: 1. ANY PERSONNEL OBSERVING A SPILL WILL IMMEDIATELY INITIATE THE FOLLOWING PROCEDURE: CONTAIN AND/OR STOP THE SOURCE OF THE SPILL. 2. DIAL 911 FROM ANY TELEPHONE AND NOTIFY THE APPROPRIATE EMERGENCY PERSONNEL. 3. THE CONTRACTORS DESIGNATED EMERGENCY COORDINATOR WILL THEN TAKE THE FOLLOWING ACTIONS: BARRICADE THE AREA, ALLOW NO VEHICLES TO ENTER OR LEAVE THE SPILL ZONE AND CONTAIN THE SPILL. NOTIFY THE INDIANA DEPARTMENT OF ENVIRONMENTAL MANAGEMENT, OFFICE OF EMERGENCY RESPONSE (IDEM-OER) BY CALLING EITHER OF THE FOLLOWING TELEPHONE NUMBERS: OFFICE 1-317-233-7745 TOLL FREE 1-800-233-7745 NOTIFY WASTE RECOVERY CONTRACTOR, MAINTENANCE PERSONNEL OR OTHER CONTRACTUAL PERSONNEL NECESSARY FOR CLEANUP. COORDINATE AND MONITOR CLEANUP UNTIL THE SITUATION HAS BEEN STABILIZED AND SPILLS HAVE BEEN ELIMINATED. COOPERATE WITH THE IDEM-OER ON PROCEDURE AND REPORTS INVOLVED WITH THE EVENT. CLEANUP PARAMETERS: 1. THE CONTRACTOR SHALL MAINTAIN LISTS OF READILY AVAILABLE PERSONNEL, VAC-TRUCKS, TANK PUMPERS AND OTHER EQUIPMENT READILY ACCESSIBLE FOR CLEAN-UP OPERATIONS. IN ADDITION, A CONTINUALLY UPDATED LIST OF AVAILABLE ABSORBENT MATERIALS AND CLEAN-UP SUPPLIES SHALL BE KEPT ON SITE. 2. ALL CONSTRUCTION PERSONNEL WILL BE MADE AWARE OF TECHNIQUES FOR PREVENTION OF SPILLS. THEY SHALL BE INFORMED OF THE REQUIREMENTS AND PROCEDURES OUTLINED IN THIS PLAN. REMOVE DEPOSITED SEDIMENT WHEN IT REACHES HALF THE HEIGHT OF THE PERIMETER PROTECTION AT ITS LOWEST POINT, OR IS CAUSING THE PERIMETER PROTECTION TO BULGE. 3. TAKE CARE TO AVOID UNDERMINING THE PERIMETER PROTECTION DURING CLEAN OUT. 4. PLACE GEOTEXTILE FILTER FABRIC BAGS WHEN THEY BECOME THREE QUARTERS FULL, OR WHEN THE SEDIMENT IMPEDES THE PASSAGE OF STORMWATER INTO THE SYSTEM. 2. INLET PROTECTION SHALL BE INSPECTED AND CLEANED AFTER EVERY RAIN EVENT, OR AT LEAST ONCE A WEEK. CONCRETE WASHOUT AREA 1. INSPECT DAILY AND AFTER EACH STORM EVENT. 2. INSPECT THE INTEGRITY OF THE OVERALL STRUCTURE INCLUDING, WHERE APPLICABLE, THE CONTAINMENT SYSTEM. 3. INSPECT THE SYSTEM FOR LEAKS, SPILLS, AND TRACKING OF SOIL BY EQUIPMENT. 4. INSPECT THE POLYETHYLENE LINING FOR FAILURE, INCLUDING TEARS AND PUNCTURES. 5. ONCE CONCRETE WASTE HARDENS, REMOVE AND DISPOSE OF MATERIAL. 6. EXCESS CONCRETE SHOULD BE REMOVED WHEN THE WASHOUT SYSTEM REACHES 50% CAPACITY. 7. THE POLYETHYLENE LINER SHOULD BE REPLACED AFTER THE REMOVAL OF CONCRETE WASTE. B15 DOES NOT APPLY TO THIS PROJECT POST-CONSTRUCTION COMPONENTS C1 THE PROPOSED PROJECT IS A STORM SEWER INSTALLATION. POST CONSTRUCTION POLLUTANT EXPOSURE INCLUDE THOSE PRODUCED BY VEHICULAR TRAFFIC SUCH AS: OIL, GREASE, ANTIFREEZE, BRAKE FLUID, BRAKE DUST, RUBBER FRAGMENTS, DUST, GASOLINE, DIESEL FUEL, AND OTHER METALS AND FRAGMENTS FROM VEHICULAR SOURCES. OTHER POLLUTANTS INCLUDE GRIT (SEDIMENT) FROM WEAR OF THE ROADWAY OR WASHING OFF VEHICLES AND TRASH FROM LITTERING AND IMPROPER DISPOSAL OF WASTE. C2 UNDERDRAINS WILL BE INSTALLED ALONG THE ALIGNMENTS OF HYBRID HITCHES. THE UNDERDRAIN PIPE SIZES RANGE FROM 4" TO 18" AND WILL BE PERFORATED HDPE PIPE THAT WILL SERVE AS A CONVEYANCE ELEMENT AS WELL AS COLLECTION POINT FOR INFILTRATIVE FLOWS. CLEAN STONE AND GEOMEMBRANE WILL BE INSTALLED ALONG THE UNDERDRAINS TO ENHANCE INFILTRATION AND REDUCE PONDING SURFACE WATER. INLETS WILL BE INSTALLED ALONG THE HYBRID DITCH SYSTEM AND ARE GENERALLY CONFIGURED TO PROVIDE MINOR PONDING PRIOR TO OVERTOPINIG INTO INLETS IN ORDER TO FORCE INITIAL FLOWS TO INFILTRATE OR ENCOUNTER MICRO-PONDING PRIOR TO BEING COLLECTED INTO THE STORMSEWER NETWORK. THE CONTRACTOR SHOULD FOLLOW THE GENERAL SEQUENCE BELOW: 1. EROSION AND SEDIMENT CONTROL PRACTICES FOR EACH HYBRID DITCH SECTION WILL NEED TO BE INSTALLED AND MAINTAINED DURING THE COURSE OF CONSTRUCTION. 2. STORMWATER POLLUTION PREVENTION PRACTICES SHOULD BE REVIEWED AND MAINTAINED AS NEEDED TO KEEP THEM EFFECTIVE. SEDIMENT WILL BE REMOVED FROM SEDIMENT CONTROL PRACTICES AND INLETS AFTER THE SITE IS PERMANENTLY STABILIZED. 3. REVIEW ALL DISTURBED AREAS FOR FINAL STABILIZATION. A UNIFORM COVER OF 70% PERENNIAL VEGETATION IS REQUIRED. 4. THE CONTRACTOR WILL REMOVE THE TEMPORARY EROSION OR SEDIMENT CONTROL PRACTICES AS THE VEGETATIVE COVER FOR EACH PRACTICE IS PERMANENTLY STABILIZED. 5. SUBMIT THE NOTICE OF TERMINATION (NOT) C3 POST CONSTRUCTION STORMWATER QUALITY MEASURES: STORMWATER QUALITY MEASURES INCLUDE SEEDING, SODDING, AND MULCHING, ETC. ALL DENUDED AREAS NOT PAVED WILL BE RE-VEGETATED. ALL PAVED AREAS WILL BE KEPT FREE OF LITTER AND DEBRIS. HYBRID DITCHES WILL SERVE AS THE MAIN SOURCE OF POST-CONSTRUCTION STORMWATER QUALITY MEASURES. THE DITCHES WILL COLLECT RUNOFF FROM THE ROADWAY AND FILTER THROUGH THE SOIL TO THE UNDER DRAINS AND ULTIMATELY THE INLETS.
