-When building shell and 1st tenant came
in, SNL discussed with M.Mance of
Paragus that 1st tenant would be suite
100, and second tenant (in this 2 tenant
building) would be suite 150. This was
a change from the alpha designation
suite identifiers used in previous
buildings at this site. SNL confirmed
with M.Mance on 2/1/06 that this suite
was to be #150, because they had
inadvertantly listed suite B on the
application and plans. SNL told Mance
that she would make changes to app. and
plans to reflect the correct #.
This permit is valid only if construction commences within one (I) year of the date of issuance of the State Commerci~ll Design Release. All construction
must be completed (C/O issued) within two (2) years of the issuance date.
I, the undersigned, agree that any construction, reconstruction, enlargement, relocation, or alteration of a structure, or any change in the use of land or structures
requested by this application will comply with, and conform to, <111 <1pplicable laws of the State of Indiana, and the "Zoning Ordinance of Carmel Indiana - 1993"
(Z- 289) and amendments, adopted under authority of I.c. 36-7 et seq, General Assembly of the State of Indiana, and all Acts amendatory thereto. I further certify
that only kitchen, bath. and floor drains are connected to the sanitary sewer. I further certify that the construction will not be used or occupied until a
Certificate of Geeupiwcy has been issued by the Dep;utment of Community Services, Carmel, Indiana.
