and license # on 3/8/06. SNL entered
the information into the contractor
screen.
Original State Release, dated 12/30/05,
covered ARCH, ELEC, MEeH, PLUM, with
three standard conditions. An Addendum
dated 3/6/06 was also submitted, due to
changes to the storefront which caused
changes in the ARCH, ELEC, MECH systems.
Copy of approved Letter of Grant, dated
2/17/06 (@06010022ADLS AMEND) was
submitted with the application.
This permit is valid only if construction commences within one (I) year of the date of issuance of the State Commercial Design Release. All construction
must be completed (C/O issued) within two (2) years of the issuance date.
I, the undersigned, agree that any construction. reconstruction, enlargement, relocation, or alteration of a structure, or any change in the use of land or structures
requested by this application will comply with, and conform to, all applicable laws of the State of Indiana, and the ~Zoning Ordinance of Carmel Indiana - ]993~
