Submittal mtg. 1/31/06 w/J.Blanchard,
S.Lillard, Mark Demerly, and Jimmy Dulin
present. Additions to existing building
are walk-in/cooler area on 1 st floor,
and conversion of some crawl to basement
area on lower level. Basement will be
bar area. 1 st floor main dining area.
2nd floor will be family dining/large
party loft area. NON-VENTING FIREPLACE
TO BE INSTALLED IN BASEMENT. Bldg is
fUllY sprinklered. Kitchen may have
wood grill. Make up air return to be
located on exterior E wall of kitchen.
No ramp to main entry, but marked path
to lift on side of building for access
for disabled persons. Re-building patio
on front to be outdoor seating area.
West side of building will have open air
seating area on hard surface, which also
provides a route of access to lift. An
entrance to the lower level bar will
also be located on W side, and has
covered roof area over it.
1.Meeting representatives are to provide
J.Blanchard with Engineer's letters
regarding the supports being used in the
building.
2.Meeting representatives agreed to
contact Gary Hoyt regarding strobe light
system.
3.Partial release at issue. Builder is
to supply plans and amendmenUrevision
application with the completion of the
State release, when it is obtained.

4.lssues with City Engineer's office to
be resolved/completed or holds may
eventually be placed on inspections.
5.lssues with Carmel Utilities must be
resolved and any fees potentially due
paid prior to issuance of a C/O.
This permit is valid only if construction commences within one (1) year of the date of issuance of the State Commercial Oesign Release. All construction
must be completed (CIO issued) within two (2) years of the issuance date.

Certificate of Occupancy has been issued by the Department of Conununity Services, Carmel, Indiana.
