It has been reviewed and approved by the City of Carmel. A copy of the IDEM Notice of Intent (NOI) approval letter for this project is included in Appendix B. A copy of the permit is included in Appendix H. All forms in this SWPPP must be completed by the Contractor. A copy of this SWPPP must be kept on-site for the entirety of construction operations and made available to IDEM or City of Carmel representatives upon request. Nature and Type of Construction Activity The project site is a part of the Mayflower Park business development and is approximately 5.2 acres in size. The proposed project includes a new two-story building addition and expansion of existing parking and hardscape. New storm sewer inlets will collect runoff from the parking lot surface drainage and building roof drainage and will tie into the existing stormwater infrastructure. Since the site detention basin was designed for a full site buildout, no changes are proposed for the detention pond or the pond outlets. b. Total Area of Site / Area to be Disturbed Total Area of Site – 5.2 acres Area to be Disturbed – 1.3 acres c. Impervious Area / Percent Imperviousness Created by Construction Activity The proposed project additions will add approximately 1.02 acres of impervious area to the site. d. Storm Water Calculations The existing regional detention pond system was masterplanned for a full build-out condition of the site per the reference reports “Mayflower Park Drainage Narrative” prepared by Fink, Roberts, & Petrie, Inc., April 11, 2001 and the “Mayflower Vet Clinic Drainage Report” prepared by DeBoy Land Development Services, Inc., June 9, 2005 and per the IDEM general permit at the time of construction. Water quality treatment is provided using an existing Aqua-swirl (AS-4) device for treatment prior to discharging into the detention pond. e. Prior Land Use(s) The existing site includes an existing MedVet hospital building and associated parking areas. There is an existing storm sewer system on site which discharges into an existing retention pond for water quantity treatment. Water quality treatment is provided for the site by utilizing an Aqua-swirl (AS-4) device prior to storm sewer discharge into the retention pond. The remainder of the site drains to the pond via swales. The retention pond outlets through a 24” CMP at the southeast corner of the site. f. Implementation Schedule – Sequence of Major Construction Operations Clearing and demolition • Construct perimeter erosion and sediment control measures, including construction entrance, silt fence, and inlet filters. Construct on-site sediment basins. • Clear and Grub. Perform on-site demolition. • Strip and stockpile topsoil on-site. Provide temporary seeding and silt fencing. Grading • Perform rough grading. Stabilize exposed surfaces per plan. Construction of infrastructure (sewers, buildings, parking lots, etc.) • Install site utilities, including storm sewers and inlet filters. • Complete final grading for hardscape areas. • Install aggregate base, walk, curbs, plazas, pavement, and buildings. • Complete final grading for lawn, landscaped areas. Stabilize with permanent seeding. Post-construction • Confirm site stabilization • Remove all erosion and sediment controls • File NOT when discharges of stormwater associated with construction activity and land disturbance have ceased. g. Cover Page Cover pages with the name and location of the site, the name and contact information for all construction site operators, the name and contact information for the person responsible for authorizing/amending the SWPPP, and the estimated dates construction with start and be complete, are on sheets 3 and 4 of this plan. h. Log Documenting Land Disturbance and Stabilization Activities A log for documenting land disturbance and stabilization activities is included in Appendix D. i. Site Map A Site Map showing information required in Part III.A.G.n of the permit is included in Appendix A. Controls a. Preservation Methods The contractor shall phase construction to limit the amount of disturbed land at any one time. All areas outside the limits of construction shall be preserved in their natural state. b. Erosion Control Practices Erosion Control Practices shall be used to provide cover over disturbed soils. Practices include: • Seeding – Requirements for permanent and temporary stabilization are listed in Table 1 and Table 2 below. Specifications and seed mixes for temporary and permanent stabilization are included in the Construction Documents. • Mulching - Straw mulch shall be applied to all seeded areas to facilitate germination. Apply mulch liberally throughout construction operations. Specifications for applying and maintaining straw mulch are included in the Construction Documents. • Matting, Sod Stabilization - Erosion Control Matting or Sod stabilization shall be installed on slopes where shown in the Construction Documents. • Vegetative Buffer Strips - Vegetative Buffer Strips shall be maintained where shown in the Construction Documents. • Phasing of Construction Operations – The contractor shall phase construction to limit the amount of disturbed area at any one time. All areas outside the limits of construction shall be preserved in their natural state. • Construction Entrances – All construction vehicles shall use designated construction entrances as shown and detailed in the Construction Documents. • Alternative Ground Cover – Alternative ground cover may be used where site locations or weather conditions prevent the use of standard site stabilization techniques. Alternative ground covers shall minimize the exposure and erosion of denuded areas. i. Site Stabilization – Disturbed areas shall be stabilized in accordance with Table 1 and Table 2 below: Table 1: Permanent Stabilization Areas Requiring Permanent Stabilization Time Frame to Apply Erosion Controls Any areas that will lie dormant for one year or more. Within seven days of the most recent disturbance. Any areas within 50 feet of a surface water of the state and at final grade. Within two days of reaching final grade. Any other areas at final grade. Within seven days of reaching final grade within that area. Table 2: Temporary Stabilization Area Requiring Temporary Stabilization Time Frame to Apply Erosion Controls Any disturbed areas within 50 feet of a surface water of the state and not at final grade. Within two days of the most recent disturbance if the area will remain idle for more than 14 days. For all construction activities, any disturbed areas that will be dormant for more than 14 days but less than one year, and not within 50 feet of a of a surface water of the state. Within seven days of the most recent disturbance within the area. For residential subdivision, disturbed areas must be stabilized at least seven days prior to transfer of permit coverage for the individual lot(s). Disturbed areas that will be idle over winter. Prior to the onset of winter weather. ii. Permanent Stabilization of Conveyance Channels – Operators shall take special measures to stabilize channels and outfalls. Measures include seeding, mulching erosion control matting, and sodding as listed above. Additional measures include: • Rock Channel Protection - Rock Channel Protection shall be installed at pipe outlets where shown in the Construction Documents. • Rock Check Dams - Rock Check Dams shall be installed in swales and ditches where shown in the Construction Documents. c. Runoff Control Practices Runoff from disturbed areas shall be controlled to prevent erosion. Practices include: • Rock Check Dams - Rock Check Dams shall be installed in swales and ditches where shown in the Construction Documents. • Pipe Slope Drains - Pipe Slope Drains shall be installed on slopes where shown in the Construction Documents. • Flow Diversion - Flow of runoff from off-site and undisturbed on-site areas shall be diverted around disturbed areas where shown in the Construction Documents. • Velocity Dissipation - Velocity Dissipation/ Rock Channel Protection shall be installed at pipe outlets where shown in the Construction Documents. d. Sediment Control Practices Structural practices shall be used to store runoff and allow sediments to settle. i. Timing – Sediment Basins and Perimeter Sediment Controls shall be implemented prior to the onset of Construction. Remaining Sediment Controls shall be installed as soon as practical. All controls shall be functional until all land disturbances are completed and disturbed areas are stabilized. ii. Sediment Settling Pond - Sediment Settling Ponds shall be installed at the onset of construction. Following site stabilization, accumulated sediment shall be removed and stabilized on-site. iii. Silt Fencing - Silt fencing shall be installed where shown in the Construction Documents. iv. Inlet Protection - Inlet Protection shall be installed where shown in the Construction Documents. v. Surface Water Protection – Erosion Control Matting and Silt Fencing shall be installed where shown in the Construction Documents to protect the existing stream to the east of the site. Inspect and maintain as detailed below. vi. Modifying controls – The contractor shall replace or modify practices as needed to control sediment and prevent it from leaving the site. A log for documenting amendments to the SWPPP is included in Appendix F. e. Post-Construction Storm Water Practices The existing regional detention basin was previously constructed per the IDEM general permit at the time of construction. No changes to the existing detention pond are planned as part of the construction operations. Erosion and Sediment Control practices shall be maintained to prevent sediment from accumulating in the detention basin. f. Surface Water Protection Silt Fencing shall be installed where shown in the Construction Documents. Inspect and maintain as detailed below. g. Other Controls i. Non-Sediment Pollutant Controls – The contractor shall take precautions to ensure that no solid or liquid wastes, chemicals, trash, sanitary wastes, vehicle fluids, building materials, detergents, landscaping materials/pesticides/fertilizers, etc. are discharged in storm water runoff. Precautions shall include: (1) Designating on-site areas for the storage and disposal of solid, sanitary, and/or toxic wastes. Said areas shall be properly maintained, and located such that pollutants will not be washed or drained onto off-site lands or the on-site stormwater collection or detention system. (2) Establishing dikes or berms, where shown on the plans or otherwise needed, to divert and capture runoff from solid, sanitary, and/or toxic waste storage areas. (3) Properly disposing of used or empty containers of toxic materials in a timely manner. Properly disposing of trash and sanitary wastes in a timely manner. All trash and construction waste materials shall be collected and stored in a securely lidded metal dumpster meeting all state and local Solid Waste Management Regulations. All trash and construction waste from the site shall be disposed in the dumpster. No waste materials shall be buried or burned on site. All hazardous wastes shall be disposed of in a manner specified by state or local regulations, or by the manufacturer. No hazardous wastes shall be discharged in storm water runoff. The contractor shall install Concrete Washout Areas where shown in the Construction Documents. Under no circumstances shall concrete truck wash out be discharged into a storm sewer, drainage channel, or surface water. Leaks or Spills In the event of a leak or spill of hazardous materials, the Contractor shall: (1) Cease work operations in the area and immediately contain the spill. (2) Properly dispose of hazardous materials off-site, including any contaminated soil. (3) Repair or replace damaged storage containers. (4) Report the leak or spill and response in Inspection Logs. The contractor must contact IDEM Emergency Spill Hotline (1-888-233-7745), the local fire department, and the local Emergency Planning Agency for any spills as defined in 327 IAC 2-6.1. Report all spills in the Inspection Logs. ii. Off-site Traffic - Stabilized construction entrances shall be constructed as shown on the plan. Construction vehicles shall be washed to remove mud and pollutants and prior to exiting the site. Wash water shall be free of detergents, and directed to a sediment settling pond or alternative treatment. Any paved roads adjacent to the site entrance shall be swept, as necessary, to remove any excess mud, dirt, or rock tracked from the site. Dump trucks hauling material to or from the construction site shall be covered with a tarpaulin. iii. Compliance with Other Requirements - This plan is consistent with state and/or local waste disposal requirements and sanitary sewer or septic system regulations, including provisions prohibiting waste disposal by open burning. iv. Trench and Ground Water Control – In the event trench or other dewatering activities are required, effluent should be drained to a sediment basin, or through a dewatering bag as shown in the plans. v. Contaminated Sediment – In the event contaminated soils are encountered within the limits of construction, they shall be disposed of per 327 IAC 2-6.1. h. Maintenance During Construction Operations: All Erosion and Sediment Control practices shall be installed as shown on the Construction Documents and in accordance with the Implementation Schedule and timing defined in this SWPPP. Practices shall be inspected as outlined below and maintained and repaired as needed to ensure continued performance of their intended function. Maintenance activities shall be continuous until permanent stabilization is achieved. Post Construction: The existing detention pond shall be maintained in accordance with any existing Operation and Maintenance Plans provided at the time of construction. i. Inspections Erosion and Sediment control practices shall be inspected by “qualified inspection personnel” to ensure that the practices are adequate, properly installed, and functional. Control practices shall be inspected at least once every seven calendar days, or within one day of any storm event of 0.5 inches or greater in a 24-hour period. • If the inspection reveals that a control practice is in need of repair or maintenance, it shall be repaired within three days of the inspection, except for Sediment Settling Ponds, which must be repaired within 10 days. • If the inspection reveals that a control practice fails to perform its intended function, or that another control practice is required, the SWPPP shall be amended and the new control practice installed within 10 days of the inspection. • If the inspection reveals that a control practice was not installed, the practice shall be installed per the Construction Plans within 10 days of the inspection. • If the inspection reveals that a control practice is not needed, the report shall include a record as to why the practice is not needed. Inspection frequency can be reduced to once per month for dormant sites if the entire site has been temporarily stabilized, or if runoff is unlikely for an extended period of time due to weather conditions. Once an area has achieved final stabilization, that area shall be noted on the SWPPP, and no further inspections are needed. Erosion and Sediment Control Inspection Reports are included in Appendix E. An inspection report shall be completed and signed following each inspection. Logs documenting corrective actions and amendments to the SWPPP are included in Appendix E and Appendix F. Notice of Termination The Contractor shall submit a Notice of Termination (NOT) when discharges of stormwater associated with the construction activity and land disturbance have ceased.
