'On 3/30/06. I called Health Dept. to
speak with Barry McNulty. He was
out. I then spoke with Amanda and
asked her if they needed to sign off
on demolition permits for 4 buildings
in the City limits of Carmel.
Amanda spoke with her supervisor and
said no, they didn't need to sign
off. On 3/31106. I dropped off 4
applications to Morris Hensley. He
will get back with me at the end of
next week.'
--VERBAL NOTICE FROM J.Blanchard on
4/18/06 that Morris Hensley has
reviewed and released these
applications. Blanchard confirmed
this by a call to Hensley, and then
gave permit applications to front
office for release.
This permit is valid only if construction commences within one (I) year of the date of issuance of the State Commercial Design Release.
All construction must be completed (CIO issued) within two (2) years of the issuance date.

land or structures requested by this application will comply \vith, and conform to, all applicable laws of the State of Indiana, and the "Zoning
Ordinance of Carmel Indiana -1993" (Z~289) and amendments, adopted under authority of I.C 36-7 et seq, General Assembly of the State of
Indiana, and all Acts amendatory thereto. I further certify that only kitchen, bath, and floor drains are connected to the sanitary sewer. 1
further certify that the construction will not be used or occupied until a Certificate of Occup,mcy has been issued by the Department of
Community Services, Carmel, Indiana.
