Michigan Rd. #102 Carmel, IN 46032 RE: MICHIGAN ROAD BUILDINGS PERMIT STATUS AND REQUIREMENTS Dear Mr. Carlson, After assessment of permit and inspection records, and a site inspection of the 9830,9840, and 9850 N. Michigan Road buildings, we have established the following status for each open permit, and require the following steps: 9830 N. MICHIGAN ROAD 1. Shell building permit # 00l455B still requires an approved Final Site inspection by the Urban Forester of the City of Carmel. 2. Verification of Carmel Fire Department approval for all tenant construction is required. 3. Final re~inspection for permit #0205122.B~ for Wild Birds, was approved by J eft Kendall. When item #2 above is complete, the full C/O on this permit will be issued. 4. Permit #030068B, for all areas of the building except suite 101, will be closed, with notes reflecting that it was updated and completed under permit #03100190. 5. Permit #03100190, for mezzanines in the building and interior finish of the suites except for #101, was inspected by Jeff Kendall, and this completes final requirements. However, one late fee for covering/insulating/dry~walling prior to an approved Rough~in will be assessed, and one late fee will be assessed for occupying prior to the Final inspection. The total in late fees due on this permit is therefore: $2,000.00. 9840 N. MICHIGAN ROAD 1. Sweet &: Savory, at suite #102, should have applied for a building permit for their tenant finish. FURTHER, there are rear exiting hindrance issues, which must be addressed; as there is an illegal locking device on the rear door. a. Late fees on the permit will be assessed at the time of issuance. b. Inspections will be required. Late fees on inspections will be assessed at that time. 2. Studio 421, at suite #103, also requires a building permit and State design release. FURTHER, a pool/spa permit may be required for the spa located at this site. . a. Late fees on the permit will be assessed at the time of issuance. b. Inspections will be required. Late fees on inspections will be assessed at that time. 9850 N. MICHIGAN ROAD 1. Verification of Carmel Fire Department approval for all tenant suites #'s 101, 102, and 103 is required. 2. Permit #03100068, for Suites 101 (U,Build It), 102 (Indiana Real Estate), &: 103 (a white,box finish), was approved by the inspection made by Jeff Kendall, for completion of the Final. Once Carmel Fire Department verification is received, the full Certificate of Occupancy will be issued. 3. Permit #04070156, for suites 102 and 103-which are the Just Trucks space, was approved by the inspection made by Jeff Kendall, for completion of the Final. Once Carmel Fire Department verification is received, the full Certificate of Occupancy will be issued. Your company's status with the City of Carmel should be considered expired and not in compliance until the following steps occur to regain compliance: 1. Obtain all required building permits; and complete all requirements of those permits. 2. Complete the requiredinspections/re-mspections required inspections by the Building &:Code Enforcement office by March 14, 2005 3. Complete the required inspections/re'inspections required by the Carmel Fire Department by March 14,2005. 4. Complete the required final Site inspection(s) by the Urban Forester by March 14,2005. 5. Pay all outstanding Late fees and/or re,inspection fees for each project, as they are assessed. The range of fees is as follows: Chapter 29.06.07 Late Fees for Permits; Carmel I Clay Zoning Ordinance; as amended per Z-419-03: Late fees shall be assessed for construction prior to obtaining required permits. 3. The construction plans provided to us yesterday at a quasi-pre submittal meeting did not match those submitted to the third floor in 1999. Reed indicates he has gotten approval for his ADLS application on the storage facilities at a more recent date. (I would research that a bit further, but someone has had the Access files for PC and BZA open all day today!) Laurence, we had Reed's representative leave a copy of the plans down here--could we ask that you bring the approved plans down and compare them with what we have, to be sure we're going to issue a permit on correct plans? We agreed to keep one set of plans and have a planning & zoning staff person compare the approved DP with what is being submitted. Also, landscape plans need to be submitted to our office. Lastly, the Affidavit has not been notarized. Mr. Sheehan left the meeting being apprised of that appear incomplete or not in compliance; we recommended he contact Reed Carlson for possible clarification. We made a commitment to have the plans checked by Planning & Zoning. The current proposal shows the constnlction of new buildings and the insta.1lat:i.on of a detention area in the Northwest comer of the site. Because the proposed improvements will drain to the US 421 right-oi-way, which then flows South, no regulated drains are affected. Therefore, no permits are required from this office.
