Please review this plat and have the director sign it at your earliest convenience. Brenwick's desire is to
have this plat signed by the commissioners at the August 13th meeting, which requires us to have it

RE: Village of West Clay - Replat of Section 7501-A (2nd review letter)
Dear Keith:
This letter is in response to your revised Secondary Plat filing for Village of West Clay - Replat of Section 750 I-A.
The revised plans were received by our office under a cover latter dated June 25, 2001. The following issues must
be resolved prior to signature of the plat by the director:
1. Revise Legend to add length of markers on legend, as required under Section 8.2 of the SCO.
2. Change monuments to concrete at boundary of section, as required under SCO 8.2.1 (2).
3. Add Key Map to Plat.
4. Have Mylar signed by property owner prior to signature by director.
Items #1, #2, and #3 are changes/corrections identified in my first review letter. Once these changes have been made
and comments addressed, please produce a mylar copy for signature by this Department. Two paper copies of the
plat must be filed with the mylar prior to recording. I will return the signed mylar to you for signature by the County
Commissioners. Once recorded, our Department needs three paper copies and a mylar of the recorded plat. Our
approval of the secondary plat (signature) will be forthcoming upon revision of the plans according to the items
listed above.

This letter serves to acknowledge receipt of a transmittal containing the plans for the Replat Sections
7501-A. After reviewing the plans. the Highway Department has the following comments:
1. The Application for Review was received, but the check was not Included. Please forward the
check immediately. No approvals will be granted until all review fees have been received.
2. No street cuts will be permitted for any utility relocations.
3. Please place the following note on the plans, Any damage to streets in areas of new alley
connections will be repaired by contractor. A representative of the HCHD will review the site
following completion of the project to determine If any repairs are required.
4. Any utility work to be performed within the right of way of Stanhope Street will need to be
pelTl'1itted. Please contact Mr. James Farrell, Permit Inspector, for the necessary forms and
information.
5. No permits will be required for the alley cuts on the existing Stanhope Street. The approved
constNctlon plans will serve as the pennlt for the cuts.

This replat previously appeared at the May 16th Technical Advisory Committee meeting. Since our
initial filing and appearance at the T AC meeting, the plat and construction plans have been revised,
adding 2 lots at the east end of the lots on the south side of Stanhope Street bringing the total number of
lots in Section 7501-A to 17. Although the lot configuration has been modified, the scope of the
proposed improvements remains virtually the same including: adding an alley along the rear of the lots
on the south side of Stanhope Street, adding sanitary and SSD laterals to the additional lots, extending
the storm sewer system in the rear of the lots to drain the proposed alley. There is one additional
improvement being proposed due to the new lot configuration, which is the relocation of the storm sewer
between lots 635B and 635C to line up with the proposed alley between the lots. Would you please
review the enclosed material and let me know ifthis revision will require us to appear before T AC.

This letter is in response to your Secondary Plat filing for Village of West Clay - Replat of Section 7501-A. The
following issues must be resolved prior to signature of the plat by the director:
1. Revise Legend to add length of markers on legend, as required under Section 8.2 of the SCO.
2. Change monuments to concrete at boundary of section, as required under SCO 8.2.1 (2).
3. Add Plan Commission Docket Number reference to Primary Plat and Original Secondary Plat.
4. Add Plan Commission Docket Number reference to PUD Ordinance Z-330.
5. Add Key Map to Plat.
6. Have Mylar signed by property owner prior to signature by director.
Please submit a revised Secondary Plat with the above-mentioned changes. Once these changes have been made and
conunents addressed, please produce a mylar copy for signature by this Department. Two paper copies of the plat
must be filed with the mylar prior to recording, and three paper copies and a mylar of the recorded plat must be
submitted following recording. Our approval of the secondary plat (signature) will be forthcoming upon revision of

We have received plans and the District's consulting engineers have completed a
preliminary review. The District has no comments relative to platting unless changes to
the sanitary sewers result. If the project involves any alterations to existing sanitary
sewers, the District must be contacted for a pre-construction meeting to coordinate
District inspection of the work. Building permits should NOT be issued without the
District's connection permit.
West Carmel Center - Block G
We have received plans and have no comments relative to platting unless changes to the
sanitary sewers result. If the project involves any alterations to existing sanitary
sewers, the District must be contacted for a pre-construction meeting to coordinate

1. The Application for Review was received, but the ct1eck was not Included. Please forward the
check immediately. No approvals will be granted until all review fees have been received.

3. No street cuts will be permitted for any utility relocations.
