Petitioner, please note the following:
1. This item will be on the April 19, 2006, agenda of the Technical Advisory Committee (TAC).
2. Mailed and Published Public Notice needs to occur no later than Friday, April 21. Published notice is
required within the Indianapolis Star.
3. The Filing Fee and Fifteen (15) Informational Packets must be delivered to Plan Commission Secretary
Ramona Hancock no later than NOON, Friday, may 5. Failure to submit Informational Packets by this
time will result in the tabling of the petition to the June 20 agenda of the Plan Commission.
4. Proof of Notice will need to be received by this Department no later than Noon, Friday, May 12. Failure
to submit Proof of Notice by this time will result in the tabling of the petition.
5. This item will appear on the May 16 agenda of the Plan Commission (under "Public Hearings").
6. This item will also appear on the June 6, agenda of the Plan Commission Special Studies Committee.

1.) Provide a copy of the official list of adjacent property owners from the Hamilton County Auditor's Office.
2.) Provide a site plan showing the location of all buildings/parking areas within the Riverview Medical Park
development (including the bank at the corner).
3.) Please submit Drainage Calculations.
4.) Please submit a Soils Map.
5.) Provide lighting fixture details for all lighting, including wall sconces.
6.) Provide signage details, dimensions, and specifications.
7.) Please label building dimensions.
8.) Showllabel mechanical equipment location & screening.
9.) Provide pavement & curbing specifications & details.
10.) On building elevations, please label the width & height dimensions.

11.) Please bring material & color samples to the Plan Commission meeting.
12.) Show/label the 146th Street road right of way.
13.) Per section 6.5.A of your PUD Z-41 0-03, provide % each building material.
14.) Per section 9.3 of Z-41 0-03, the height of pole lights shall not exceed 20-ft; you show 22-ft.
15.) Lighting plan: show foot candles along the actual north property line; they should not be greater than .3
foot candles.
