The Carmel Plan Commission will meet on the 16th day of May, 2006, at 6:00

and Architectural Design, Lighting Landscaping and Signage ("ADLS") applications for
the property located one property East ofthe southeast comer of 146th Street and Hazel
Dell Parkway. The Petitioner requests Development Plan and ADLS approval to build an
approximately 9,000 square-foot medical office building on approximately 2 acres.
The property that is the subject ofthe Development Plan and ADLS applications
is located one property East ofthe southeast comer of 146th Street and Hazel Dell
Parkway in Carmel, Indiana and is further described as Parcel 2 of the Riverview Medical
