The Crooked Stick West Homeowners Association Building Connnittee has reviewed the
. provided plans for the addition to your residence at the above address, and have found the. design
to be consistent with the appearance and design of other houses in your area, and in keeping with
the provisions of the Protective Covenants of Crooked Stick West.
The plan as presented is therefore approved.

Dear Jim:
This letter is in response to your Secondary Plat filing. The following issues must be resolved prior to signature of
the plat by the director:
1. Move title around (see attached plat).
2. Add "Subdivider" after Owner. Also add phone number.
3. Add Legend, as required under Section 8.2 of the SCO.
4. Add monuments at boundary oflot, as required under SCO 8.2.1 (2). (see attached plat)
5. Delete "(Proposed)" from building line identification.
6. Change plat approval language fro Directors signature (see attached).
7. Add a Key Map of Crooked Stick West.
8. Add Note: Add Note on face of Plat for Primary Plat, Secondary Plat and Replat do~kp.t nnmhp.Ts,
They are as follows: ??-?? PP, ??-?? SP, ??-01 SP.
9. Delete Board of Works approval.
10. Change County Commissioners Certificate (see attached).
11. Have Mylar signed by property owner prior to signature by director.
Please submit a revised Secondary Plat with the above-mentioned changes. Once these changes have been made and
comments addressed, please produce a mylar copy for signature by this Department. Two paper copies of the plat
must be f1led with the mylar prior to recording. Three paper copies and a mylar of the recorded plat must be
submitted following recording prior to the issuance of any building permits. Our approval of the secondary plat
(signature) will be forthcoming upon revision of the plans according to the items listed above.
