As the presiden1 and owner of Wrigh1's Custom Plumbing I am writing this letter
to inform all persons involved that I had no involvement with the plumbing
installations after the underslab. I completed the underslab at the Lot 12
Bridlebourne Hagman residence, but was never paid by Scott Armacost the
builder. When we found out that the home had been completed, and the
homeowners had moved in we contacted the City of Carmel. We informed the
City of Carmel that someone else had completed the plumbing, and that they had
probably used our permit, which the builder applied for when the slab work was

RE: Building Permit #0 03060214. Single Family Dwelling. 3516 Hintocks Circle.
We were contacted by the property owners at the above address, regarding concerns about the
construction of this Single Family Dwelling. A full Certificate of Occupancy, (ClO) had been
issued, however, our inspector found severallife/health/safety items after an inspection at the
home on August 4,2004. You were then contacted by rhe inspector about the items which
needed correction. You sent a reply via facsimile transmission on August 24, 2004, stating the
items would be corrected: A re-inspection was scheduled for September 10,2004, but all of the
items had not been completed.
At this time, the items need to be corrected (see attached computer copy of inspection results
and items cited as needing correction at the 8/4/04 inspection) and a re-inspection scheduled. If
we do not hear from you within 10 days from the date of this letter, or October 13, 2004,
our office may revoke the full Certificate of Occupancy which was issued on July 13, 2004,
and we will not issue any further permits to you, your firm, or any individual representing
your firm until all permits, inspections, and Certificates of Occupancy have been issued and
all fees paid.

I have reviewed your list of items to be addressed dated August 8, 2004 and have listed
below what G. S. Armacost Builders LLC, will do or will not due.
I. Tom Harmon is going to walk exterior with you to do repairs as needed to brick
and remove foreign objects. Featherston concrete has removed as many spatters
as possible from brick and limestone.
2. Limestone will be cleaned and repaired as needed by Tom Harmon.
3. Front door panels will be turned around to fit properly. Holes will be filled as
needed and paint win be touched up as needed. Neither door will be replaced.
4. A Anderson window rep will be out to repair windows as needed when parts are
received. This can be scheduled through me or you directly with Anderson.
5. Wood will be painted at front porch.
6. Sump pump hole will only be filled it you sign a release for any flooding that may
happen due to removal of pit. That pit serves the same function as the pond
behind your house. It gives water a place to accumulate and slowly drains to your
internal sump pits.
7. Drivit cut outs have been scheduled by you per our last conversation.
8. Theater room ceiling has already been addressed and per our conversation was
satisfactory.
We will measure carpet gaps and compare to BAGI standards. At that time if
they do not meet BAGI standard we will repair.
9. Crack in foundation is normal, we will find what other source is causing water
leakage and repair. It is more likely the condensation from the furnace and not
the foundation.
10. Install damper lock in fireplace.
11. Kitchen hardwoods will not be repaired. The other floor issues will be dealt with
using BAGI standards. (NOTE: Contract item 12 under Terms and
Conditions states "Owners agree that the direction and supervision ofthe
subcontractors and suppliers rests exclusively with Contractor, and Owners
agree not to issue any instructions or to otherwise interfere with
subcontractors and suppliers. Owners further agree not to negotiate for
additional work with any subcontractor or supplier without Contractors written
consent. Any such additional work allowed shall not interfere with Contractors
completion ofthe Work. Contractor shall not be liable for costs or charges
incurred resulting from Owner directing subcontractors or suppliers to furnish
additional work, without Contractor's express written consent.) G. S. Armacost

Sllilday. April Hagman demanded floors not be finished lliltil painting was done

13. Caulk on baseboards will have to be marked before repairs will be done. Sink
will be caulked.
14. Green paper will be removed, nail holes wiIl be fiIled. Door to garage wi\l not
receive a lock.
15. Hole in cabinet will have a cover instaIled. Master tub handles will be replaced,
tub will be caulked, bath panel does have an access and wiIl not be redone, loose
piece on toilet wiIl be tightened, center piece on cormter will not be replace (We
cannot assume liability for scratch when you bad another vender in bathroom, we
did pay for the repair and that is all we will do). Trim issue wiIl need to be
addressed on site. Glass piece in shower is what April ordered and will not be
addressed.
16. Lights will be adjusted with the caulking.
17. Plumbing will be repaired and exterior leak will be repaired.
18. Doors wiIl be adjusted with the locks. Gaps will be referred to BAGI standard.
19. Cabinet doors wiIl need to be listed on paper before we can proceed, all touchups
will need to be listed. BAGl standards will apply on all repairs.
20. We tried to complete and April interfered with work. If our repairs are to be done
timely and in a professional manner we cannot have any interference from April.
Location of outlets will remain the same, no outlets wiIl be removed.
21. Grout will be repaired.
22. No reimbursements will be made.
23. Refer to number 22 above.
24. Instructions to HV AC rmits are sitting on top of unit in basement fireplace and
water shut off instructions will be given on site.
25. Garage doors are per print. If Bruce Berry says minimum distance is 8' wide and
7' -4" high he needs to put that on print. See exhibit A. Print calls for 8' -0" x 7'-
0" which is the door itself. When you add trim you loose approximately 2" on
top and side.

PLEASE NOTE: Installation of building sewer shall be per the specifications of the Clay Township
Regional Waste District (see reverse) and any conditions noted below. All installations shall be inspected by District
personnel during "open trench" phase and before backfilling with sand or stone to six inches above the pipe. NO
footing or foundation drains, or other sources of ground or stormwater, shall be permitted to e'nter the District's
sanitary sewer system. The District will assume no liabilitv for drains which are below the qrade hivel of the
nearest downstream manhole nor for laterals which are extended beneath drivewavs or sidewalks. The
permit holder (property owner, developer or builder) will be responsible for damages to the District's sewer system.
This includes damages to manholes, castings, manhole lids and the like; caused by construction activity on the
building site which is the subject of this permit.

Inspections by the District are MANDA TORY and shall be arranged by contacting the District's office at 844-9200
24 hours in advance. All new construction will be placed on billing six months after connection has been made or
when water is connect'1d, whichever comes first.
