Dear Mr. Griffin:
I want to thank you again for meeting with me on Friday, March 3rd 2006, at your office. The information and
insight you were willing to provide was very helpful in preparing the ADLS Amendment submission for the USTA
Midwest Section building sign resubmission.
Please find the following items enclosed with this letter as a submission packet. Please note I have provided 15
copies of the submission packet per your instructions.
. One site plan showing the building location relative to the streets and the sign locations on the building.
. One building sign rendering by Sign-By-Design for the UST A sign.
. One building sign rendering by Sign-By-Design for the First Merchants sign.
. One building sign rendering by Sign-By-Design for the Raymond James sign.
. One copy of the original UST A PUD illustrating the allowance of 3 signs.
Also enclosed please find two checks. The original check was inadvertently short $1.50 so the total of both checks
is equal to $427.50 which is equal to the submission fee of $267.00 plus $160.50 the cost of three signs at $53.50
a sign.
