HomeMy WebLinkAboutAddendum #1 to drawings & specs esr42 891-021 June 6, 1990 ADDENDUM NUMBER ONE to the drawings and specifications prepared by the Everett I. Brown Company for the ADDITION TO AND RENOVATION OF CARMEL HIGH SCHOOL- PHASE IA for Carmel Clay Schools, Cannel, Indiana. All Contractors bidding on this project shall read all of the items covered below and shall comply with all of the requirements as set forth, including any necessary refinements or additions generated by this Addendum and required by the intent of the original contract documents. All Contractors shall acknowledge on their bid form that they have received this Addendum and included the appropriate content of same within their bid proposal. For the convenience of the bidders, bid categories possibly affected in price and/or construction by the various items in this Addendum are listed at the beginning of each item. 1. ALL CONTRACTS: Specification Section 00100-6 - Instructions to Bidders: A. Add reference to Alternate #1 in all contracts, 1-8. Provide separate bid amount line for the alternate. • 2. ALL CONTRACTS: Specification Section 01005 - Administrative Provisions: A. Insert Section 1.09. Renumber remaining sections. "1.09 Alternates A. Alternates quoted on bid forms will be exercised as Owner option. Accepted alternates will be listed in Owner- Contractor Agreement. B. Schedule of Alternates. 1. Alternate #1: All payments approved and due will be extended to the respective contractors after the week of December 17, 1990." 3. ALL CONTRACTS: Specification Section 01027 - Payment Applications and Payment: A. Revise Section 1.04A as follows: "A. Submit applications monthly beginning September, 1990, based on the cost of labor and materials incorporated in the work and for the value of material suitably stored at the site." Page#1 891-021 p 4. CONTRACT 1: Specification Section 02218 - Landscape Grading: A. Add to paragraph 3.01: E. Chisel plow to a depth of 8" all subgrade other than paving subgrade immediately preceding the placement of topsoil at the site north of 136th Street. 5. CONTRACT 1: Specification Section 02226 - Site Excavation,Filling and Compacting: A. All topsoil stripped from the area of work north of 136th Street is to be stockpiled, where directed by the Architect, on the site north of 136th 006oleT16 Street for future use in this phase of work. Topsoil remaining at the Q completion of Phase IA is to remain stockpiled and the property of the Co "N�'�� Owner. re' l�13 B. Change Paragraph 3.02 J to read: At the site south of Main Street, bow" excess topsoil and subsoil shall be removed from the site and stockpiled, where directed by the Architect, on the High School site north of Main Street in the vicinity of the athletic fields just south of Cool Creek, where directed by the Architect. C. Unit price bids are required for quantities of over excavation and compacted approved backfill replacement of unsuitable soil as determined by the Architect. Prices shall be stated in terms of $/cubic yard. 6. CONTRACT 1: Sheet#C-4: A. Change Plan Note #16 to read: Finish grade, ready for seed, the area bound by work limit designators A, B, C, D, E, F, G, H, I. On Plan, add work limit designator G at the northwest end of existing fence shown at the upper right hand corner of the sheet and add work limit designator H to the southeast end of existing fence shown at the upper right hand corner of the sheet. 7. CONTRACT 1: Sheet vicerzositer(r A. Remove all trees and brush required for the installation of the new storm sewer west of 4th Avenue Southeast shown on Sheet #M-6. 8. CONTRACT 2: Specification Section 02511 - Crushed Stone Paving: A. This specification section applies to those areas on plan in which crushed stone paving is denoted. -.ram° 7 Page#2 891-021 9. CONTRACT 2: Specification Section 02570 - Exterior Concrete: A. Change paragraph 2.10 A 1 to read" Quality Industries, Inc., #5406 portable soccer goals with #5413 soccer goal net by Jennings of Indiana, Inc. (317-875-9490). B. Delete Paragraph 2.10 A 2. 10. CONTRACT 2: All Civil Sheets: A. Walks that are poche' on plan are part of Phase IA. Walks that are not poche' on plan are not part of Phase IA. 11. CONTRACT 2: Sheet#C-1: A. Section 21: Delete the artificial sideline turf. 12. CONTRACT 2: Sheet#C-2: A. Change Reference Note #3 in the table in Section 1 to have an inlet- outlet size from 30" to 24" and the"A" dimension from 6'-10" to 5'-8". 13. CONTRACT 2: Sheet#C-6: A. Provide and install two 6" standard galvanized pipe 8'-0" long filled with concrete and set in 12" round x 4' deep concrete foundation. Location shall be at the northeast corner and southeast corner of the south parking lighting panel pad shown on Sheet #E-1. 14. CONTRACT 3: Specification Section 02231 - Granular Base Course: A. Unit price bids are required for quantities of ground stabilization fabric 2002 manufactured by Amoco and distributed by Midwest Construction Products Corporation (317-241-1042). Prices shall be stated in terms of $/square yard and include installation. 15. CONTRACT 3: Specification Section 02513 - Asphaltic Concrete Paving: A. Delete all references to tack coating. B. Delete all references to All Weather Surface. 16. CONTRACT 3: All Civil Sheets: A. Bituminous paving poche'on plan is part of Phase IA. Page#3 891-021 17. CONTRACT 3: Sheet#C-3: A. Change the radius dimension on the inside of the track from 130' to 102.93'. Change the track width dimension from 32.5' to 33.33'. B. Provide and install 5' wide x 10' long bituminous paving adjacent to track at finish line marker post per Section 2 on Sheet #C-1. 18. CONTRACT 3: Sheet#C-6: A. Plan Note #7 is part of Contract #3. B. Repair utility cut in existing bituminous paving 8'wide x 60' long with 6" #53 crushed stone, 2-1/2" bituminous binder, and 1-1/2" bituminous surface course on a prepared subgrade. Location of utility cut is in parking area of apartment complex west of 4th Avenue Southeast for new storm sewer. 19. CONTRACT 4: Sheet#M-1: A. To "Drainage Structure Schedule"make the following changes: a.) DS#7 invert in should read W 805.40'. b.) DS#50 invert in should read N 799.52', west and northeast are • okay and depth 6.20'. c.) DS#51 invert in should read NE 798.84' and depth 6.41'. d.) DS#60 add invert in N 807.38'. e.) DS#68 invert in NE 803.22' should read NW 803.22'. B. To "Storm Manhole Schedule" make the following change: a.) MH "1" invert in W 802.77'. 20. CONTRACT 4: Sheet#M-2: A. Provide 36' of 21" ST (Reinforced concrete pipe) at .83% slope, at the outlet of the water detention area (between metal end section in el.797.30' and metal end section in el.797.00') instead of what is shown. Also change the "Detail of Temporary Silt Control Stand Pipe" to reflect the same change: 21"ST(permanent). 21. CONTRACT 4: Sheets #M-2,#M-3,#M-4, M-5 A. Change the underdrain system on the three football fields and two soccer fields to Stripdrain 80 by Contech Construction Products, Inc. (317-842-7766) or Warren's TerraFlow by Warren's Turf Professionals, Suisun City, California. See drawings in this Addendum for layout and installation detail. Provide and install premolded fittings. Page#4 4 891-021 22. CONTRACT 4: Sheet#M-3: A. Change the slope of the 117' of 12" ST between DS#21 and DS#14 to .22%. 23. CONTRACT 4: Sheet#M-4: A. Provide 12' of 8" SAN and install at a point south of MH "A", exact location to be determined with the placement of a grease trap, to be C.� � / determined under a separate contract. Refer to Note #9 for IT' �"" termination requirements. B. Change the slope of the 144' of 24" ST between DS#10 and DS#11 to .46%. C. Change the slope of the 154' of 12" ST between DS#22 and DS#24 to .30%. 24. CONTRACT 4: Sheets #M-4 & #M-5: A. 135' of 12" ST shown from DS#54 on Sheet #M-5 to DS#55 on Sheet #M-4 should refer to Plan Note #7 on Sheet #M-4. 25. CONTRACT 4: Sheet#M-6: A. The ST between DS#9-S and MH#3-S should be: 80' of 48' ST at .25% slope, not what is shown. B. Add Plan Note #11: "11. All ST piping considered to be a regulated drain by the local authority having jurisdiction shall be reinforced concrete pipe. This includes the drains between DS#12-S and MH#6-S, also between MH#2-S and DS#11-S, also between DS#11-S and MH#4-S, also between MH#4-S and MH#5-S." C. Provide a 1' section of 8" ST and grout in place at the outlet of DS#11- S. Install at the invert elevation shown on the Drainage Structure Schedule. 26. CONTRACT 5: Specification Section 16530 - Site Lighting: A. In Paragraph 2.01 B, add K-W Industries as an acceptable pole manufacturer. 27. CONTRACT 5: Specification Section 16540 - Sports Lighting. A. Acceptable manufacturers for sports lighting poles are K-W Industries, Ti.��µ - �4'(� Union Metal, and Valmont. Page#5 891-021 B. Change Subparagraphs 2.03 A and 2.03 B to read as follows: "A. Provide hot dipped galvanized steel poles. Pole height shall be as follows: Pole Height "Al FB 1 FB2 120'-0" 115'-0" FB3 105'-0" FB4 115'-0" FB5 115'-0" FB6 115'-0" B. Provide a service platform with crossarms on each pole for the light fixtures, with minimum rack space for mounting the fixtures. Provide an additional service platform (14'-0" wide) with crossarms on poles FB3 and FB4 for mounting of fixture loudspeakers. Entire platform and crossarm construction shall be hot dipped galvanized steel." 28. CONTRACT 5: Sheet#E-1: A. On the Incandescent/HID Fixture Schedule, Type K-7 shall have a 20'- 0"pole instead of 30'-0"pole. B. On the Incandescent/HID Fixture Schedule, the following light fixture manufacturer catalog numbers are approved equals for the types shown. Add them to the schedule. Add the following to Site Plan Note #10: "VERIFY WITH THE POWER COMPANY WHAT THE POWER COMPANY WILL FURNISH AND INSTALL. PROVIDE ALL MATERIALS, LABOR, AND OTHER COSTS NOT BORNE BY THE POWER COMPANY FOR COMPLETE CONNECTION BETWEEN PANEL 'SPL' AND THE EXISTING POLE MOUNTED TRANSFORMER." 29. CONTRACT 5: Sheet#E-2: A. In Plan Notes #8, #9, #11, #12, and #14, the marking of the conduit stub shall be made using surveyor stakes. B. In Plan Notes #11, #12, and #14, change the last sentence to read as follows: "STUB CONDUIT AT THE LOCATIONS INDICATED." 30. CONTRACT 5: Sheet#E-3: A. Plan Notes #2 and #3: Delete the last two sentences of each note concerning the field drainage system and the PVC-to-conduit adapters. B. Plan Note #7: Change the last sentence to read as follows: "STUB CONDUIT AT UNIT'A'AS SHOWN." C. Plan Notes #8 and #11: Delete the last sentence of each note concerning stubbing the conduits to Electrical A-114. Stub conduits to the locations shown on the Electrical Plan of Football Field. D. Plan Note #9: Delete the words "to Electrical A-114" from this note. Install conduits as shown on the Electrical Plan of Football Field. E. Plan Note #17 and #18: The junction box dimensions shall be 12"x 8"x 6" deep instead of 8"x 8"x 6" deep. F. Change Plan Note #21 to read as follows: "ONE 1-1/2" PVC WITH PULLWIRE. STUB CONDUIT UP INTO POLE BASE TO A HEIGHT 12" ABOVE THE TOP OF THE CONCRETE BASE OF LIGHTING POLE." G. Add the following sentence to Plan Note #22: "MARK CONDUIT LOCATION WITH SURVEYOR STAKE." Page#7 891-021 H. Poles FB1, FB2, FB5 & FB6 Detail: Pole FB1 shall be 120 feet in height instead of 100 feet. Poles FB2, FB5 and FB6 shall be 115 feet in height instead of 100 feet. Pole FB4 ikann shall be 115 feet in height instead of 100 feet. J. Pole Base Detail: 1. The underground conduit shall be 3'-6" below the final finished grade instead of 3'-0". 2. The top of the concrete pole base shall be 4" above the final finished grade. 31. CONTRACT 5: Sheet#E-3: A. Electrical Plan of Football Field: 1. All conduits installed below the playing field area inside the boundary of the running track shall be installed below the level of the field drainage system piping, with a minimum conduit installation depth of 3'-6"below the final finished grade. All other underground conduits shall be installed 3'-6" below the final finished grade. 2. In Unit "A" at Electrical Room A-114, change the conduit shown stubbed under this room to be stubbed 5'-0" west of the west wall of this room and add Plan Note #22 to the stubbed end point of conduit. 3. All conduits shown stubbed, capped and marked in the area between Unit "A" and the west side of the running track shall be stubbed 5'-0" east of the east foundation wall of Unit"A". 4. At the west side of the running track adjacent to the south 20- yard line of the football field: a. Add Plan Note #19 to the conduit running between the two junction boxes noted with Plan Note #14. b. Add Plan Note #20 to the conduit connected to the junction box noted with Plan Note #15. Page#8 4 891-021 5. Near the 50-yard line of the football field, add Plan Note #19 to the conduit running between the two junction boxes at each sideline that are noted with Plan Note #14. 6. In Unit "B" at Electrical Room B-101, change the two conduits shown stubbed under this room to be stubbed 5'-0" west of the west foundation wall of this room and add Plan Note #22 to the stubbed end points of the conduits. 7. At each of the junction boxes noted with Plan Note #10 at the north and south ends of the football field, provide one conduit of the same type and size that is shown connected to the box stubbed out 5'-0" from the box and add Plan Note #22 to this conduit stub out. 8. At the two junction boxes in the southeast corner of the running track: a. Stub one 1" PVC conduit out 5'-0" from the box identified by Plan Note #14 to the northeast parallel to the track. b. Stub two 1" PVC conduits out 5'-0" from the box identified by Plan Note #16 to the northeast parallel to the track. 9. At Unit "B location,provide five 2" diameter steel conduit sleeves, each 18" in length, on each of the four underground conduits routed through this area (one 1-1/2" PVC and three 1-1/4" PVC). These sleeves are to be installed in the foundation walls of Unit "B",which will be constructed under the future Phase IB work. 10. All pullwires shall be galvanized steel to provide a means of locating underground PVC conduits in the future. 32. CONTRACT 6: Specification Section 02936 - Seeding: A. Under Part 1 - General 1.11 Sequencing and Scheduling, change Subparagraph "B"to read as follows: "B. Coordinate the work of this section with the installation of the underground irrigation system, subdrainage system, utilities, and remainder of the sitework." B. Under Part 2 - Products 2.01 Amendments, change Subparagraph "C" to read as follows: "C. Granular sulphur: Commercial grade." Page#9 891-021 • C. Under Part 2- Products 2.01 Amendments, add the following: "D. Peat Moss: Sphagnum Peat Moss as specified in ASTM D2607. Provide peat moss with PH in the range of 5.9 to 7.0 according to ASTM D2976." "E. Tackifying Agent: Commercial grade." D. Under Part 3 - Execution 3.01 Seed Bed Preparation, change subparagraph"A" as follows" "A. After landscape grading, before installation of subdrainage system (ball fields only), scarify soil to 6" depth. Till to a homogeneous blend. (Subdrainage system is a part of Contract #4. Coordinate installation with the Architect.)" E. Under Part 3 - Execution 3.02 Fertilizing and Add Amendments, change Subparagraph"A" to read as follows: "A. Apply fertilizer and amendments after the installation of the subdrainage system (subdrainage system is a part of Contract #4). Apply fertilizer and amendments as recommended by the soil testing laboratory. In the ball field areas apply no less than 2" of peat moss, more as required by the soil test. Fertilize and amend only the areas to be seeded within 24 hours." F. Under Part 3 - Execution 3.03 Seeding, change the following Subparagraphs to read as follows: "B. 1. In the fall, broadcast and incorporate the seed specified using a Brillion seeder in the following locations: Football Fields Seed Mix A Slopes 10% and greater Winter Ryegrass Other seeding methods must be approved by the Architect. 2. In the spring seed as shown on the following tables: Football fields Hydroseed All other areas Hydroseed or Broadcast and Incorporate 3. Till all winter rye under prior to seeding in the spring. Eliminate rills and depressions. Seed immediately as weather permits with seed mix'B'. Page#10 891-021 "C. Mulch all seeded areas. Crimp the straw into the soil surface with a roller (straw rate: 4000 lbs/acre). Spray mulch with the tackifying agent." G. Under Part 3 - Execution 3.04 Hydroseeding, change the following Subparagraphs to read as follows: "A. Hydroseed with seed mix 'B' in the spring only. Apply the slurry evenly in two intersecting directions with a hydraulic seeder. Apply 500 pounds of slurry/acre with the seed mix." "B. Mulch may be applied with a hydroseeder in the fall as an alternate to crimping straw. In the spring mulch only with a hydroseeder on the ball fields. Mulch with 1500 pounds/acre immediately following seeding. Apply tackifying agent." H. Under Part 3 - Execution 3.10 Grass Seed Mix, change the following Subparagraph to read: "A. Seed Mix 'A' (seed in the fall): Turf type tall fescue and annual ryegrass. Blend these varieties: 28% Mustang 27% Bonanza 27% Pacer • 18% Annual Ryegrass Seed at a rate of 12 pounds/1000 square feet." "B. Seed Mix 'B' (seed in the spring): Turf type tall fescue. Blend these varieties: 33% Mustang 33% Bonanza 34% Arid Seed at a rate of 8 pounds/1000 square feet." "C. Winter rye-2 bushels/acre. 33. CONTRACT 6: Specification Section 02938- Sodding. A. Under Part 2 - Products 2.01 Amendments, change Subparagraph "C" to read as follows: "C. Granular sulphur: Commercial grade." Page#11 891-021 34. CONTRACT 6: Specification Section 02950 - Trees, Plants,and Ground Covers: A. Under Part 1 - General 1.08 Testing, add Subparagraph "C" as follows: "C. Test topsoil for herbicide carryover and micronutrient deficiencies." B. Under Part 1 - General 1.09 Submittals, add to Subparagraph "A" the following: "A. ... adjusting ph if required. Include test results for micronutrient deficiencies and herbicide carryover. Submit test results to the Architect prior to any work as a part of this Contract." C. Under Part 2 - Products 2.01 Nurseries, add Subparagraph "F" as follows: "F. Indiana State Nursery,Jasper." D. Under Part 2 - Products 2.03 Amendments, delete Subparagraph "E" and insert "E" as follows: "E. Granular Sulphur: Commercial Grade.: E. Under Part 2 - Products 2.04 Mulches, add Subparagraph "D" as follows: "D. Bark Mulch 'B' - Shredded branches, twigs, bark, and trunks, composted for no less than one year. Mulch available from utility companies is acceptable. Remove any piece over 4" in any dimension." F. Under Part 2 - Products 2.05 Accessories, add Subparagraph "F" as follows: "F. Pesticides: Herbicides, Fungicides and Insecticides as required to maintain the planting in a vigorous growing condition. Secure approval of the Architect prior to pesticide application." G. Under Part 3 - Execution 3.01 Fertilizing and Adding Amendments, add Subparagraphs"C" and "D" as follows: "C. Adjust PH as required by the soils test in the backfill mix." "D. Plant bulbs with two tablespoons bone meal in planting mix per bulb. • Page#12 891-021 35. CONTRACT 6: Sheet#L-1: A. Add these items to the Notes: 5. Pyrus calleryana 'Stone Hill' may be substituted with Pyrus calleryana 'Aristocrat'. 6. Plant King Alfred Daffodils (Narcissus "King Alfred"), install topsize bulbs - 4"-8" o.c., plant a total of 600 bulbs naturalized in the locations shown on Sheet #L-2. Plant the bulbs 8" deep. 7. The quantities of plants shown on the drawings take precedence over the quantity listed in the Notes. B. The detail"Tree Planting and Guying"should be numbered 5/L-1. C. Plant Campsis radicans as shown. Plant±30' on center, install at fence posts only. 36. CONTRACT 6: Sheet#L-4: A. The Quereus borealis located south and west of the north soccer field is to be relocated 25' south and 10' east of the position shown. 37. CONTRACT 6: Sheet#L-6: A. The unlabeled tree in the planting bed on the northwest corner is a Pyrus Calleryana"Stone Hill". 38. CONTRACT 7: Sheet#C-2: A. Extend the 6' chain link fence shown 4' south of the bituminous parking area, east approximately 210', then northeast on top of berm approximately 100'to existing fence. • t54).)'' 39. CONTRACT 7: Sheet#C-4: A. Plan Note #15 applies to the new chain link fence shown on 3 sides of the discuss throw, 55' total length. Add a 3' swinging gate in the 6'chain link fence north of new bituminous parking and approximately 60' east of new Phase IB storage building. 40. CONTRACT 8: Sheet#M-7: A. On the "Irrigation Supply Piping Diagram" reverse the location of the two notes referring to irrigation supply and well pump discharge. B. On the Irrigation Control and Maintenance Building Plan, rotate the building 90 degrees in the counter clockwise direction. The overhead door should be on the south wall. The well pump discharge and the irrigation supply lines are correct in flowing from the east to the west.
