This letter serves to acknowledge receipt of a transmittal containing the revised plans for the Hayden Run
Subdivision, Section 2. After revIeWing the plans, the Highway Department has the following comments:
1. It appears that all comments have been satisfactorily addressed. Therefore, after all comments from
the Hamilton County Surveyor's OffICe have been addressed, the Highway Department request seven
sets of plans for final approval.
2. Please note that no construction may begin In Section 2 until 1111 plans have been approved, a Testing
and Inspection Agreement has been completed and a Pre-Construction meeting has been held.

The two developers. Centex Homes and Shelborne Park Ll-C, will share the cost of the installatlon of
the surfQce material and pavement markings. The placement of the new surface should not be
performed until both sides of the widening has been completed. The Highway Department will require
that the new surface be placed at the time of the construction of the second entrance.
The existing 12" CMP east of the entrance will require a concrete collar around the joint. Please
make a note on the plans stating such.
All utility poles that are to be relocated wfll need to be done prior to the stert of paving. Paving around
the poles will not be permitted.
The Stop Bar at the entrance should be Preformed TQpe, not thermoplastic. All other strlplng,
centerline and edge line. shall be thermoplastlc.
The centerline spacing of the pavement markings Should be 4H, not aD as shown on the plans.
A 4D white edge line is needed across the widening.
In note B In Section 6, Site Grading, the note should read 0.06 feet, not 0.10 feet.

applies to the construction of the above.referenced proposed sanitary sewer system to be located
along JW Brendle Drain from 126111 Street to 131" Street in Clay Township.
Please review the enclosed pennit carefully and become familiar with its tenns and
conditions. In addition, it is imperative that the applicant, consulting architect/engineer (AlE).
inspector, and contractor are aware of these tenos. conditions, and reporting and testing
requirements.
It should be noted that any person affected or aggrieved by the agency's deci5ion in
authorizing the construction of the above-referenced facility may, within fifteen (1S) days from
date of mailing. appeal by filing a request with the Office of Environmental Adjudication for an
adjudicatory hearing in accordance with Ie 4-21.5.3-7 and IC 13-15-6. The procedure for appeal
is ou~lined in more detail in Part III ofthe attached construction permit.

Dear Dennis:
This letter is in response to your Secondary Plat filing for Hayden Run, Section 1. The following issues must be
resolved prior to signature of the plat by the director:
1. Submit a copy of the Landscape Plan as approved by Scott Brewer.
2. The monuments and markers should be as required under Section 8.2 of the SCO.
a. Add concrete monuments at all comers that form the boundary of plat, as required under SCO
8.2.1 (2). (The comers between section one and two as an example.)
b. Add markers to comers of common areas, as required under SCO 8.2.4. (area at entrance as an
example.)
c. Add markers at all points where lot lines intersect curves (all lines along streets), as required under
SCO 8.2.4.
3. Provide Primary Plat (69-01 PP), next to Secondary Plat Docket Number.
4. Add source of title instrument number to page one.
5. Add ponds with bottom elevation, normal pool, and 100-YR flood elevation for pond(s) to plat.
6. Forward a copy of the fmal review letters from the following agencies:
a. County Highway
b. County Surveyor
c. Hamilton County Soil and Water
7. Change note of "Developed by" to "Owner/Subdivider".
8. Change note of "This instrument prepared by" to "Surveyor".

1. Additional right of way Wll' be needed for the Installation of the decellane. Construction plans will

Hello everyone:
Following are CTRWD comments on the agenda items effecting our sanitary sewers or service
area for the November 21 TAC meeting. Please feel free to copy and distribute this email
or attach it to your meeting minutes.
Hayden Run (Secondary Plat); Lakes at Hayden Run (Primary Plat); and The Ridge at Hayden
Run (Primary Plat)
Please have the developer or their representative contact the District to discuss these
projects as we only seem to have plans for Section 1 on 23 acres. The District has not
received the plats for the other named sections and it is unclear if Section 1 of Hayden
Run is the same as any of the listed TAC agenda items. An offsite interceptor and lift
station must be constructed by the District before any of this project will be able to
receive sanitary sewer service, so the Developer should ensure that proper coordination
with the District is achieved as soon as possible.
The Hamptons (Primary Plat)
The District has not received the plat for this development; please have the developer
provide plans for the District to review. Service to this site is also dependent on the
construction of the same offsite interceptor and lift station which must be constructed by
the District near the Hayden Run developments. The Developer should ensure that proper
coordination with the District is achieved as soon as possible.
Shelbourne Park
The District has not received the plat for this development; please have the developer
provide plans for the District to review. Since the District has not received any plans or
plats, service to this site may also dependent on the construction of the same offsite
interceptor and lift station which must be constructed by the District near the Hayden Run
and Hamptons developments. The Developer should ensure that proper coordination with the
District is achieved as soon as possible.
North Haven Replat
While this project is technically in the service area of Carmel Utilities, the District
has advised the petitioner's attorney of the location of the District's 16" force main in
96th Street. If this project has any impact on utilities in 96th Street, the District
should be notified and Holey Moley should be contacted.
Village of West Clay, Commercial/Apartment Building 3
The plans received by the District show the removal of a portion of existing 8" line and
construction of a new section of 8" sewer and 6" lateral. Approval of the sewer plans by
the District's engineering consultant will be required. Building permits should NOT be
issued unless the District has issued its connection permit.

To follow on our comments from the last two TAC meetings, the District has reviewed the
. i plans and notes that the sewer connection is proposed through core drilling a manhole.
District specifications need to be added to the plans and special permit conditions will
apply for the core drill. The District still has not received any revised plans as yet.
Building permits should NOT be issued unless the District has issued its connection
permit.
As a final general comment, the last few TAC meetings have included a number of projects
where the District has not been provided any plans to review in advance. Please remind all
TAC applicants to provide plans directly to the District at our main office address at:
10701 N. College Avenue, Suite A, Indianapolis, IN 46280-1098.

This letter serves to acknowledge receipt of a transmittal containing the plans for the Hayden Run
Subdivision. After reviewing the plans, the Highway Department has the following comments:
1. An Appllcatlon for Project Review needs to be completed and the appropriate review fees for a
Secondary Plat and Construction Plan paid, before any approvals will be granted from this office.

2. Please note that decorative signs, sprinkler systems, trees, landscaping mounds, light poles or
other such amenities are not permitted in the right of way.
3. A 50' triangular right of way cut Is needed at the entrance intersection.
4. Before any approvals are granted for the Secondary Plat, an engineers Estimate broken into the
following categories will need to be submltted, along with a Perfonnance Bond or LeUer of Credit
in the amount of 100% of the estimate:
. Stone Base
. HAC Binder
· HAC Surface
. Concrete Curbs
. Street Name and Regulatory Signs

5. Please include the 25 MPH design speed on the cover sheet.
6. Please dimension the accel/decellanes at the entrance. The decellane should consist of a 100'
taper, 150' lane and be 12' in width. The eccel taper should be 100'.
7. Please show the sub surface drain under the curbs on the Site Development Plan.
8. All offslte utility work wll need to be placed beyond the future right of way. Road cuts will not be
permitted for utilities to cross 13181 Street. Pu8hlbores must be used.
9. The east bound travel lane of 13181 Street will need to be widened 3' across the entire parcel. A
three foot stone shoulder Is required behind the wldenlna. Please correct section A-A on sheet 10.
10. A passing blister is required along the north side of 131 Street at the entrance. Right of way will
need to be acquired for the installation of the blister. Additional right of way wUl also be needed for
the installation of the decellane. Construction plans will not be approved until the necessary right
of way Is acquired.
11. Please note that decorative signs, sprinkler systems. trees. landscaping mounds, light poles or
other such amenities are not permiUed in the right of way.
12. Chairback curb with reverse slope must be used around the median at the entrance.
13. On the east side of the entrance, the curb must be allow for all of the storm water to enter the
subdivision. The water from the streets cannot enter the roadside drainage.
14. Is a guard house or entrance building planned for the median at the entrance?
15. Please include the SSD on the entrance detail on Sheet 10.
16. On the Traffic Control Plan, a 25 MPH sign is needed on Tantara Boulevard. The end of road
markers and the necessary standard drawing for the end of road markers are also needed.

20. Please include all the necessary written specifications.
21. Elevation shots are needed for the road side drainage along 131. Street
22. Please verify that the storm water pipe between structures 107-108 will not Interfere with the SSD.
Blind taps of the storm sewer pipe will not be permitted.
23. Flow lines are needed on the Site Development plans.

Hello Gentleman....
I know this is no shock to either one of you but I will not be able to attend this months T AC meeting due to prior
commitment and responsibility to work the radio in Dispatch that day. We will be short on manpower that day due to
Vacations, I will have to cover the radio. But I do have a few things that I would like for either one of you to pass on for me
or even have your own input on these issues as you see fit....I welcome any and all comments from you both to the
Petitioners of the projects:
HAYDEN RUN: Please make sure they recieved my address list for Section 1. I faxed it to Dennis Olmstead.
LAKES@ HAYDEN RUN & THE RIDGE@ HAYDEN RUN: Laurence..I'm going to leave this up to you to decide ref
whether or not we should let these names stand or noUts just like Haverstick...1 know thats what Centex will probably say.
Another alternative would be to name them Hayden Lakes & Hayden Ridge...I don't have a problem with these
names..it would be better for Plice and Fire to find it if it was named this way instead of the other. Let me know what they
say.
THE HAMPTONS: If the street layout stays the same, I will need approx. 13 street names submitted to me that they want
to use.
SHELBOURNE PARK: If layout stays the same throughout developmenLI will need at least 6 street names submitted to
me; also in Section 2 the street cut off will be Lot 37 and Lot 48(This is a circle street).
NORTH HAVEN: Myself and Gary Hoyt waiting to have a meeting with them to discuss street layout in order to get names
approved for addressing purposees.
VILLAGE OF WEST CLAY:COMMERCIAUAPARTMENT BUILDING#3: Address has been assigned to
