Construction Plan - General Plan Components (Section A)
A1 - Index of the location of required plan elements in the construction plan:
The plan index should include a list of the required items in the CSGP and
where they occur in the plan. This section of A1-C6 should serve as the
index of required items. Locations of items is noted in individual
sub-sections.
A2 - A vicinity map depicting the project site location in relationship to
recognizable local landmarks, towns, and major roads:
A vicinity map is located on sheet C000 - Cover Sheet of the construction
drawings.
A3 - Narrative of the nature and purpose of the project:
The project consists of the construction of a new office building and parking
structure.
A4 - Latitude and longitude to the nearest fifteen (15) seconds:
North 86 degrees 09 minutes 33 seconds

A5 - Legal description of the project site:

There are no 100-year floodplains, floodway fringes or floodways on the site.

The project does not fall within a watershed for which there is a TMDL.

Meridian Suburban Drain, US 31 Right-of-way ditch, Vernon Asher Drain.

impaired waters and the pollutant(s) for which it is impaired:
None of the receiving waters are on the current 303(d) list.
A12 - Soils map of the predominate soil types:
Soil map is shown on this sheet. Characteristics range from poorly drained to
somewhat poorly drained. The soil used for structural fill will be compacted
as specified by the geotechnical engineer. Dewatering may be necessary in
deeper excavations. Lime stabilization may be necessary to alleviate problems
with excess moisture.
A13 - Identification and location of all known wetlands, lakes, and water
courses on or adjacent to the project site (construction plan, existing site
layout):
A wetland previously existed on the site but was mitigated as part of the
Illinois Street improvements project. The Williams Creek arm regulated drain
flows through the site although upstream water was diverted around the site
as part of the US 31 and 111th Street bridge construction project.
A14 - Identification of any other state or federal water quality permits or
authorizations that are required for construction activities:
No other state or federal permits associated with land disturbance at this site
(e.g. Construction in a Floodway from DNR, 401 Water Quality Certification
from IDEM) are known to be required.
A15 - Identification and delineation of existing cover, including natural buffers:
Sheet 23 identifies and delineates the existing cover at the site. It is currently
a vacated, abandoned residential subdivision.
A16 - Existing site topography at an interval appropriate to indicate drainage
patterns:
Existing site topography is shown on sheets C300-C304.
A17- Location(s) where run-off enters the project site:
No measurable offsite drainage areas enter the site.
A18 - Location(s) where run-off discharges from the project site prior
to land disturbance:
The stormwater exit point is shown on sheet C403.
A19 - Location of all existing structures on the project site:
Abandoned houses exist on the site and are shown on sheet C101.
A20 - Existing permanent retention or detention facilities, including
manmade wetlands, designed for the purpose of stormwater
management:
No existing permanent retention or detention facilities or manmade wetlands
exist on the site.
A21 - Locations where stormwater may be directly discharged into ground
water, such as abandoned wells, sinkholes, or karst features:
No abandoned wells, sinkholes or karst features are known to exist on the
site.
A22 - Size of the project area expressed in acres:
The project area is approximately 20 acres.
A23 - Total expected land disturbance expressed in acres:
The area of land disturbance is approximately 16 acres.
A24 - Proposed final topography:
Proposed final topography is expressed by proposed contours on sheet C300.
A25 - Locations and approximate boundaries of all disturbed areas:
Proposed construction limits are show on all plan sheets.
A26 - Locations, size, and dimensions of all stormwater drainage
system such as culverts, stormwater sewer, and conveyance channels:
Storm sewer plan information is shown on sheets C301-C304. Profiles of each
pipe are shown on sheets C601 and C602.
A27 - Locations of specific points where stormwater and non-stormwater
discharges will leave the project site:
Stormwater discharge points are shown on sheet C403.
A28 - Location of all proposed site improvements, including roads,
utilities, lot delineation and identification, proposed structures, and
common areas:
This information is shown on sheets C201-C204 and C501-C504.
A29 - Location of all on-site and off-site soil stockpiles and borrow areas:
No stockpiles or borrow areas are proposed.
A30 - Construction support activities that are expected to be part of the project:
The construction staging area is shown on sheet C403.
A31 - Location of any in-stream activities that are planned for the project
including, but not limited to, stream crossings and pump arounds:
No streams exist on the site.
Stormwater Pollution Prevention - Construction Component (Section B)
B1 - Description of the potential pollutant generating sources and
pollutants, including all potential non-stormwater discharges:
Potential pollutant sources include material and fuel storage areas, fueling
locations, leaking vehicles and equipment, etc. Sediment is one of the major
pollutants that is associated with active construction sites. In order to
minimize the discharge of these pollutants, the contractor should attempt
to not spill fuel onto the ground, maintain its vehicle and equipment so that
they do not leak. The contractor should also construct the project per the
plans so that sediment leaving the site is minimized.
B2 - Stable construction entrance locations and specifications:
The construction entrance is shown on sheets C402 and C403.
B3 - Specifications for temporary and permanent stabilization:
Temporary and permanent stabilization measures are shown on sheets C402 and
C403. Details can be found on sheet C401.
B4 - Sediment control measures for concentrated flow areas:
Sediment control measure for concentrated flow areas are shown on sheet
C403.
B5 - Sediment control measures for sheet flow areas:
Sediment control measures for sheet flow areas are shown on sheet C403.
B6 - Run-off control measures:
Run-off control measures are shown on sheet C403.
B7 - Stormwater outlet protection location and specifications:
Outlet protection locations and details are shown on sheets C401 and C403.
B8 - Grade stabilization structure locations and specifications:
Grade stabilization information (erosion control blanket) is shown on sheet C403.
B9 - Dewatering applications and management methods:
Dewatering may be necessary but locations are unknown and will depend
on the time of year when construction occurs. Dewatering will occur by a
temporary pump system brought to the site by the earthwork
subcontractor.
B10 - Measures utilized for work within waterbodies:
No streams or waterbodies exist on the site.
B11 - Maintenance guidelines for each proposed stormwater quality measure:
The Specifications on sheet C900 (Section 31 25 13 Part 3) outline
maintenance guidelines for temporary stormwater quality measures.
B12 - Planned construction sequence that describes the implementation of
stormwater quality measures in relation to land disturbance:
See General Construction Sequence on sheet C400 (this sheet).
B13 - Provisions for erosion and sediment control on individual residential
building lots regulated under the proposed project:
The project does not involve any new individual residential building lots.
B14 - Material handling and spill prevention and spill response plan
meeting the requirements in 327 IAC 2-6.1:

B15 - Material handling and storage procedures associated with construction
activity:
Material handling and storage procedures are outlined in the Material
Handling and Spill Prevention and Response Plan on sheet C400 (this sheet).
Stormwater Pollution Prevention - Post-construction Component (Section C)
C1 - Description of pollutants and their sources associated with the proposed
land use:
Common pollutants include, oil, grease, antifreeze, brake fluid, brake dust,
rubber fragments, gasoline, diesel fuel and other hydrocarbons, metals from
vehicular and other sources, grit (sediment) from wearing of the road surface
and falling or washing off of vehicles, trash (including bacteria and other
biological agents contained in the trash) from littering and other types of
improper disposal or storage, and elevated receiving water temperatures
from stormwater run-off contact with impervious surfaces.
C2 - Description of proposed post-construction stormwater measures:
Post-construction stormwater measures consist of hydrodynamic
separators and a wet pond.
C4 - Sequence describing stormwater measure implementation:
The wet pond will be constructed as part of the rough and mass grading of the
site. This is outlined in the construction sequence on sheet C400 (this sheet).
The hydrodynamic separators are part of the storm sewer system and will be
installed with the storm structures and pipes as outlined in the construction
sequence of sheet C400 (this sheet).
C5 - Maintenance guidelines for proposed post-construction stormwater
measures:
A separate Operation and Maintenance Manual has been prepared and is
available upon request.
C6 - Entity that will be responsible for operation and maintenance of
the post- construction stormwater measures:
The responsible entity is noted on this sheet.
