The City of Carmel has approved a Right of Way permit for the project and applicant shown above. Any changes to the scope of
work after permit issuance impacting city rights -of-way or dedicated easements may require additional approvals and must be
coordinated with the Engineering Department prior to performing the work. This includes, but is not limited to, pavement cuts ,
lane/street closures, path/sidewalk closures, easement encroachments, and new utility poles or aerial service lines.
CONDITIONS OF RIGHT-OF-WAY PERMIT
1.The applicant/contractor is required to post and maintain a valid License and Permit bond with the City of Carmel for the
duration of the project.
2.Indiana’s “Call Before You Dig” law (IC 8-1-26) requires everyone that digs to contact Indiana 811 at least two (2) full
working days before starting their project in order to have existing underground facilities properly identified and located.
3.The City Engineer’s office (317-571-2441 ) is to be notified before the start of a Right of Way pavement repair so that a
City Inspector can observe the placement of patch materials. A minimum of two (2) hours of notice is required. Failure
to notify may result in removing and replacing the repair.
4.Asphalt pavements that are required to be removed shall be saw cut, and then carefully removed to minimize pavement
damage. The cut must be repaired with flowable fill with 12” of concrete and 1 1/2” of surface asphalt. All seams shall
be tack coated and shall be ironed as necessary to provide a perfect joining of the patch with the surrounding pavement .
No cold mix asphalt is to be used except as temporary patches.
5.Concrete pavement shall be saw cut and then carefully removed to minimize pavement damage. The new concrete
patch shall be placed using at least 4000psi concrete for driveway aprons and sidewalks, and Class A concrete for road
cuts. The edges of the adjoining pavement shall be coated with an epoxy adhesive intended for concrete patching. The
patched area shall be properly barricaded for at least seven (7) days and the patch shall be treated with two (2) coats of
a curing compound or a curing membrane.
6.All excavations in grassed areas shall be capped with a minimum of 6” of topsoil and seeded with high quality bluegrass
seed equal to other existing grass. All excavations within 5’ of the edge of pavement shall be filled with a granular fill
material and topped with at least 6” of topsoil. All ditches disturbed by excavation shall be properly restored and
sodded.
7.Upon completion of all repairs, the Applicant shall request a final inspection from the City Engineer’s office by email .
Upon inspection if all repairs are satisfactory the Inspector shall release the permit.
8.The City of Carmel will hold the Applicant directly responsible for all work performed under this permit .
9.If the City Engineer and /or his Inspector determine that the workmanship is incomplete or unacceptable, the City
reserves the right to withhold release of the permit for up to one year.
