ARCHITECTURAL REQUEST FORM The Association documents require that before making
any changes to your lot, you must first submit an architectural review from the Board of
Directors or its appointed Architectural Review Committee. The person making the proposed
change to his/her lot must submit an Architectural Request Form. He/she is responsible for
providing all information necessary to render a decision on their particular request. Any and all
incomplete forms and /or requests with insufficient information will be automatically denied. Any
and all changes must be made in accordance with the plans and specifications submitted and
approved and may not deviate from said changes without prior approval by the Board of
Directors and/or the Architectural Review Committee. The sanctioned time limit for request
consideration shall not begin until all information necessary for the review of the architectural
request have been submitted. This request shall be deemed active for a period of thirty (30)
days when the completed form and all necessary and relevant information is received. PLEASE
COMPLETE THE FORM BELOW. ATTACH ANY AND ALL PLANS AND SPECIFICATIONS
SHOWING THE NATURE, SHAPE, KIND, HEIGHT, MATERIALS AND LOCATION OF SAME.
● While the Covenants say to allow thirty (30) days for the approval process, it typically
does not take that long. However, please prepare as far in advance of your anticipated
construction/installation date as possible, as the Committee cannot guarantee a timely
response to a “rush” request. Furthermore, failing to include all of the required
documents will ultimately delay the process.
● All proposals MUST include a Plot Plan or Surveyor’s Report with location or outline of
proposed improvement clearly marked as well as drawings and/or photos with sufficient
detail to accurately represent the proposed improvement. A representative picture is
required for all fence proposals regardless of the level of detail provided in the
description area below.
● Homeowner responsible for obtaining any necessary building permits. Approval by this
Committee does not negate the homeowner’s responsibility to obtain any required
building permits and approvals from any governing agency.
● You may apply for more than one change per form (i.e. a fence and a deck). Please
itemize in the description area below.

The Undersigned hereby authorizes Family Leisure, their subcontractors and their suppliers to use
west side
for access to construct the pool and complete all work called for in the contract.
Family Leisure is not responsible for any damage or injury in our access area and / or construc on zone only.
This includes: Sidewalks, invisible fencing, sod, sewer, sep c lines and or tank, down spouts, curbs, landscaping, phone
lines, electric lines, gas lines, water lines, trees and landscaping. By the nature of work being
done there is, of necessity, damage to be expected to the areas and items listed above.
