Addresses listed on original applications/paperwork will not be altered,
but are now incorrect. This form officially updates the file as to the
change of the address.

"There have been some changes made per Gunstra Builders for buildings 1,2,3 and 4.
They are all now addressed off of W Main St with the exception of two units... .2A
and 3H are off of 3rd Ave NW.
