1. Submit the standard Residential Improvement Location Permit Application three-part form, and its
completed submittal requirements sheet. (Square footage chart and signature areas. Note the name of
the model type on the front of the application.)
2. Submit a completed Master Permit Application sheet.
3. Copies of the appropriate sewer and/or water permits.
4. Two copies of the Site (or plot) plan.
S. One copy of the Subdivision Development Plan, or "as-built" for the area of the subdivision containing
the lot to be built on and its surrounding lots. (This is NOT a copy of the recorded plat section. This will
show the property lines, most likely a building pad, or building pad elevation, and show ground topography
lines.)
6. If a revision needs to be made to an original plan that has been submitted, an "APPLICATION FOR
REVISIONS TO ORIGINAL PLAN SUBMITTED" must be filled out and accompany the ILP. Also, two sets of
legible plans drawn to scale, and showing the revised construction must be submitted.

The un rsigned appli nt hereby applies to the ity 0 armel, Permit Services office for an
Improvement Location Permit (ILP) for a home within the master permitting program. The
Applicant hereby agrees to build the same structure that is represented in the approved Master
Plan File on record with the City of Carmel, division of Building & Code Services. Any alteration to
this approved plan will require submitting an Aoolication for Revisions to Oriqinal Plan Submitted.
Under the penalties of perjury (Indiana Code 35-44-2-1), I hereby affirm under oath that all of the
information I have provided in this Master Permit Application is true and accurate to the best of
my knowledge and belief, and that I have not knowingly or intentionally provided or omitted any
information that would tend to hide, obscure or otherwise mislead the Dept. of Community i
Services regarding the truth of the matters addressed therein.
