We offer the following comments: GENERAL INFORMATION 1. The project site is currently within current City of Cannel Corporate Limits. 2. Jurisdictions: • Street and Right of Way—City of Carmel(Medical Drive) • Water and Sanitary Sewer-City of Carmel Utilities. • Storm Sewer/Drainage—City of Carmel. The Hamilton County Surveyor's Office has jurisdiction over legal drains and discharge to legal drains. 3. Board of Public Works and Safety Approval Requirements: • Water and Sanitary Sewer Availability approval: - Water and Sanitary Sewer Availability Approval request to the Board is based upon EDU requirements as determined by the proposed use of the building. Water and Sanitary Sewer Connection Fees will be based upon EDU calculations. - If entryway or other irrigation systems are planned,additional Water Availability Approval from the Board will be required and additional Water Connection Fees will be assessed based upon the size and usage of the system. • Commercial Curb Cut-Please provide 8'h x 11 exhibits to accompany the request for commercial curb cut approval. Include all pertinent data including radii,drive width at throat and street,opposing drives etc. As you are modifying the existing cut,Board of Public Works and Safety approval is required. • Temporary Construction Entrance-Board approval is required if the temporary entrance is located at a site other than the proposed curb cut. Please provide exhibits as detailed above,if applicable. • Open Pavement Cuts in dedicated streets. Any open pavement cut in the Medical Drive pavement will require separate Board of Public Works and Safety approval. It appears the construction of water and sanitary sewer service lines has the potential for open pavement cuts. If it is possible to directional bore these services lines without open cutting the pavement,that is what will be expected. If open cutting is unavoidable,prepare letter of requests to the Board accompanied by detailed drawings of the open cut. • Secondary Plat approval if applicable. • Dedication of right of way if applicable. • Consent to Encroach Agreements for any permanent improvement to be installed within dedicated right of way and/or dedicated easements. Please use the Engineering Department deadlines for submissions to the Board. Any submission to the Board requires prior final approval by the Cannel Clay Plan Commission and/or the Board of Zoning Appeals and completion of review by the Technical Advisory Committee. All written requests to be place on the Board's agenda must include the appropriate Docket Numbers and the date(or dates)of approval by the Plan Commission and/or BZA. 4. TAC Review/Drawings submitted for approval: We request that all comments and comment letters generated by this office be answered in writing and be accompanied by a drawing reflecting requested revisions. Final drawings will not be approved for construction until all issues have been resolved. The design engineer must certify all drawings submitted for final approval. This office will require a minimum of five-sets of approved drawings after all issues have been resolved and the construction drawings have been approved. The drawings will be stamped as approved and signed by the City Engineer and by the Director of Carmel Utilities. The Developer/Owner will receive one-set which is to be maintained on the construction site at all times. Cannel Utilities will receive two sets. The Engineering Department Construction Manager will receive one-set and one-set will be maintained in the Department of Engineering. If more than the minimum of five-sets are requested to be approved, the additional sets(maximum total of two additional)must be submitted with the five required sets. 5. Please be advised that any installation of signs,walls,irrigation systems,etc.within dedicated right of way and/or dedicated easements will require a Consent to Encroach Agreement between the Owner/Developer and the City of Carmel. This Agreement requires Board of Public Works and Safety approval. 6. Please provide Carmel Utilities with all initial and subsequent drawings submitted for review. They will provide a separate review regarding these issues. BONDING REQUIREMENTS 9. Upon initial review,it appears the following bonding requirements may apply to this project: Performance/Maintenance Guarantees This project may be required to provide Certified Engineer's Estimates and Performance/Maintenance Guarantees for the following improvements: • Right of Way Improvements including demolition activities in the right of way,construction of the commercial curb cut or cuts,sidewalks and utility service lines in the right of way. • Erosion Control, Sediment control,and post construction BMP's Provide detailed/itemized Certified Engineer's Estimates in the amount of 100%of the cost of labor and materials for the requested improvements. Provide individual Performance Guarantees in the amount of the Engineer's Estimates. When the improvements have been constructed and accepted and the Performance Guarantee is released by our BPWS,3-Year Maintenance Guarantees will be required in the amount of 15%of the Performance amount for streets and curbs and 10%of the Performance amount for all other improvements. Street Cut/Right of Way Permit and Bonding Requirements Any work in the dedicated right of way of Medical Drive will require a Right of Way Permit. If the work is included in the scope of work of a required Performance Guarantee,a separate License and Permit Bond will not be required. If the work is not included in a Performance Guarantee,a License and Permit bond will be required. The bond amount will be determined by the number of instances of work in the right of way @$2,000.00 per instance. Oxen navement cuts in dedicated streets require annroval by our Board of Public Works and Safety. Please contact Fred Glaser,Department of Engineering Right of Way Inspector/Manager for details regarding this issue. 10. We have engaged Crossroad Engineers,PC to review all drainage plans and drainage calculations submitted to this office for review. We will share Crossroad's comments as they are received. When construction plans are submitted for review for this development,please provide a set of drawings and drainage calculations directly to Crossroad. Contact Amanda Foley,City of Cannel Storm Water Administrator,regarding bonding requirements for erosion control. AVAILABILITY AND CONNECTION FEES 11. Water and Sanitary Sewer Availability(acreage)Fees this site will not be subject to Water and Sanitary Sewer Availability Fees. 12. Water and Sanitary Sewer Connection Fees are required to be paid after Engineering Department approval of the project's construction plans and priorr to obtaining a building permit. Connection Fees are based upon the EDU calculation of the proposed usage of the building at the current rate of$1,310.00 per EDU for water and$795.00 per EDU for sanitary sewer. Connection Fees are current as of the date of this review but are subject to future revisions. Please provide this office with a detailed list of the various uses planned for these buildings. Following are some retail uses that may or may not apply to this project: • Bars&Cocktail Lounges—0.10 EDU/Seat • Retail Space—1.0 EDU/1000 ft2 • Office—0.50 EDU/1000 ft2 • Restaurant—0.10 EDU/Seat • Barber Shop—0.30 EDU/Chair • Beauty Shop—0.90 EDU/Chair If an irrigation system is planned for this development,additional Water Connection Fees will be assessed based upon the recommendations of the Director of Camel Utilities. CONSTRUCTION DRAWING COMMENTS A seven-page Plan Certification Form is enclosed with this letter. This form was created with the desire and intent to streamline the plan review process. We request that you please complete this form and return the original of this form along with complete 24"x 36"civil drawings to the Department of Engineering as soon as possible. Comments will be generated after the Plan Certification form is returned. Please treat each item on the form as a requirement of the Department of Engineering. Providing a"no"or "N/A"response to any of the items does not relieve the Developer of the responsibility of meeting the requirements of the Department of Engineering. The Department of Engineering will make the final determination as to the acceptability of any"no"or"N/A"responses. 13. Title Page. • If the double consonant form is used for the work"Shops',a silent"e"must be at the end of the word. Please revise to either"Shoppes"or"Shops". • Revise spelling of Racket Club in the location description. 14. Any on-site and off-site easements necessary for installation of utilities,sidewalks and storm drainage must be secured prior to the Department of Engineering approving the drawings. 15. Be advised that the City of Cannel has a strict ordinance regarding debris and dirt in dedicated City Streets. Be prepared to minimize tracking of dirt and debris during demolition and construction activities at the site. The contractor or contractors may be required to clean Medical Drive daily at a minimum or more often as required. 16. All paving within the existing and proposed City right-of-way shall conform to the requirements of the Department of Engineering.Please add a note to the drawings that the Contractor shall contact the Department of Engineering to schedule a pre-construction meeting to review the Department's construction requirements,staff notification requirements,required inspections for certain stages of the work and to review the authority of the Department as it relates to work within the existing and proposed City right-of-way. 17. ADA ramps are required at all locations where the sidewalks or asphalt paths cross the streets or entrances. 18. Please delineate the curb and pavement removal required for proposed entrances or removal of existing to be saw cut,removed,and replaced. Curbs shall be removed to the nearest joint. 19. For sidewalk within the right-of-way,please specify the width of the sidewalks to be 5 feet. Do not reduce the width of the east end of the sidewalk as shown on Sheet C6.0. Instead maintain the uniform 5-foot width across the entire frontage. 20. Please add the following notes to the sidewalk detail: "SPACING BETWEEN TRANSVERSE JOINTS SHALL NOT EXCEED 1.25 TIMES WIDTH,""SIDEWALK BUILT WITH DRIVEWAYS SHALL HAVE MINIMUM THICKNESS OF 6"OR THICKNESS OF DRIVEWAY,WHICHEVER IS GREATER,"and"JOINTS SHALL BE TOOLED OR SAWED TO A DEPTH OF'A THE SIDEWALK THICKNESS." • 21. For curbing,the maximum spacing between transverse expansion joints shall be 50 feet. The maximum spacing between transverse contraction joints shall be 10 feet on tangent segments and 5 feet on radius segments. 22. Please include the City of Cannel straight curb standard and the City of Cannel roll curb standard in the plans and replace the curb details indicated. 23. Please remove the Sundown Gardens Landscape Plan,Exterior Elevations(color rendering and Sheet A2.1)and Site Lighting Plans(Sheets E1 and E1.1)from plans submitted to the Department of Engineering for approval. The Engineering Department does not approve these items and they should not be included in drawings submitted to Engineering for review and/or approval. Also remove these items from the Sitework Drawings Index on Sheet C1.0 24. For subsurface drain within the right-of-way,please add the City of Carmel subsurface drain standard and add the note"ALL SUBSURFACE DRAIN TO BE DOUBLE WALL HANCOR HI-Q OR EQUNALENT"in the vicinity of the subsurface drain standard. 25. Please indicate dedication of the Right-of-Way prescribed by the City's 20-year Thoroughfare Plan if the existing Right-of-Way is not of sufficient width. 26. This project is subject to the Cannel Stormwater Management Ordinance,which was adopted February 6,2006. 27. Please include the note"THERE SHALL BE NO WATER VALVES OR MANHOLES WITHIN THE PAVEMENT,CURBS,OR SIDEWALKS,OR ASPHALT PATHS OF THE RIGHT-OF- WAY"on all appropriate sheets. 28. If it will be necessary to relocate existing utility poles,they must be relocated to within one foot of the proposed right-of-way. The expense of such relocation shall be borne by the Developer. 29. The Department requires that the construction drawings be developed in accordance with the City of Cannel digital submission standards and that all required submittals for primary plat,secondary plat,and construction drawings be made. The digital files must be submitted to the Department of Engineering prior to approval of the construction plans. 30. The City has adopted a new paving policy. Please revise any notes,specifications,and details to accommodate this policy. The language of this policy must be added to a specification sheet on the plans. 31. The City has adopted a new curbing policy. Please revise any notes,specifications,and details to accommodate this policy.The language of this policy must be added to a specification sheet on the plans. 32. The portions of commercial driveways within the right-of-way shall be constructed of concrete and shall have a depressed curb along them to maintain drainage. 33. Please designate the type of interior curb provided surrounding the parking areas on the layout plan. 34. Indicate and label the curb return radii at the entrance. They must be a minimum of 30 feet. 35. We request that you share all comment letters and enclosures with the Owner/Developer and eventually with any Contractors chosen to work on this project. 36. Please contact Amanda Foley to review storm water quality aspects of this project. 37. Is a drain needed behind the wall? 38. Is there any benefit to storm water quality to provide a high permeability soil as the swale bottom to allow infiltration into the wall backfill or is this detrimental to the wall/structure system? 39. Does the oil/water separator remove sediment? Where is the oil trapped? Will it come right out of the 24"riser pipe without the bottom elevation of this pipe being below the inlet drop tee? 40. If structures NO. 11 and NO. 12 cannot be deleted,these structures shall be new inlets/not doghouses over the existing pipe. 41. The runoff entering structures 10, 11,and 12 is not being detained and is only being treated once or not at all. Is it possible to delete structure 11 and regrade the swale so a low point is not necessary? Can structure 10 be connected to the detention system? Can structure 12 be moved back onto the site and directed to the detention system? 42. It appears from the storm water summary on Sheet C9.0 that both the 10-year and 100-year peak runoff exceeds the maximum allowable for the 3"orifice. Due to site constraints,is it not possible to restrict the peak runoff further? 43. The connection to structure 24 must be performed by a contractor bonded with the City and is • subject to inspection by the City. 44. Are curb stops being installed where the parking is adjacent to the wall? Typically,spaces in such locations are 20'with a parking bumper to protect cars and the wall. 45. Please add GENRAL NOTE 22: "No earth disturbing activity shall commence without a proper storm water management permit being issued." 46. Please add GENERAL NOTE 22: "No work in the right-of-way shall commence without a proper right-of-way permit." 47. Please revise NOTE 20: "The contractor shall maintain at least one lane open to traffic at all times with appropriate approval for such lane restrictions being obtained by the Board of Public Works and Safety." 48. I know that we have discussed this,but please confirm that structure#29 is or is not in an existing easement. 49. Sheet C4.0—Please indicate removal and replacement of the existing curb and any mainline pavement as needed for the new entrance or to replace the curb at the existing eastern entrance if it is damaged. 50. Please indicate replacement of the removed curbing and pavement on Sheet C6.0 and the correction or repair of any damaged subsurface drain. 51. Please indicate location of construction entrance on C5.0. Refer to comment 3(c)regarding Board of Public Works and Safety approval. 52. Please indicate City standard depressed curb across the new entrance unless the existing curb is roll curb. If the existing curb is roll curb and is in a state of poor repair,indicate for removal and replacement per City standard. These comments represent the Department of Engineering's initial review of the construction drawings for this project. We request that all responses to our comments be provided in writing. Failure to provide written responses may result in the delay of the review process. It is critical that our office be made aware of all modifications made on the plans being resubmitted, particularly if any such changes are considered"new"or fall outside of our previous reviews. Please provide revised plans including all revisions. Please notify us of any changes and specifically state any changes,including changes resulting from Plan Commission,BZA or other committee meetings. All bonds and performance guarantees must be posted prior to Engineering Department approval of construction plans. There is a problem with the size and legibility of the graphic planting details. The whole graphic, which was apparently lifted from elsewhere, is much too small. The contractors will have to read from this graphic. Perhaps you could make the landscape notes smaller, but still legible, and then make the whole details graphic larger. Otherwise the graphic should be submitted on a separate sheet. Also, we need the monetary commitment for the 24 trees that will not be planted but are required. I can figure a 10/17/2006 Page 2 of2 cost based on my knowledge of tree costs and installation costs for that amount of trees, or you can supply the figures based on the costs from your contractor. Let me know shortly which you chose to do, or I will go ahead with my estimate. We have reviewed the construction drawings submitted for this project for the September 20, 2006 Technical Advisory Committee meeting and offer the following comments: BONDING REQUIREMENTS 1. The following bonding requirements apply to this project: Performance Guarantees An Erosion and Sediment Control Performance Guarantee is needed for this project. STORMWATER QUALITY REVIEW COMMENTS-CONSTRUCTION PLANS The following items of the storm water pollution prevention plan have not been satisfied: B2. Sequence describing storm water quality measure implementation relative to land disturbing activities Include construction of the building. All post construction BMPs should be cleaned at the end of construction. B6. Sediment control measures for storm sewer inlet protection The straw bales are not adequate for inlet protection. A catch basin insert would be more appropriate along the existing street and in the inlets surrounded by pavement(in the parking lot). A basket inlet protection product would be more appropriate around inlets in grassy areas. For all other inlets, a mat fastened to the top of the grate will be acceptable. B7. Runoff control measures Rock check dams should be used instead of straw bale dams in the swale. B10. Location,dimensions,specifications,and construction details of each stormwater quality measure Erosion control blankets are needed in the swales. Include the detail for an erosion control blanket. Also include the installation of the erosion control blanket in the construction sequence(B2). B14. Monitoring and maintenance guidelines for each proposed stormwater quality measure Inlet protection should be cleaned after every rain event or at least once per week. • C4. Location dimensions,specifications,and construction details of each stormwater quality measure Sizing calculations for the stormceptor are needed. Need specifications for the swale plantings. Chapter 700 of the City of Carmel Stormwater Technical Standards Manual contains information regarding water quality swale design requirements. The north swale needs to meet the minimum 1%slope requirement for a water quality swale. C5. Description of maintenance guidelines for post construction storm water quality measures An O&M manual specifying information regarding maintenance of both of the water quality BMPs is needed. An owner acknowledgement agreement for the BMPs has been included and should be completed and submitted with the O&M manual. We request that all responses to our comments be provided in writing. Failure to provide written responses may result in delay of the review process. It is critical that this office be made aware of all modification made on the plans being re-submitted, particularly if any such changes are considered"new"or fall outside of our previous reviews. Please provide revised plans indicating all revisions. Please notify us of any changes and specifically state any changes,including changes resulting from Plan Commission, Special Studies or other committee meetings. All bonds and performance guarantees(if required)must be posted prior to Engineering Depai intent approval. Also,Board of Public Works and Safety approval and any other governing agency approvals(if required)must be obtained prior to Engineering Department approval.
