State Release, dated 12/15/06, has one
condition and one note re:
1. The complete installation of the
exhaust hood and hood suppression system
shall be in accordance with Section 507,
IMC (675 lAC 18-1.4).
NOTE: Release includes the suppression
systtem for the hood.
When applicants submitted, they said
that these were the full plans that had
been sent to the State, and in fact,
these were more detailed, per request of
the State plan reviewer. SNL questioned
no other releases beside HOOD, and they
could not say whether or not further
releases were needed, or if they were,
why they were not included by the State,
since the plans are the full plans.

PER JIM BLANCHARD: He talked to State
plan reviewer during his review, and
that person told hime that the release
was full, for ALL the work being done.
They indicated this by listing it as
'Standard' on the release. This is
different than we have seen it before,
(since all trades are not listed as
separately released in the scopes) but
was verified, per Blanchard.
This permit is valid only if construction commences within one (1) year of the date of issuance of the State Commercial Design Release. All construction
must be completed (C/~ issued) wit.hin two (2) rears of t~e issuance date.

requested by this application will comply with, and conform to, all applicable laws of the State of Indiana, and the "Zoning Ordinance of Carmel Indiana - 1993~
(Z- 289) and amendments, adopted under authority one. 36-7 et seq, General Assembly of the State of Indiana, and all Acts amendatory thereto. I further certify
that only kitchen, bath, and floor drains are connected to the sanitary sewer. I further certify that the construction will not be used or occupied until a
Certificate of Occupancy has been issued by the Department of Community Services, Cannel, Indiana.
