Sign Placement Affidavit form (found in Handout) once the sign is placed. f)
Please provide the filled out & notarized Petitioners Affidavit of Notice of
Public Hearing form (found in Handout). g) Please provide the filled out
Notice of the Public Hearing form/template (found in Handout). h) Please
provide proof of notice, such as Publishers Affidavit from the newspaper,
once you receive them. i) Please make sure to list out your practical
difficulties and hardships associated with having to comply with the zoning
ordinance. j) Please note that Transportation Plan Compliance is required per
UDO section 1.07, such as sidewalk installation along the street (or payment
into the Thoroughfare Fund). Please contact Alex Jordan in the Carmel
Engineering Dept. for more info about what will be required, if anything.

Please show/label the bicycle parking rack locations, as well as provide the
bike rack designs. See UDO section 5.29. (Please work with David Littlejohn
in our office on this.)

Would property owner be open to donating the portion of the wooded
property within the flood zone to Carmel/Clay Board of Parks and
Recreation? CCPR would resume responsibility for maintenance of trees and
would remove approximately 5100 sf from taxable property.

south of the flood zone setback to the Parks

Please properly mulch trees and remove/fix any 'volcano' mulch around the
existing trees. Please allow the shrubbery around the building foundation to
at least grow to a 3' height - stop hedging shrubbery down. These are both
requirements of the UDO sec 5.19.H.2.

Sidewalk required along Pro-Med Ln
City standards require sidewalks along all interior streets and the City's
Mobility and Pedestrian plan requires 5' sidewalks along Pro-Med Ln. Revise
the plans to show the required sidewalk along this projects frontage of Pro-
Med Ln. You may elect to provide a monetary commitment equal to the
value to otherwise design and construct the improvements along the
roadway frontage. If you choose to provide a commitment in lieu of
constructing the sidewalk, please work with the Engineering Department to
determine the appropriate commitment amount to fulfill this requirement.

sidewalk connection to existing sidewalk
Please revise the plans to include a direct sidewalk connection from the new
sidewalk on the south side of the building to the existing sidewalk along the
east side of the building. This will provide a direct pedestrian connection and
accessible route from the adjacent path to the south of the building/site to

of the building are too steep to make a connection. The
slope of the walk would be 9% and not an accessible
route. The connection has not been added to the plans
