and specifications filed in conjunction:with this project shall comply with afl of the applicable rules and laws of Fire Prevention and Building Safety
Commission. Providing false informatio,n constitutes an act of pe~ury, which is a Class D felony punishable by a prison term and a fine up to $10,000.
Ir accordance with S~cticm 19 of the General Administrative Rules (675 lAC 12-6-19) a complete set of plans and specifications that conform
exactly to the design that was released by the office of the state building commissioner shall be maintained on the construction jobsite as well as a
copy of the design release.

No addition, alteration, or repair shall reduce existing exit capacities to less than that required under
the- provisjons of the rules of the Indiana Fire and Building Safety Commission for new construction
in ,accordance with 675 lAC 12-4-12(d).
A plan showing the dimensioned location of the building relative to all property lines and existing
buildings on the property, as well as the widths of any streets or easements bordering the property,
shall be submitted in accordance with 675 lAC 12-6-7(h)(1).
The fire alarm system shall be designed as specified in Section 907, IFC (675 lAC 22-2.3) and
N.F.P.A. 72 (675 lAC 22-2.2-17) in accordance with Section 907, IBC (675 lAC 13-2.4).
Manual fire alarm pull stations shall be located and installed in accordance with Section 907.4, IFPC
(675 lAC 22-2.3)
3W0907A2 Upon completion of the installation of the fire alarm system, a satisfactory test of the entire system
shall be made. All functions of the system or alteration shall be tested in accordance with Section
907.17, IFPC (675 lAC 22-2.3).
3B10031 The means of egress is to be physically constructed in such a way that the exit path is permanently
maintained in accordance with Section 1003.2.8, IBC (675 lAC 13-2.4).
3M920A Suspended-type unit heaters shall be installed and supported in accordance with Section 920, IMC
(675 lAC 18-1.4).
Please be advised that if an administrative review of this action is desired, a written petition for review must be filed at the above address with the
Fire Prevention and Building Safety Commission identifying the matter for which a review is sought no later than eighteen (18) days from the above.
