BMP Location and Description
BMP’s are designed for the benefit of water quality or quantity. There is one BMP
proposed for this development:
The primary BMP for this development is a permeable paver system. Runoff from
the site shall sheet flow to the permeable paver areas where it will be conveyed to
the aggregate bed below. Sediment shall be removed as the runoff is filtered
through the stone section. The BMP is located within the front parking lot.
Inspection & maintenance procedures shall be followed as outlined in this manual.
The overall property shall contain expansive areas of grass and plantings that shall
continue to filter out sediment. The grass shall intercept upstream runoff from the
developed project site and will remove pollutants via sheet flow properties.
The Bickford of Carmel development is located at on the north side of Smoky Row Road
approximately ¼ mile west of the N Rangeline Road and Smokey Row Road intersection in
the City of Carmel, Indiana. Please refer to Appendix A for site drawings of the BMP’s.

Maintenance and Inspection Guidelines
Maintenance and inspection as described below is the responsibility of the BMP owner.
Any additional procedures deemed necessary by annual City inspections shall be
incorporated into these guidelines.
Routine Maintenance
The owner shall be responsible for the following minimum operations for
maintaining the BMP’s:
Inspect the permeable paver surface to check for any surface ponding that
might indicate possible clogging of the pavers. In addition, no contaminants
shall be dumped onto the pavers, or into the drainage inlets.
Inspect pavers for ruts, deformations, or broken units.
Any accumulated snow that requires plowing shall not be stored on the
permeable paver areas. In addition, sand or other abrasives for snow or ice
removal shall not be used.
Prevent debris and grass clippings from being discarded onto the pavers. In
addition, all leaves shall be raked and removed in a timely fashion.
Maintain and update the BMP Operations and Maintenance Manual as
necessary.
Remedial Maintenance
The owner shall be responsible for correcting and/or replacing any portions of the
BMP’s in unsatisfactory condition. This includes, but is not limited to:
Replacing any broken or deformed paver units.
Replacing any portion of outlet pipes or underdrain piping.
Grading and reseeding eroded areas of the grass filter strips & lawn areas.
BMP Inspection
Annual inspections of permanent BMP’s may be performed by the City. Routine
inspections are the responsibility of the BMP owner. These inspections shall be
accomplished as scheduled below:

The approved maintenance plan and inspection forms provided in Appendix B shall
be used as guidance for performing maintenance and inspections activities. These
forms shall be retained by the BMP owner and produced upon request by the City.

The City shall be notified of any changes in BMP ownership, major repairs, or BMP
failure in writing within 30 days. Furthermore, an annual inspection report shall be
submitted to the City of Carmel. All reports and notification shall be addressed to:

In the event that the City finds a BMP in need of maintenance or repair, the City will
notify the BMP owner of the necessary maintenance or repairs and give the
landowner a timeframe for completing the maintenance or repairs. If the
maintenance or repairs are not completed within the designated timeframe, the City
shall perform the repairs or maintenance and bill the landowner for the actual costs
for the work. All maintenance costs associated with the site BMP shall be the
responsibility of the owner.
Sediment Removal
Sediment accumulation shall be inspected on a quarterly basis and after major storm
events. Sediment accumulation shall be inspected on a monthly basis and after major
storm events. Removal of the sediment accumulation shall be completed as necessary as
visual inspection indicates. The sedimentation and any other debris taken from the site
shall be disposed of in compliance with all applicable Federal, State and Local
requirements. Immediate remedial maintenance shall be completed in the case of any of
the aforementioned occurrences.

I understand that this BMP O&M Manual has been created per requirements of the City of
Carmel Stormwater Design and Construction Specifications Manual and that the
recommendations and requirements shall be revised and/or updated to comply with any
changes implemented within that document. The BMP’s governed by this O&M Manual
are the following: Porous asphalt detention basin, Nyloplast catch basins, & a grass filter
strip.
I recognize that it shall be the right of the City and authorized representatives to enter the
property containing the BMP for maintenance and inspection purposes, and that this
access shall be granted without hindrances by means of the utility and drainage easement
previously described in this manual.

Owner Acknowledgement Agreement (“Agreement”)
For good and valuable consideration, the receipt and sufficiency of which are hereby
acknowledged, the undersigned owner (“Owner”) hereby submits this Operation and
Maintenance Manual (“Manual”) to the City of Carmel, Indiana (“City”) as a written
acknowledgement of Owner’s warranty and agreement to institute, maintain, and follow
the water quality Best Management Practices (“BMPs”) listed below, and to follow and
abide by the inspection schedule and maintenance activities listed in this Manual. The
Owner also hereby agrees to provide, at Owner’s cost, all additional maintenance, repair,
and/or replacement services reasonably necessary to maintain the function and longevity
of the BMPs from and including the date this Agreement is executed by Owner to and
including the date on which a new Agreement is filed with the City by another party who
assumes all of the obligations and responsibilities of Owner as set forth herein.
