On the Plans
The technical review and comments are intended to evaluate the completeness of the erosion and sediment control plan for the
project. The erosion and sediment control plan submitted was not reviewedfor the adequacy of the engineering design. All
practices included in the plan, as well as those recommended in the comments should be evaluated as to their feasibility by a
qualified individual with structural practices designed by a qualified engineer. The plan has not been reviewedfor local, state,
or federal permits that may be required to proceed with this project. Additional information, including design calculations may
be requested to further evaluate the erosion and sediment control plan. ~
The erosion and sediment control plan has been reviewed and it has been determined that the plan:
X Satisfies the minimum requirements and intent of 327IAC 15-5 (Rule 5). Notification will be forwarded to the
Indiana Department of Environmental Management.
Proper implementation of the erosion and sediment control plan and inspections of the construction site by the developer or a
representative are necessary to minimize off-site sedimentation. The developer should be aware that unforeseen construction
activities and weather conditions may effect the performance of a practice or the erosion and sediment control plan. The plan
must be a flexible document, with provisions to modify or substitute practices as necessary.

RE: Shelborne Park, Section 2, Secondary Plat Application
Dear Mr. Olmstead:
This letter is the second in response to your Secondary Plat filing for Shelbome Park, Section 2. The
following issues must be resolved prior to signature of the plat by the director:
1. Add Submission Date and Source of Title to all applicable sheets
2. Addnote to plat regarding public access to trails within the common area. Why has
the notation for this between Lakes at Hayden Run and Ridge at Hayden Run from
note on plat to an easement?
3. Forward a copy of the final review letters from the following agencies: County Highway,
County Surveyor, Hamilton County Soil and Water.
4. The monuments and markers should be as required under Section 8.2 of the seo.
a. Add concrete monuments at far comers of plat.
5. Provide letter from owner regarding status of Road Improvements per Commitment made at
time of Primary Plat. We will evaluate this to determine the appropriate action in receiving
the improvements per the commitment.
Please submit a revised Secondary Plat with the above-mentioned changes. Once these changes have
been made and comments addressed, please produce a mylar copy for signature by this Department. Two
paper copies of the plat must be filed with the mylar prior to recording. I will return the signed mylar to
you for signature by the County Commissioners. Once recorded, our Department needs three paper
copies and a mylar of the recorded plat. Our approval of the secondary plat (signature) will be
forthcoming upon revision of the plans according to the items listed above.

the revised plans for the above-mentIoned project. After reviewing the plans the Highway
Department has the following comments:

submitting the seven sets of plans.

Note: All erosion and sediment control measures shown on the plans and referenced in this review must meet the design
criteria, standards, and specifications outlined in the "Indiana Handbookfor Erosion Control in Developing Areas" from the
Indiana Department of Natural Resources, Division of Soil Conservation or similar Guidance Documents.
Attached is a construction sequence for residential sites. Initial perimeter defenses should include protecting
the existing inlet on the east property line.
A check dam is recommended for the rear swale on the south end oflots 42 & 43.

Hello everyone:
Following are CTRWD comments on the agenda items effecting our sanitary sewers or service
area for the December 18, 2002 TAC meeting. Please feel free to copy and distribute this
email or attach it to your meeting minutes. We have also created a new page on our
website linked to the "Construction" page where we have posted all CTRWD TAC comments from
2001 and 2002 meetings held to date. We will continue to add to this page every month so
please let everyone know they can view our comments online now too.
Village of Mount Carmel, Section 10 (Primary Plat)
The District has not received any plans for this proposal. Building permits should NOT be
issued unless the District has issued connection permits.
Wexley Chase (Primary Plat)
The plans received by the District show the sanitary main lines and manholes, but will
need to be revised to show sewer profiles, manhole, lateral and bedding details. The
District's most recent version of sanitary sewer specifications also needs to be included
on the plans. Revised plans need to be submitted. A preconstruction meeting will need to
be held before the construction of the proposed sanitary sewers. Building permits should
NOT be issued unless the District has issued connection permits.
University High School (Special Use, DSV)
The District has not received any plans for this proposal. Building permits should NOT be
issued unless the District has issued connection permits.
Shelborne Park, Section 2 (Secondary Plat and Construction Plans)
The plans that the District received are in accordance with the District specifications.
Building permits should NOT be issued unless the District has issued connection permits.
West Carmel Center, Block D, Lot 3 - Applebee's (Development Plan)
The plans received by the District show the sanitary lateral, but will need to be revised
to show lateral and bedding details. The District's most recent version of sanitary sewer
specifications also needs to be included on the plans. Revised plans need to be

This letter serves to acknOWledge receipt of a transmittal received 12114/02 containing
the plans for the abovl;1Rmentloned project. After reviewing the plans the Highway
Department has the following comments:
1. Please replace Sharon C. Clark with Christine Altman In the Board of Commissioners
signature block.
2. Before any approvals are granted for the Secondary Plat, an Engineers Estimate
broken Into the follOWing categorles will need to be submitted. along with a
Performance Bond Or Letter of Credit In the amount of 100% of the estimate:
* Stone Base
* HAC Binder
* HAC Surface
* Concrete Curbs
* Street Name and Regulatory Signs
3. On Sheet #3, in note B In Section 6. Site Grading, the note should read 0.05 feet. not
0.10 feet. .
4. ADA ramps will be needed In each direction at the street Intersections. One single
curb depression will not be permitted.
5. On the Traffic Control Plan. Include a legend of the signs and the appropriate
number of each sign.
6. Please Include flow lines on the Site Development Plan.
7. Please Include the centerline stationing on the street plan and profile sheets.
8. Relocate the valve at the Intersection of Lance Drive and Bellshire Lane to a point
. behind the curb.
9. A Testing and Inspection agreement will be necessary for Section 2. The necessary
documents will be forwarded to Logan Limited for completion.
10. Please note that further comments may be necessary In the future.

Dear Mr. Olmstead:
This letter is in response to your Secondary Plat filing for Shelborne Park, Section 2. The following
issues must be resolved prior to signature of the plat by the director:
1. Provide an overall landscape plan for the entire subdivision.
2. File the landscape plan for common area # 6 with the secondary plat.
3. Revise all sidewalk ramp approaches and depressions at corners of streets to County
Highway specifications (see attached example from the Claybourne Estates subdivision).
4. Add the secondary plat docket # to each sheet of the plat, and remove secondary plat
amendment docket # from all sheets.
5. Change "Survey & Plat Prepared By" to "Surveyor" on all sheets.
6. Remove Sharon Clark from the Board of Public Works Certificate and replace her with
Christine Altman.
Please submit a revised Secondary Plat with the above-mentioned changes. Once these changes have
been made and comments addressed, please produce a mylar copy for signature by this Department. Two
paper copies of the plat must be filed with the mylar prior to recording. I will return the signed mylar to
you for signature by the County Commissioners. Once recorded, our Department needs three paper
copies and a mylar of the recorded plat. Our approval of the secondary plat (signature) will be
forthcoming upon revision of the plans according to the items listed above.
