Submittal meeting on 2/5107 wi John
Jaffe, Greg Levell, Jim Blanchard, and
Sarah Lillard. Builders stated that
elevator will not have sump pump. Jim
Blanchard stated they needed a variance
from the State for this. They said they
would check into it, but felt their
construction design itself might exempt
them from this; or this may not be a
requirement any longer. Inspections
fees were discussed, (how extras can be
incurred) as well as suite numbers. SNL
asked that when they have a tenantlve
lease layout, they contact either her
or Bill Akers to determine the proper
numbering assignments.
This permit is valid only if construction commences within one (1) year of the date of issuance of the State Commercial Design Release. All construction
must be completed (C/O issued) within two (2) years of the issuance date.
I, the undersigned, agree that any construction, reconstruction, enlargement, relocation, or alteration of a structure, or any change in the use of land or structures
requested by this application will comply with, and conform to, all applicable laws of the State of Indiana, and the ~Zoning Ordinance of Carmel Indiana - 1993~

The building & Sewer Shall be pve sewer pipe meeting ASTM specifications 3034 SDR 35 of latest revision; or vitrified clay pipe, meeting
ASTM specifications C-700 for extra strength clay pipe of latest revision unless other materials are hereby permitted in writing. The sewer
shall be installed in accordance with ASTM 2321 for pve pipe and the Uniform Plumbing Code for the State of Indiana. All installations shall be
in strict compliance with pertinent City of Car me] ordinances. Back Water check valves shall be installed in accordance with City Code Section
9-122(a), and sections P3008.1 and.2 of the International Residential Code. All building sewers shall be 6" diameter.
All installations shall be "onen trench" insnected and aooroved bv the Cannel Sewer Dcnartment before anv backfillinl! is done. Non-
compliance may result in digging up the sewer installation and/or denial of future sewer permits and/or denial of water connections.
No footing or foundation drains or other sources of ground water or storm water shall be pemitted to enter the public sewer.
Sewer insoections should be reauested at (317) 571-2648 one to four hours in advance.
No inspections or installations will be made on Saturday or Sunday or holidays unless arrangements are made at least 24 hours in advance. All
plumbers or contractors installing sewer (or water) lines shall have a plumbers bond posted with the CITY ENGINEER'S OFFICE. Ifany street
must he cut. a senarate street cut nermit shall he oht::Jinco.
