Dear Members:
The Carmel/Clay Plan Commission offers you the following report on the application (Docket No. 164-
02 Z) of plum Creek Partners, LLC petitioning the Commission for a favorable recommendation to
rezone property generally located at the southeast corner of 1461h Street and Hazel Dell Parkway:
The Carmel/Clay Plan Commission's recommendation on the petition of the applicant is
"FAVORABLE."
At its regularly scheduled meeting of May 20,2003, the Carmel/Clay Plan Commission voted ten (10) in
Favor, zero (0) Opposed, zero (0) Abstaining, to forward to the Cornmon Council the proposed
Ordinance No. Z-410-03 with a Favorable Recommendation.
Please be advised that by virtue of the Plan Commission's Favorable Recommendation, pursuant to IC
36-7-4-608(t)(4), the Council has ninety (90) days to act on this petition before it becomes effective as
Certified by the Commission. Ninety days from the date of this Certification is Thursday, August 28,
2003.

Last evening, the Carmel Plan Commission unanimously approved the rezone request of
Plum Creek Partners for the eleven (11) acre parcel of real estate located in the southeast comer
of East 146th and Hazel Dell Parkway. I would appreciate your forwarding certification of the
action of the Plan Commission to the City Clerk Treasurer in anticipation of a presentation to the
City Council on June 16, 2003. I would appreciate your providing me with a copy of the
Certification by return mail in the self-addressed, stamped, enclosed envelope. Thank you for

The Subdivision Comminee extended insightful suggestions. including revisions to the landscape pla11, the
revision of the ligbtine plan to incorporate "daD; sky frieudly" lights, and revisions to the PUD Ordinance. After revisions
to the landscaping plan are imp1emeoted, the landscape architect wants to confer with Marilyu. Anderson and Scott Brewer.
After the revisions to the lighting pbu1 and PUP Ordinance are completed, I WBllt to confer with you to obtain your
thoueths. Unfortunately, the revisions and meetings will take us beyond the:: Subdivision Committee meeting scheduled for

Please submit the following with your construction plans:
1. Drainage calculations
2. An outlet permit witJ be required if you connect to the stonn sewer In Ashmore Trace subdivision. This
lower outlets to the Emily VOItaI Drain.
3. The annexation status at the time ofconstl'\lction will cletermlne lfyour stonn sewers are regulated and

RE: Riverview Medical Park-Project Review #1
Dear Mr. Frankenberger:
We have reviewed the drawing submitted for this project and offer the following comments:
GENERAL INFORMATION
1. The project site is currently located outside of current City of Carmel Corporate Limits. However, the
area is being proposed for annexation via Annexation Ordinance C223. This annexation should be
approved and recorded in early 2003. Based upon annexation, the following jurisdictions will apply.
2. Jurisdictions:
. Street and Right of Way - City ofCarmel/Hazel Dell Parkway.
Hamilton County Highway/146th Street
. Water, Sanitary Sewer, Storm Sewer - City of Carmel.
3. Board of Public Works and Safety Approval Requirements:
. Water and Sanitary Sewer Availability.
. Commercial Curb Cut. Please provide an exhibit that shows the location of the curb cut and
acellldecellanes if applicable; appropriate dimensions including drive width, overall width at the
street, acell/decel dimensions, radii, etc.; and location of existing opposing curb cuts, if applicable.
The exhibit should be 8 Y2 x 11 or of a size that can be reduced to 8 Y2 x 11 such as 8 Y2 x 14 or 11
x 17. Hamilton County Highway will have jurisdiction over any curb cuts on 146th Street. Carmel
Board of Public Works will have jurisdiction over any curb cuts on Hazel Dell Parkway.
. Temporary Construction Entrance. Same requirements as for the Commercial Curb Cut.
. Any permanent improvement to be installed within dedicated right of way.
. Consent to Encroach Agreements for any permanent improvement to be installed within dedicated
easements. Ifan irrigation system is planned for this project, additional Water and Sewer
Availability approval will be required from the Board.
. Secondary Plat approval (if applicable).
. Right of Way Dedication (if not platted).
I am enclosing a schedule for Board of Public Works and Safety meeting dates and agenda deadlines
for your use. Please use the Engineering Department deadlines for submissions to the Board.
Any submission to the Board requires prior final approval by the Carmel Clay Plan Commission
and/or the Board of Zoning Appeals and completion of review by the Technical Advisory
Committee. All writtcn rcquests to bc place 011 the Board's agenda must include the appropriate
Docket Numbers and the date (or dates) of approval by the Plan Commission and/or BZA.
4. Water and Sanitary Sewer Availability (acreage) and Connection Fees:

I am enclosing copies of our Average User Factors for Water and Sanitary Sewers and our current
fee schedule. I have also highlighted the Average User Factors that might apply to this
development (Medical Office and Immediate Care Center).
Note: Any deviation from the Connection Fee calculations listed in our Ordinance will require
Carmel Utilities and Board of Public Works and Safety approvals.
5. TAC Review/Drawings submitted for approval:
We request that all comments and comment letters generated by this office be answered in writing and
be accompanied by a drawing reflecting requested revisions. Final drawings will not be approved for
construction until all issues have been resolved. All Carmel Utility issues must also re resolved. The
design engineer must certify all drawings submitted for final approval. This office will require five-
sets of approved drawings after all issues have been resolved. The drawings will be stamped approved
and signed by both the City Engineer and Director Carmel Utilities. The Developer/Owner will
receive one-set which is to be maintained on the construction site at all times. Carmel Utilities will
receive two-sets. Our Public Works Inspector will receive one-set and one-set will be maintained in
the Department of Engineering. If more than five-sets are requested to be approved, the additional sets
must be submitted with the five required sets.
6. Please be advised that any installation of signs, walls, irrigation systems, etc. within dedicated
easements will require a Consent to Encroach Agreement between the Owner/Developer and the City
of Carmel. This Agreement requires Board of Public Works and Safety approval. Installations within
dedicated right of way require separate Board approval.
7. Carmel Utilities does not subscribe to "Holey Moley" and should be contacted directly for all water
main and sanitary sewer main locations.
8. Carmel Utilities will provide a separate review of this project for water and sanitary sewer issues.

Please provide individual, detailed certified Engineers Estimates for 100% of the cost oflabor and
materials for the above improvements. Post individual Performance Guarantees in the amount of the
Engineer's Estimates. Three-year Maintenance Guarantees will be required upon release of the
Performance Guarantees.
Street Cut/Right of Way Permit and Bonding Requirements
Any work in dedicated right of way will require a Street CutlRight of Way Permit and a License and
Permit Bond. The bond amount will be determined by the number of instances of work in the right of
way (curb cut, storm sewer work, etc.) @ $2,000.00 per instance.

13. Has there been a Traffic Study completed. Please share a copy with this department.
14. 146th Street access, curb cut, median cut and other right of way issues are under the jurisdiction of the
Hamilton County Highway Department.
15. The curb cut on Hazel Dell Parkway is under the jurisdiction of the City of Carmel Board of Public
Works and Safety. We will require additional information prior to giving final approval regarding the
drive on Hazel Dell:
. Copy of Traffic Study.
. Show existing pavement markings on Hazel Dell Parkway.
. Show the existing Hazel Dell Christian Church curb cut on the drawings.
. How many lanes wide at Hazel Dell.
. We are concerned with the proximity of the drive to the intersection of 146th Street and Hazel Dell
Parkway. We would request alternative locations be provided or perhaps a shared drive
arrangement with the adjacent Hazel Dell Christian Church be pursued.
16. What is the "Future Cross Access" along south border?
17. The storm sewer connection to the Ashmore Trace subdivision will require the submission of drainage
calculations and may possibly require Board of Public Works and Safety approval since the area has
not been formally annexed.
18. Show, label and dimension the Hazel Dell Parkway on the drawings.
19. Show, label and dimension the Landscape Easement along Hazel Dell Parkway.
We reserve the right to provide additional review and comments upon receipt of detailed construction
drawings and drainage calculations.
The above comments are based upon the Engineering Department's initial review. Please provide our
office with a follow up letter and revised plans, indicating all revisions. A second review will focus only
on revised items. It is critical that our office be made aware of all modifications made on the plans being
re-submitted, particularly if any such changes are considered "new" or fall outside of our initial review. If

the plans for the above-mentioned project. The following comments are to follow up
issues that were discussed at our meeting of November 18, 2002.
1. The meeting was attended by Corby Thompson - Plum Creek Partners, Janet
Sterling - the Schneider Corporation, Jim Neal - Hamilton County Highway Engineer
and Steve Broermann - Hamilton County Highway Department:
2. Parcels 2 and 3 will be used for Primary Medical Uses.
3. The current median cut that exists for the house at the corner of 146th Street and
Hazel Dell Parkway will be closed.
4. The current proposed location of the entrance does not allow for the necessary left
tum lanes to be constructed.
5. In accordance with the Hamilton County Thoroughfare Plan, additional right of way is
required at the intersection of 146lh Street and Hazel Dell Parkway. On Hazel Dell,
the right of way from the intersection to a point 500' to the south is an 80' half. From
that point to the southern property line the right of way is 70'. On 146111 Street, the
right of way from the intersection to a point 500' to the east is an 85' half. From that
point to the eastern property line the right of way is a 75' half. A 50' triangular cut is
also needed at the intersection, connecting the right of way for the perimeter streets.
6. The entrance for the Medical Park should align with the entrance to the Akin property
on the north side of 1461h Street.
7. A left turn lane will need to be constructed for the entrance.
8. A right in/right out entrance will be permitted at the property lines of Parcel 1 and 2.
Additional directional signage will be required for the entrance.
The following items are additional comments that the Highway Department has added:

are submitted.
10. Commercial Drive Permits will need to be obtained before any construction begins
on the entrances. Permits will also be required for any utility work performed within
the right of way.

Soils Information:
Brookston silty clay loam- this soil is dark colored, silty in texture and on depressional uplands.
It is deep and very poorly drained with moderate permeability. It has high available water for
plant growth and high organic matter content. It has compact till starting at a depth of 40 to 60
inches. The main soil features that affect the urban development uses are seasonal high water
table, high potential frost action, moderate shrink-swell potential, moderate permeability and
ponded surface water.
Crosby silt loam, 0-3 percent slopes- this soil is light colored, silty in texture and on sloping
uplands. It is deep and somewhat poorly drained with slow permeability. It has high available
water for plant growth and medium organic matter content. The soil has compact till starting at a
depth between 20 -40 inches. The main soil features that affect urban development uses are
seasonal high water table, moderate shrink-swell potential, high potential frost action and slow
permeability.
Miami silt loam, 2-6 percent slopes- this soil is light colored, silty in texture and on sloping
uplands. It is deep and well drained with moderate permeability. It has moderate available water
for plant growth and a medium organic matter content. It has compact till starting at a depth
between 20- 40 inches. The main soil features that adversely affect urban development uses are
moderate potential frost action, moderate shrink-swell potential, moderately slow permeability,
low strength and erosion during construction.

Ockley silt loam, 0-2 % slopes- This nearly level, deep well drained soil is mainly on broad
terraces. Permeability and available water capacity is moderate. This soil is suitable for urban
development. Features that adversely affect engineering uses of this soil are moderate frost
action, moderate shrink-swell potential, moderate pe~eability in the subsoil and very rapid
permeability in the under lying material.
Westland silty clay loam - This nearly level, deep, very poorly drained soil is in depressions,
swales, and narrow drainageways on outwash plains. Runoff from higher adjacent soils is
ponded on this soil. Permeability is slow. The water table is commonly at the surface or is at a
depth of less than one foot in winter and early spring. Available water capacity is high. Surface
runoff is ponded or is very slow. The main soil features that adversely affect engineering uses of
this soil are a seasonal high water table, high potential frost action, moderate shrink-swell
potential, and slow permeability. The base material for roads needs to be replaced or
strengthened with suitable material.
Additional information about soils is available in the "Soil Survey of Hamilton County". The engineer and/or
developer should consider the need for an onsite soils investigation.
I have reviewed the plans for this project and have the following comments:
. Improvements are needed to the existing drainage way at the southwest comer.
. Existing wells need to be capped properly and according to local, county and state
regulations.

Enclosed please find one (1) original and one (1) copy of a rezone application for the above
referenced matter, as well as two (2) sets of plans for the same.
My understanding is that the tentative schedule is as follows:
1. October 18, 2002 - filing deadline;
2. October 24, 2002 - conference with Jon Dobosiewicz and clients;
3. November 4,2002 - TAC mailing;
4. November 18, 2002 - deadline to fax notice to paper;
5. November 20,2002 - TAC meeting;
6. November 22,2002 - deadline to mail and publish;
7. December 6, 2002 - deadline to submit Plan Commission brochures;
8. December 17, 2002 - Plan Commission hearing.
I have not included the additional schedule through the new year. However, we will have plenty
of time to confirm the remaining schedule. Please contact my office after you have reviewed the
application and determine the filing fee. Once you advise me of the filing fee, I will pay the same in
