Following are responses to the Carmel Department of Community Services' plan review comments dated
October 23, 2002:
3. Plans illustrate future parking garage encroaching into 60' building setback. Revise plans
accordingly. I would suggest that you include this future structure in the Variance request for the
Central Plant.
RESPONSE: The edge of the future garage has been revised to align with the central plant.
6. Pedestrian paths:
f. 10' asphalt path is required within the right-of-way along both Pennsylvania Parkway and
College Avenue.
RESPONSE: They have been added.
g. Provide articulated pedestrian access from the paths to building at each entrance to site.
RESPONSE: They have been added.
h. How do people get to pavilion area from the building and/or 10' multi-use path within
right-of-way?
RESPONSE: Sidewalks have been added.
6. Landscaping:
c. Shrubs are required per 23B.IO.2.b(5)(a)(iii) and subsection D of the previously stated
section within the parking areas and site perimeter. Revise plans accordingly. A variance
of these requirements will not be supported.

RE: Clarian North Hospital Campus - Docket No. 146-02 DP/ADLS
Dear Mr. Scimia:
This letter is in response to the applications you have filed for Clarian North Hospital Campus. The
following comments need to be addressed and/or plans/applications updated accordingly:
1. The right-of-way for Pennsylvania Parkway is 120' pursuant to the Thoroughfare plan. A
40' half is shown. Revise plans accordingly to illustrate a 60' half and prepare dedication
instrument for additional 20' . As discussed there is also the option to revise plans to show
40' of additional right-of-way off the north side of street and dedicating that area of real
estate. Regardless, it is my expectation that improvements will need to be made within the
right-of-way to provide dedicated left hand turn lanes into the site at each full-access
entrance in addition to the two thru lanes in each direction. This would necessitate the
additional right-of-way on the north side of the existing alignment.
2. Verify 45' half ROW along both sides of College Avenue. It is my expectation that
dedicated left hand turn lanes will be needed at the south entrance and the intersection of
Penn and College with added travel lanes south to the improvements being made to the
96th/College intersection (five lane cross-section to just north ofPennlCollege intersection).
3. Plans illustrate future parking garage encroaching into 60' building setback. Revise plans
accordingly. 1 would suggest that you include this future structure in the Variance request
for the central plant.
4. Central Plant:
a. Required front setback for the plant is 85',68' +/- is shown. Variance has been
requested (25.1.2.B.3(a)(i)(b). The Department will support this request subject to
the proposed building elevation.
b. Maximum height of the plant is 25'; 46' is shown. Variance has been requested
(25 .1.2.B.I). The Department will also support this request.
5. 1-465 frontage:
a. Change "building setback line" to read "build to line". A Variance has been
requested from the 90' build-to requirement per Section 23B.8.1(c).
b. A 30' greenbelt is required. Detention encroaches into the greenbelt. A Variance
has been requested per Section 23B.I0.2.A.(I) ofthe Ordinance.

7. Signage: multiple variances have been requested. Out department is not in support of the
height of the Primary Campus Directional sign: DC1. Please revise request.
8. The proposed FAR exceeds .7. A Variance has been requested from Section 23B.8.6. Our
department will support the variance subject to the applicant providing necessary
improvements to site access that will support increased intensity of use on the site.
9. Landscaping:
a. Variance has been filed regarding the protection of existing trees (23B.10A).
b. Variance has been filed regarding landscaping within parking lots (23B.10.2.D).
c. Shrubs are required per 23B.10.2.B(5)(a)(iii) and subsection D of the previously
stated section within the parking areas and site perimeter. Revise plans accordingly.
A Variance of these requirements will not be supported.
d. Provide revised landscape plan per illustrating variance request number 8 (your
application).
e. Please schedule a meeting with Scott Brewer, DOCS Urban Forester and myself to
discuss reforestation and other landscaping options to offset negative impacts
associated with the granting of said variances.
10. Building height: maximum permitted building height is 104' per 23B.8.3 .B(2) of the
Ordinance. Be advised that any future increase in building height may require the granting
of a variance.
11. Architecture:
a. Provide exhibitsfrom /-465 consistent with 23B.9.G (computer enhanced
perspective renderings and golden section exhibit).
b. Provide illustration showing compliance with 23B.9.C (70% of building fayade area
covered by glass).
12. Per Section 23B.17A of the Ordinance please forward a letter with a site plan of the
proposed DP to the Commissioner of Indiana Department of Transportation advising him of
the proposal and requesting any comments.

LETTER OF APPROVAL
The undersigned has reviewed the proposed plans for
Clarian Health Hospital
and has approved the plans subject to the following:
1. Our office requests a meeting to discuss the location of the fire department connection(s). We
are also requesting that an exterior entrance be provided to the riser room for our access.
2. We are requesting that the fire alarm enunciator panel be located at the main entrance of the
facility.
3. Our office is requesting more detail regarding the underground parking that is depicted on the
building elevation drawings, including the size, levels and approximate parking spaces planned.
4. Will the building have a basement, other than the parking facilities?
5. Is it known yet if the building will be required to have a fire pump?
6. As the building is over four stories in height, the building code will require a standpipe system in
the stairwells. We will also request that this system be expanded to include the underground
parking garage.
7. We are requesting the installations of Knox boxes at key areas of the facility to be determined
once more detailed plans are available. Also we are requesting the purchase of Knox fire
department connection caps for the system exterior hook-up(s). Applications for the Knox
products are available through our office.
Please respond to the above noted condition(s) in writing and submit to our office prior to the
scheduled Technical Advisory Meeting for this project.

This letter serves to acknowledge receipt of a transmittal received 10/10/02 containing the plans
for the above-mentioned project. After reviewing the plans the Highway Department has the
following comments:
1. Please show the location of the regulated drain storm sewer that is a part of the 10ath Street
and College Avenue intersection Improvement project. The location of the outlet must also be
shown.
2. It appears that many trees are shown in the location of the regulated drain. the Highway
Department asks that none of the trees be installed untO the new drain has been installed.
3. College Avenue is under the jurisdiction of the Highway Department, improvements to
College Avenue will be required. The extent of the improvements will be detennlned after the
Traffic Study has been reviewed. Please forward of copy of the study to my attention.
4. Please note that decorative signs, sprinkler systems, trees, landscaping mounds, light poles
or other such amenities are not permitted In the right of way.
5. In accordance with the HamRton County Thoroughfare Plan. additional right of way is
required along College Avenue. A 45' half right of way is required at this location. A 50'
triangular cut is also needed at the Intersection, connecting the right of way for Pennsylvania
Street and College Avenue.

Urban Forester Notes
Carmel Clav Historical Society: There are no objections to the Use Variance, but no landscape
plan has been submitted with the Development Plan. Comments will be made after a landscape
plan has been submitted.
Lakeside Park. Section One: The Landscape Plan for the Secondary Plat and Construction Plans
does not meet the Open Space Plan, Landscape Plan, Buffering requirements, and Tree
Preservation Plan commitments submitted for the approved Primary Plat received by this office
on August 29, and September 10, 2002. The Secondary Plat plans and Construction plans must
meet, or equal these approved plans.
Sheet 6 shows the installation of a storm sewer between yard inlet 159 and yard inlet 160. This
sewer installation bisects a tree preservation area (Common area #1). The installation by
trenching through this tree preservation area is not acceptable. Sewer installation through tree
preservation areas must be accomplished by tunneling to sufficient depth not to injure trees in the
woodlands; otherwise, drainage should be accomplished by grading, earthwork, or trenching
outside the tree preservation areas. These woodland preservation areas are primary natural
openspace conservation areas.
Lakes at Havden Run. Section Two: Four "street trees" shown along Lorenzo Boulevard on the
Primary Plat are missing from the Secondary Plat landscape plan for Section Two. These need
to be replaced. The developer's commitment to plant at least one large maturing shade tree near
the street but outside the right-of-way in the front of each lot not on Lorenzo Boulevard (a county
thoroughfare) must be included in Section Two as it was in Section One. The individual amounts
for species are missing from the submitted landscape plans. These must be added to the plan.
The Primary Plat Landscape plans show the woodland on the north perimeter being saved in its
entirety. The Section Two plans show an encroachment into the woods by lots, a drainage
easement, and a 6' asphalt path. Sheet 22 shows Tree Preservation Detail F (fencing), but there
is no corresponding fencing on the Landscape Plan. This woodlands in its entirety is a primary
conservation area on the open space plan for this subdivision as submitted to the Carmel Clay
Plan Commission on October 21,2001, and subsequently approved. This issue must be
resolved before this plan can be approved.
Fence row or native trees along the north-east perimeter should be preserved by utilizing tree
protection measures as well.
Villaae of West Clav. Helen Wells Buildinq:
The Villas at Sweet Charity Farms: Landscape plans must include planting notes and graphic
planting details. Please also include a diagram detailing required bufferyard plantings and
proposed plant numbers. Any additional shade trees committed to be planted in the front yards of
lots must be near the street but outside the right-of-way. Please detail the species of the 102
shade trees committed for the 51 lots on the landscape plans.
Additional plant materials for buffering should be added behind Lots 1-7 and along the path
between Benevolence Court and Beaument Boulevard South to screen views into the PSI
property. More should be added along the east perimeter south of Lot 27. It my suggestion that
the open drainage on the south east corner be graded outside the existing strip of brush and
woods.

Laura Vista Subdivision: Due to the wildlife in this area, connectivity of the common areas should
be a priority. The US Fish and Wildlife Service has acknowledged this riparian corridor (forested
floodplain) as an ecologically sensitive area (possible habitat for the federally protected Indiana
bat) that should not be disrupted. A written tree preservation plan needs to accompany the
secondary plat including graphic details of the tree and all woodland protection measures.
Wooded common areas need to be listed as tree preservation areas on the plat and in the
neighborhood covenants. Requirements and allowed activities must be detailed for tree
preservation areas within the neighborhood covenants.
If a monument sign is proposed, a plan of the sign the required landscaping must be submitted.
There has been no landscaping plan submitted at all. If one is not proposed, a written
explanation must be provided. I will be glad to meet separately and on site if needed. Shade
trees should be planted along the portion of the multi-use path from Laura Vista Drive west to the
woods. (Path on plat?)
Clav Terrace: There have been significant changes between the Rezone PUD plan and the
Development plan currently submitted. The current plan does not include any landscaping, tree
protection, or reforestation plans as required with the Rezone application. Comments will be
made when those are submitted. The areas to plant the required landscaping and reforestation
seem to have significantly lessened due to changes in storm water detention and change of
layout.
Motorcvcles of Indiana: No change of use?
Cherry Creek Estates: No submittal
Providence at Old Meridian: Correct mistakes on plant schedule. No Austrian pine, no red
maples, no cockspur thornless hawthorn, change Malus Coral Burst to recommended species.
American Standard for Nursery Stock (1996) not ANSI. Significant changes to PUD development
plan - doesn't match original plan (Mark Timmons) or installed (Rick Riddle). Label all plants
individually and give #'s in schedule. Planting details - plant trees with root flare at grade level
(not same as nursery), don't pile mulch up on trunk of tree. Remove large dead Oak at entrance
and others that are risk trees. Donate large White Oak sections for community use. Plant
evergreen screen on Arbors border as agreed when development plan approved. Areas of
constructed soils?
Clarian Hospital North: Haven't had sufficient time to make a complete review. Construction
causes a removal of a significant section of trees. No planting schedule, needs table of
requirements vs. plan, NO Austrian Pines, NO Pin Oaks, replace Tilia cordata with Tilia
tomentosa. Planting details - flare root at grade level. Why planting pits for containerized trees?
Suggest constructed soils.
Carmel Transportation Facilitv: Correct mistakes on plant schedule. No Austrian pine, no red

District's Sanitary Sewer Specifications. Revised plans need to be submitted including the
District's specifications. A preconstruction meeting will need to be held before the
construction of the proposed sanitary sewers. Building permits should NOT be issued
unless the District has issued connection permits.

The plans received by the District do not contain the most recent version of the District

Hello everyone:
Following are CTRWD comments on the agenda items effecting our sanitary sewers or service
area for the October 16, 2002 TAC meeting. Please feel free to copy and distribute this
email or attach it to your meeting minutes. We have also created a new page on our
website linked to the "Construction" page where we have posted all CTRWD TAC comments from
2001 and 2002 meetings held to date. We will continue to add to this page every month so
please let everyone know they can view our comments online now too.
Lakeside Park, Section One (Secondary Plat and Construction Plans)
The plans received by the District do not contain the most recent version of the
District's Sanitary Sewer Specifications. Revised plans need to be submitted including the
District's specifications. A preconstruction meeting will need to be held before the
construction of the proposed sanitary sewers. Building permits should NOT be issued
unless the District has issued connection permits.
Lakes at Hayden Run, Section Two (Secondary Plat and Construction Plans)
The plans received by the District do not contain the most recent version of the District
specifications. The plans contain two sheets labeled Sht.13. On sheet 13 the sanitary
manhole numbers do not match on the plan and profile. Storm sewers are not shown on the
sanitary plans and profiles. Final revisions will be required before the District will
submit for an IDEM permit. A preconstruct ion meeting will need to be held before the
construction of the proposed sanitary sewers. Building permits should NOT be issued
unless the District has issued connection permits.
Village of West Clay, Hellen Wells Building (Construction Plans)
The plans received by the District do not contain the lateral specifications. An easement
will need to be obtained for the portion of the lateral that is not on the above property.
Final revisions will be required. A preconstruct ion meeting will need to be held before
the construction of the proposed sanitary sewers. Building permits should NOT be issued
unless the District has issued connection permits.
The Villas at Sweet Charity Farms (Primary Plat)
The plans received by the District do not contain the sanitary sewer profiles. Revised
plans will be required. A preconstruct ion meeting will need to be held before the
construction of the proposed sanitary sewers. Building permits should NOT be issued
unless the District has issued connection permits.
Motorcycles of Indianapolis
The plans received by the District do not show any changes to the sanitary sewer. If
changes are required, revised plans will need to be submitted to the District for review

We have reviewed the construction plllDS and submitted to the Hamilton County Surveyor's office on
August 20, 2002 for the Meridian Corporate Plua and the revised plans submiued on September 12, 2002.
This site wUl affect the Clara Knotts HlID1iIton County Regulated Drain. All storm water discharge to
R.egulated Drains requires the County Surveyor's approval, permitting and inspection. The comments are
for the pond relocation project unless otherwise noted.
1. Please provide the signed fonn to make the storm sewers regulated.
2. Provide the signed Non-enforcement Pennlt form with the appropriate ~heck.
3. Submit original letters of credit or bonds for the construction estimate.
4. Provicl.e the petition for reconstruotion and relocation.
