. A check in the amount of Three Hundred twenty and 50/100 Dollars ($320.50) made
payable to the City of Carmel.
. Materials sample board/finish schedule accompanied by storefront drawing(s) or rendering

If it helps the pantone colors for the "revol green" is 7496C. And for the dark grey I believe it is
Pantone Black 4U.

signage, has been approved for preliminary design concept. As directed by your
tenant coordinator please forward the following files to 3rd Works so that we can
prepare the required storefront renderings on your behalf for review by the City of
Carmel ADLS staff:
. Drawing File Type: Autocadd 2000; Storefront drawing with base
building drawing in background (Base building drawing must be on one
layer and a single block)
. Specify Exterior Materials and Colors: Window frame, storefront base,
door hardware, canopy/awning, signage
. Provide Material Finish Board with Legend for presentation at ADLS
Meeting
. Provide Tenant Information: DBA Name, Address, Unit Number,
Frontage Dimension
. Provide Signage Information: Font type and Style, Letter height, Logos
(Illustrator format), Colors
. ADLS application form completed and notarized
. Application fee for $320.50, made payable to the City of Carmel.
Upon receipt of the electronic files, 3rd Works will confirm back to you the
anticipated submission date to the City of Carmel and will outline additional

Bill:
The landlord has approved the storefront design in your construction
documents recently submitted. (The landlord approved drawings will be
forthcoming). We now need to get the package in to City of Carmel for ADLS
(Architectural Design Lighting and Sign) review. Please note, the City of
Carmel will not issue a building permit until ADLS approval is granted.
The landlord is helping to coordinate this and using our consultant, 3rd- Works,
to assist as well. 3rd- Works will prepare the color rendering of the tenant
storefront in context with the entire building. Please see the text below on what
will be needed for submission to Carmel.
You should now proceed with preparation for ADLS submission. Please
contact Nalin Yosek@ 3rd Works (nyosek@3rd-works.comor216-685-9800)
to discuss the ADLS submittal for the City of Carmel. Nalin is coordinating all
ADLS submissions. I've attached some notes on what Nalin will need in order
to attach Revol's storefront drawing to the Landlord rendering that will be
submitted to the City of Carmel.

Upon receipt of the ADLS package, 3rd-Works will send it to me and I
will make an appointment with the City representative for review. Once I
