The following issues must be resolved prior to signature of the plat by the director: 1. Change title to read, "She1bourne Park Repljt of Section One." 2. Hatch the site area on the key map. I 3. Add Primary Plat Amendment Docket # (105-02 PP Amend) and the Secondary Plat Docket # that we will assign once an application is r~ceived. 4. Verify the source of title. Is there not a mor9 recent deed? 5. On page I, redraw the lake to show the expansion onto new area. 6. Please explain the modification in lot numtJring (i.e. 4A and 23A as opposed to the numbering identified in the primary plat.) I 7. The monuments and markers should be as required under Section 8.2 of the sea. I a. Add concrete monuments at all corners that form the boundary of the plat, as required under sea 8.2.1. Please submit a revised Secondary Plat with the above-mentioned changes. Once these changes have been made and comments addressed, please produce a ritylar copy for signature by this Department. Two paper copies of the plat must be filed with the mylar pribr to recording. I will return the signed mylar to you for signature by the County Commissioners. Once tecorded, our Department needs three paper copies and a mylar of the recorded plat. The erosion and sediment control plan submitted was not teviewed for the adequacy of the engineering design. All practices included in the plan, as well as those recommended in the comments should be evaluated as to their feasibility by a qualified individual with structural practices designed by a qualifik engineer. The plan has not been reviewedfor local, state, I or federal permits that may be required to proceed with this project. Additional information, including design calculations may be requested to further evaluate the erosion and sediment control )ran. I The erosion and sediment control plan has been reviewed and it has been determined that the plan: I X Satisfies the minimum requirements and intent of 327 AC 15-5 (Rule 5). Notification will be forwarded to the Indiana Department of Environmental Management. Proper implementation of the erosion and sediment control plan aAd inspections of the construction site by the developer or a I representative are necessary to minimize off-site sedimentation. Tre developer should be aware that unforeseen construction activities and weather conditions may effect the performance of a ~ractice or the erosion and sediment control plan.
