The approved set of plans that are to be maintained on the construction site are now available in our office. Please arrange to have these plans picked up. There are still two requirements that have not been completed: . Payment of Water and Sanitary Sewer Connection Fees in the total amount of $27,996.00. I am enclosing a copy of our calculation sheet for our use. Payment has been deferred with the approval of Carmel Utilities. Water meters will not be installed until Connection Fees have been paid for this development. . Dedication of additional right of way, including Old Meridian Street, Guilford Road and the proposed roundabout at Old Meridian and Guilford through our Board of Public Works and Safety. Right of way dedication will be required prior to the issuance of a Certificate of Occupancy (e.O.) for the site. Also, it appears (from the drawings) that you are planning to open cut Guilford for gas service. Please be reminded that open pavement cuts in dedicated streets require Board of Public Works and Safety approval prior to the commencement of construction. If you have questions, please call. Resolution of these final issues will result in approval of the plans for this project. We will require a minimum of 5-sets of final approved construction plans. Carmel Utilities will receive 2-sets, our Public Works Inspector will receive I-set, I-set will be maintained in the Engineering Office and the final set is to be maintained on the construction site. Item #11 Updated Old Meridian Plans should be incorporated. Median break on Old Meridian will be centered on St. Vincent's new Ambulatory Center Drive. When will accelldecel design be complete to the point where they can be added to the drawings? Please provide an entrance detail on the plans, Item #17 Roundabout right of way has not been indicated. Right of way should be shown in accordance with commitments attendant to approval of rezone petition Z-389-02. Item #21 It is our understanding that this development will be responsible for the cost of the path on Old Meridian, whether they are installed with this project or as part of the Old Meridian project. Item #26 Pond outlet (Structure 8) has conflicting TC Elevation between table on C200 and Detail 7, C601. Other It was our understanding that an emergency inlet/outlet would be provided to Guilford. Please indicate on the plans. We the undersigned, represent a majority of the residences that own homes on Guilford Avenue and/or Wildwood Drive. Our concern stems from what we see as a number of modifications to a comprehensive plan (the Old Meridian Corridor plan) that was passed with much debate and expense. The debate was proper and constructive and rightly included those of us that are neighbors to the area. The city hired a talented architect from Scheer and Scheer Inc. Cincinnati to assist in the foonulation and presentation of the plan. The plan was passed a short time ago. Now as the frrst developments in the designated area are being planned and presented, this Commission is being asked to modify the plan. Some modifications are substantial some may not be considered substantial. However, in their entirety, they represent a morphing of the original plan to a final product that will likely look and feel different from the original vision. Specifically, one such modification is of great concern to us. During the OMC planning process, our neighborhoods expressed great concern about the future of the property bordered by Old Meridian on the west, Guilford Ave. on the east and Main street (131 st Street) on the south. Much to our surprise, we were asked to participate in the process. We truly appreciated having our concerns and input heard. After the initial plan was established, we asked the architect to visit our neighborhood. We wanted her to see first hand the proximity of our street to the OMC area and discuss how our residential street could coexist with the planned development. Ingress and egress on Guilford along with the type and look of neighboring development was our biggest concern. Our first meeting concluded with our group having a better understanding of the OMC goal and we scheduled second meeting. In both cases the architect visited us in our home during the evening hours. After the two visits she was convinced that Guilford access was an issue and that the traffic should be directed to the boulevard that was going to be upgraded to accommodate the commercial traffic. Eliminating a commercial use curb cut on Guilford also worked to enhance the integration of the Meadow Lark Park trail with the OMC area and the schools. She also understood the safety issue of adding a commercial use curb cut on Guilford. She made modifications to her plan, the OMC plan was submitted and was passed. ;'I.". When the developers for the "Spine Institute" TWO building project was proposed, the developers showed a curb cut on Guilford. Our neighborhood raised the issue of safety and moving traffic to the road that would best handle the commercial traffic, that road being Old Meridian Street. In 1992 the city agreed with our assessment and the curb cut was eliminated. The committee should be reminded that the original "Spine Institute" proposal was for TWO buildings. This past spring Mayor Brainard was gracious enough to meet with us regarding the OMC project. We asked to see the Mayor because a number of us were considering substantial home modifications both in scope and expense. Mayor Brainard asked Mike Hollibaugh to attended. During our meeting Mayor Brainard reiterated the city's commitment the OMC project as it was passed. We left that meeting again feeling good about the plan and how our neighborhoods could coexist within the area. Now you are receiving some of the first proposals for development in the OMC area. One such proposal is the Carmel Ambulatory Surgery and Endoscopy Center, proposed as 13421 Old Meridian Street. This proposal is essentially for the second building of the site although it has been modified from the original plan of having the mirror image building of the original Spine Institute building. The developer, consistent with the OMC plan is NOT proposing a curb-cut on Guilford Ave. However, much to our disbelief, a Department of Community Services employee is asking for a curb cut. This is unbelievable to us. We can only conclude that there is some mistake or the Mr. Dobosiewicz is unaware ofthe historyofthis issue or does not take the careful planning of the original OMC very seriously. We were certainly hopeful that after the city of Carmel agreed with our position on two prior occasions and a reaffirmation of the city's commitment to the project by the Mayor, we could rest easily about the process of OMC development moving forward. Apparently that may be a mistake. We are hopeful that this issue can be put to rest once and for all. Over the past 10 years our neighborhood has been inundated with development around our homes. On more than one occasion we have had to come before this body (Carmel Clay Planning Commission) to report about promised landscaping plans that have been abandoned. On one occasion this body had to impose a bond requirement to hold the developer to the original landscaping plan. North Entrance along Old Meridian 2. South Entrance Along Old Meridian 3. Wildwood Entrance along Guilford (attached) All three entrances operate satisfactorily under existing conditions. With the potential addition of a third access along Guilford, site traffic was redistributed to evaluate this impact on the Wildwood/Guilford intersection. The site traffic expected to utilize this access will not have a significant impact at this intersection. The Wildwood! Water and Sanitary Sewer Availability. . Right of Way Dedication. I am enclosing dedication documents approved by the City of Carmel. Please use these for the formal dedication of the additional right of way for Old Meridian. These documents may be sent electronically if you desire. . Commercial Curb Cut. Please provide an exhibit that shows the location of the curb cut and acell/decellanes if applicable; appropriate dimensions including drive width, overall width at the street, aceWdecel dimensions, radii, etc.; and location of existing opposing curb cuts, if applicable. Final drawings will not be approved for construction until all issues have been resolved. All Carmel Utility issues must also re resolved. The design engineer must certify all drawings submitted for final approval. This office will require five-sets of approved drawings after all issues have been resolved The drawings will be stamped approved and signed by both the City Engineer and Director Carmel Utilities. The Developer/Owner will receive one-set which is to be maintained on the construction site at all times. Carmel Utilities will receive two-sets. Our Public Works Inspector will receive one-set and one-set will be maintained in the Department of Engineering. If more than five-sets are requested to be approved, the additional sets must be submitted with the five required sets. 6. Please be advised that any installation of signs, walls, irrigation systems, etc. within dedicated right of way and/or dedicated easements will require a Consent to Encroach Agreement between the OwnerlDeveloper and the City of Carmel. This Agreement requires Board of Public Works and Safety approval. 7. Carmel Utilities does not subscribe to "Holey Moley" and should be contacted directly for all water main and sanitary sewer main locations. 8. Carmel Utilities will provide a sepamte review of this project for water and sanitary sewer issues. 9. I am including copies of the following with this correspondence: . Upon initial review, it appears the following bonding requirements will apply to this project: Performance Guarantees Performance Guarantees will not be required for this project. Right of Way Permit and Bonding Requirements Any work in dedicated right of way will require a Right of Way Permit and a License and Permit Bond. The bond amount will be determined by the number of instances of work in the right of way (curb cut, storm sewer work, etc.) @ $2,000.00 per instance. It appears that all right of way work involves the existing south entrance. Based upon the scope of work at this site (demolition, repaving, storm pipe, gas line), we request that a $8,000.00 bond be posted for this Permit work. Permit forms are available from this office. There is no charge for the permit. PROJECT COMMENTS \ RoadslRWlPavement 11. Is one of the two entrances off of Old Meridian planned to be the "main" entrance? If so, we would ask that to the extent they can be worked in with the Old Meridian Road Plans, accelemtionldeceleration lanes be added. 12. On Sheet CIOO, a "proposed 40 foot r.o.w." is indicated on Old Meridian. Should be 60 feet. 13. All proposed drives should align with existing or proposed drives within the St. Vincent's Master Plan. Indicate on site plan (in light type) location of existing drive. Label all others as existing or new (relocated) drive. 14. Indicate or note where new utilities going in will require pavement cuts (in dedicated streets if applicable). On Sheet C200, it appears that an open cut across Guilford for the gas line is planned. All open pavement cuts in dedicated right of way require prior Board of Public Works and Safety approval. We will require the contractor to mill and resurface the cut in an acceptable manner and in a total area as recommended by this office and approved by the Board of Public Works and Safety. 15. Sheet C300- The new right of way asphalt should extend all the way to the new, proposed 60-foot half right of way line. 16. Sheet C300 - The 40-foot half right of way of Guilford is labeled as proposed. Has this right of way already been dedicated? If so, do you have access to the original dedication documents? 17. Show proposed roundabout, proposed right of way take for Old Meridian project (@ corner Guilford) and the proposed Old Meridian layout (label as Proposed Old Meridian Layout-by City) including median cuts and proposed St. Vincent's entrances on the west side. Indicate the proposed lines by lighter or dashed line type. Our office can assist you in obtaining the designs for that project, as prepared by Butler Fairman Seufert. hl addition, show. 18. Sheet C600 - The City of Carmel R.O.W. Asphalt Detail must match the pavement section to be utilized on the Old Meridian project, a copy of which is enclosed with this correspondence. 19. It appears that a portion of the existing sidewalk along Guilford is constructed outside of the right of way. This "sliver" between the detention pond and Guilford may eventually need to be reconstructed to be within the right-of-way, or additional right-of-way dedicated to include it. For the purposes of this review, no changes are required to be made at this time. 21. The Alternative Transportation Plan (A TP) indicates 10-foot asphalt paths are required along this portion of Old Meridian. Drainae:e: 22. Sheet C200 - Indicate storm pipe sizes, material, slope, inverts and top of casting. Indicate pipe material on water and sanitary lines. 23. It is our assumption that the storm sewer system will be private. 24. Please show on at least one drawing, the location of the new versus the old parking lot limits. This will help to understand the changes to the site and how it impacts the drainage calculations. 25. Within the drainage calculations, you indicate that the predeveloped conditions are residential and open field. This would not be the correct way to label the initial conditions. Pre-developed conditions would be considered the existing, prior to this prooosed development, not prior to all development. Therefore, the initial conditions should indicate the buildings and parking lot as currently found on the site. Parameters used in the drainage calculations should reflect these conditions. 26. Clarify both pond outlet structures on Detail Sheet C60l. . Shows conflicting size outlet pipes. . Conflicting locations of orifice opening on side and front views. . Conflicting Elevations. 27. Please clarify the information for Structure 8 outlet pipe from the dry pond) on all sheets: . Is it a 12" outlet pipe with 8 " orifice inside pipe? . Does it also have a 6" orifice opening in structure? . What is the elevation of the 6" orifice, 838.00 or 840.00? 28. The drainage report is confusing in the mentioning of the maximwn stage of both ponds. It indicates that there is a 2" depth above the spillway for the dry pond and a 6" depth above the spillway for the wet basin for a 100 year stonn, yet it mentions the maximwn stage for the 100 year elevation to be at lest 1 foot below the emergency spillway elevation. Is this a typo in the report? Why have an open grate elevation for the outlet structure (of the dry pond) if the spillway plus the 2" is still below this? Is it assumed this grate elevation will be reached? 29. Please show the outlet pipe with all information (size, elevations, etc.) from the existing detention basin. 30. Is any swale, underdrain or other created flowline between the inlet pipe and th~ outlet structure within the dry detention basin planned? 31. Why is the existing control structure from the wet basin being reduced in size from 60" to 48"? 32. Please provide a summary of the parameters of the before and after conditions, i.e. acreage existing, curve nwnber, time of concentration that are used in the calculations. Do not resend the printout of the hydrographs. With the above summary, we would like a copy of the basic drainage basin map, showing current basinslland cover versus proposed basins, storm sewerslland cover. Please share these comments with all persons to be associated with this project including the OwnerlDeveloper, Contractors, etc. The above comments are based upon the Engineering Department's initial review. Please provide our office with a follow up letter and revised plans, indicating all revisions. Second and subsequent reviews will focus only on revised items. On the Site Demolition Plan there is a "limit of construction" line but no indication of where tree protection fencing will be placed. On the south perimeter it appears that the construction will go to the parcel's edge and into the tree line. Tree protection fencing needs to be placed outside the tree line on the south property line to prevent injury and death to the adjacent trees. The location of perimeter tree protection fencing shall be shown on the plan. On individual trees to be protected, a minimum diameter from the trunk should be specified (or actual 'dripline' distance listed) on the plan for tree protection fencing, or included as a separate document. 2. TREE PROTECTION AND TRIMMING PROCEDURES AND RESPONSIBILITES are referenced in the first note. These need to be on the plans, or in a separate written document, and submitted to this office. 3. There are no planting notes or specifications on the plans. 4. Total number of plants for each species is to be included in the plant schedule. 5. Thornless Cockspur hawthorn is not recommended because of disease and insect pest pressures in this area. An alternative should be chosen. A large maturing shade tree would do well in this location (large soil volumes for roots), species such as london plane tree, oak species (not pin oak), tulip tree, black gum or ginko are acceptable. 6. Some ofthe relocated trees on the southeast comer appear to be located on the plan within the detention pond. This should be corrected. If the pond will hold water continually, the perimeter should be landscaped. 7. Relocated trees that die will have to be replaced as part of ADLS commitments. 8. On the east perimeter there is a section of buffering landscaping that it appears from the plan, is being removed during construction and not being replaced. This section needs to be replanted with plant material similar to the original upon completion of the construction project. 9. Tree planting detail 01: the note which says "GRADE AT WHICH TREE GREW AT NURSERY" shall be changed to read "PLANT ROOT FLARE AT GRADE LEVEL". The note on removing the burlap shall match the note in Detail 02 which reads "REMOVE UPPER 1/3 OF BURLAP AND REMOVE ANY BINDINGS FROM THE STEM." A note needs to be added that states "WIRE, HOSE, AND WRAPPINGS ARE TO BE REMOVED AFTER ONE YEAR." Please reply to these comments in writing and by amended plan. Comments: 4A. The instructions for the demolition plan are direct and site specific. The same type of instruction is needed for the erosion control sequence. Include in the sequence only those practices that will be used on this site. 48. The maintenance instructions for the silt pit are excellent. Additional information should be added to silt fence and catch basin details. The "Indiana Handbook for Erosion and Sediment Control in Urbanizing Areas" has maintenance criteria for each practice. I will send the appropriate pages if you don't have a copy of the manual. 4C. The proposed sediment basin could be the primary sediment basin for the project during construction. The basin needs to be identified in the construction sequence and would need to have a suitable outlet. A slotted riser makes a good outlet for a dry basin. A temporary pipe could be run from the basin to the existing catch basin to the north. The two silt pits are not needed if the larger basin is installed early in the construction process. 4D. Seeding needs to be spelled out in the construction sequence. If this project is started this fall, typically the basin will be seeded and protected with EC blankets. Other disturbed areas will not be seeded until after the building is completed and the plantings installed next summer. Following is a response to each request: Building Footprint The building footprint of 32,903 square feet is approved with the condition that the entire fa<;ade facing Old Meridian Street will be two stories. Parking Plan The proposed amount of parking (383 spaces) is approved. The Department reserves the right to further review the need to provide access to Guilford Road, the alignment of the relocated drive cut onto Old Meridian Street, and alternative parking lot designs as part of the Architectural Design, Landscaping, Lighting and Signage approval (ADLS). In addition to these comments, it has come to our attention that there are several individual parcels involved in this site. At the southeast corner of the Spine Institutes parking lot there would be sufficient area that could be used as an access point of Guilford Road. A few trees might have to be removed or relocated and a few parking spaces might be lost, but an entrance at this point would save time in our responding to the buildings. It would be the request of our office, if allowed by the County Highway Department or City Engineer, that an access road be constructed off Guilford Road. With the impending improvements to Old Meridian Street, which will tie up traffic, this would seem the time to address this alternate access to the Spine Institute and the proposed Ambulatory Surgery Center. Any work to be performed within the right of way of Old Meridian will need to be permitted through this office. Please contact Mr. Robb Chadwell, Permit Inspector, for the necessary forms and information. 2. The Highway Department request the dedication of right of way In accordance with the Thoroughfare Plan.
