RE: Sunrise Senior Living construction plans-Project Review #1 Correction
Dear Ms. Varga:
We misspoke in our original review letter and indicated that this project was in the service area for the Clay
Township Regional Waste District for sanitary sewers. It is actually in the service area of Carmel Utilities.
Please accept the following revisions to our review letter dated February I, 2006.
GENERAL INFORMATION
2. Jurisdictions:

3. Board of Public Works and Safety Approval Requirements: (such approvals are required prior to
drawing approval).
. Water and Sanitary Sewer Availability approval:
Water and Sanitary Sewer Availability Approval request to the Board is based upon
EDU requirements as determined by the proposed use of the building. Water and
Sanitary Sewer Connection Fees will be based upon EDU calculations.
4. TAC Review/Drawings submitted for approval:
We request that all comments and comment letters generated by this office be answered in writing
and be accompanied by a drawing reflecting requested revisions. Final drawings will not be
approved for construction until all issues have been resolved. The design engineer must certify all
drawings submitted for final approval. This office will require a minimum of five-sets of
approved drawings after all issues have been resolved and the construction drawings have been
approved. The drawings will be stamped as approved and signed by the City Engineer and by the
Director of Carmel Utilities. The Developer/Owner will receive one-set which is to be maintained
on the construction site at all times. Carmel Utilities will receive two sets. The Engineering
Department Construction Manager will receive one-set and one-set will be maintained in the
Department of Engineering. If more than the minimum of five-sets are requested to be approved,
the additional sets (maximum total of two additional) must be submitted with the five required
sets.
6. Please provide Carmel Utilities with all initial and subsequent drawings submitted for
review. They will provide a separate review regarding these issues. Please submit to:

II. Water and Sanitary Sewer Availability (acreage) Fees are required to be paid after Engineering
Department approval of the project's construction plans and prior to obtaining a Building Permit.
Availability Fees are based upon total platted acreage or legal description acreage at the current
rate of$I,01O.00 per acre for Water and $1,365.00 per acre for Sanitary Sewers, subject to future
revisions. Based upon the legal description on the plans, the preliminary Water and Sanitary
Sewer Availability Fee calculations are:
Water -4.781 Acres @ $1,01O.00/Acre = $4,828.81 or $4,829.00
Sanitary Sewers - 4.781 Acres @ $1,365.00/Acre = $6,526.07 or $6,526.00
12. Water and Sanitary Sewer Connection Fees are required to be paid after Engineering
Department approval of the project's construction plans and prior to obtaining a building permit.
Connection Fees are based upon the EDU calculation of the proposed usage of the building at the
current rate of$I,31O.00 per EDU for Water and $795.00 per EDU for Sanitary Sewers, also
subject to future revisions. It appears that the Institutions Other Titan Hospitals classification
from our Equivalent User Contribution Multiplier tables would apply to this project:
. A total of 178 beds based upon 102 I-bedroom units and 38 2-bedroom units.

Dear Ms. Varga:
We have reviewed the construction plans submitted for this project for the January 17, 2007 Technical
Advisory Committee meeting. We offer the following comments:
GENERAL INFORMATION
I. The project site is currently within current City of Carmel Corporate Limits.
2. Jurisdictions:
. Street and Right of Way -City of Carmel, Old Meridian Street
INOOT, U.S. 31
. Water-City of Carmel Utilities.
. Sanitary Sewer- The project is within Clay Township Regional Waste District service area.
. Storm SewerlDrainage - City of Carmel. The Hamilton County Surveyor's Office has
jurisdiction over legal drains and discharge to legal drains.
3. Board of Public Works and Safety Approval Requirements: (such approvals are required prior to
drawing approval).
. Water Availability approval:
Water Availability Approval request to the Board is based upon EDU requirements
as determined by the proposed use of the building. Water Connection Fees will be
based upon EDU calculations.
If entryway or other irrigation systems are planned, additional Water Availability
Approval from the Board will be required and additional Water Connection Fees will
be assessed based upon the size and usage of the system.
. Commercial Curb Cut-Please provide 8~ x II exhibits to accompany the request for
commercial curb cut approval. Include all pertinent data including radii, drive width at throat
and street, opposing drives etc.
. Temporary Construction Entrance-Board approval is required if the temporary entrance is
located at a site other than the existing approved curb cuts. Please provide exhibits as detailed
above, if applicable.
. Open Pavement Cuts in dedicated streets (does not appear applicable).
. Secondary Plat approval if applicable.
. Dedication of right of way. The City requests fee simple transfer to the City of the portion of
the property still described to the centerline encumbered by existing right-of-way.
. Consent to Encroach Agreements for any permanent improvement to be installed within
dedicated right of way and/or dedicated easements.

BONDING REOUlREMENTS

accepted and the Performance Guarantee is released by our BPWS, 3- Year Maintenance
Guarantees will be required in the amount of 15% of the Performance amount for streets and curbs
and 10% of the Performance amount for all other improvements.

requirements for erosion control.
10. We have engaged Crossroad Engineers, PC to review all drainage plans and drainage calculations
submitted to this office for review. We will share Crossroad's comments as they are received.
When construction plans are submitted for review for this development, please provide a set of
drawings and drainage calculations directly to Crossroad.

DEVELOPMENT PLAN DRAWING COMMENTS
13. A seven-page Plan Certification Form is enclosed with this letter. This form was created with the
desire and intent to streamline the plan review process. We request that you please complete this
form and return the original of this form along with complete 24" x 36" civil drawings to the
Deparbnent of Engineering as soon as possible. Comments will be generated after the Plan
Certification form is returned. Please treat each item on the form as a requirement of the
Department of Engineering. Providing a ''no'' or ''N/A'' response to any of the items does not
relieve the Developer of the responsibility of meeting the requirements of the Deparbnent of
Engineering. The Deparbnent of Engineering will make the final determination as to the
acceptability of any ''no'' or ''N/ A" responses.
14. Please indicate the City's road plans and indicate all above and below grade improvements from
the road project on these plans. The end of the project is approximately 500 feet southeast of the
Pennsylvania/Old Meridian Street intersection with an additional 1 00 feet of incidental
construction.
1 5. Any off-site easements necessary for installation of utilities and storm drainage must be secured
prior to the Deparbnent of Engineering approving the drawings. Please confirm if easements exist
to the north for water, sanitary, and storm services that are in favor of this site.
16. All on-site curbing shall be per City standards.
17. ADA ramps are required at all locations where the sidewalks or asphalt paths cross the streets
and/or drives. The current entrance does not indicate such ramps.

18. Please delineate any curb cuts on the plan for sawcut, removal, and replacement.
19. Please add the following notes to the sidewalk detail: "SPACING BETWEEN TRANSVERSE
JOINTS SHALL NOT EXCEED 1.25 TIMES WIDTH," "SIDEWALK BUILT WITH
DRIVEWAYS SHALL HAVE MINIMUM THICKNESS OF 6" OR THICKNESS OF
DRIVEWAY, WHICHEVER IS GREATER," and "JOINTS SHALL BE TOOLED OR SAWED
TO A DEPTH OF ~ THE SIDEWALK THICKNESS."
20. Please indicate dedication of the Right-of-Way prescribed by the City's 20-year Thoroughfare
Plan. Please label the right-of-way on all plan views. While we have discussed that, long term,
the City's need for right-of-way for improvements to this section of roadway beyond the existing
right-of-way is limited, compliance with the Thoroughfare Plan is still a requirement and a formal
variance must be obtained if additional right-of-way is not being considered for dedication.
21. As the drainage design progresses, the design shall accommodate detention volume for the fully
developed site and for the fully developed thoroughfare plan right-of-way for the perimeter road
frontages. This requirement shall apply regardless of watershed limits.
22. This project is subject to the Carmel Stormwater Management Ordinance, which was adopted
February 6, 2006. Please also confirm any restricted release rates with the County.
23. All Old Meridian Street drainage structures and bridges across the frontage of this development
must be indicated on the plans and replaced or improved by the developer to accommodate the
widening, auxiliary lanes, multi-use path, and any other required improvements to the property or
the roadway.
24. Please include the note "THERE SHALL BE NO WATER VALVES OR MANHOLES WITHIN
THE PAVEMENT, CURBS, OR SIDEWALKS, OR ASPHALT PATHS WITHIN THE RlGHT-
OF- WAY" on all appropriate sheets.
25. If it will be necessary to relocate existing utility poles, the expense of such relocation shall be
borne by the Developer. All utility poles must be located within one foot of the eventual right-of-
way line.
26. The Department requires that the construction drawings be developed in accordance with the City
of Carmel digital submission standards and that all required submittals for primary plat, secondary
plat, and construction drawings be made. The digital files must be submitted to the Department of
Engineering prior to approval of the construction plans.
27. The City's Thoroughfare Plan requires construction of half of the improvements prescribed by the
Thoroughfare Plan across the frontage. Typically, developers provide the money equal to the
value of constructing these improvements to the City for deposit in the City's non-reverting
Thoroughfare Plan fund.
28. On the cover sheet, please remove the "#" preceding 31 in "U.S. 31" on the Site Map.
29. On the cover sheet, please remove the disclaimer in the lower right comer. The Department of
Engineering will not approve the plans with this disclaimer.
30. On the cover sheet, please remove the titles of the Landscaping Plan & Details and Landscaping
Details & Specs sheets from the Index. Also please remove Sheet LI01, Sheet L801, and Sheet
L901 from the plans submitted for review and approval by the Department of Engineering.
31. Sheet C803 - please remove Subsurface Drain detail 6/C803 and replace with City of Carmel
standard detail. Add the note "ALL SUBSURFACE DRAIN SHALL BE DOUBLE WALL
HANCOR ID-Q OR EQUIVALENT."
32. Sheet C803, CURB JOINT DETAIL - Please revise such that the maximum transverse expansion
joint spacing is 50 feet instead of 100 feet. Please indicate that this is a maximum. Please revise
such that the maximum transverse contraction joint spacing is 10 feet instead of 20 feet on
tangents. Please indicate that the maximum transverse contraction joint spacing on radii is 5 feet.
33. Please contact Amanda Foley to review storm water quality requirements for this project.
34. Detai112/C803 - Please require "ribbing" where ADA ramps are proposed in the right-of-way.
35. A City standard depressed curb detail may be required dependent upon final entrance location.
36. Please replace detail 3/C803 with the City standard detail.
37. Engineering will support the proposed access from Old Meridian Street provided that it is aligned
with the existing entrance to Pinnacle Point and that it is understood that the City cannot guarantee
long term full access at this point given the potential for the INDOT to restrict some protion of the
roadway to one-way. If other access points are desired that have a higher potential for long term
full access, we would be happy to have a meeting to discuss options.

38. The existing driveway to be abandoned per Keynote "Q" shall be abandoned in accordance with
City code section 6-227 (hXIO).
39. There are utility specific notes on the site plan that should be turned offas they reference items not
turned on in the site plan.
40. Pavement within the right-of-way apparently references only the entrance drive, which shall be
concrete. A depressed curb may be required dependent upon final location of the drive in relation
to the end of the road project.
41. The City requests continuation of the road project curbing to the proposed entrance if the curbing
stops north of the final entrance location. Ifexisting curbing is required to be removed, please
indicate and require removal to the nearest joint.
42. Revise site note 10 to conform to City standards.
43. Please confirm with the Carmel Fire Department the need for two points of access.
44. Thank you for indicating concrete sidewalk across frontage. Please indicate connection to where
the road project walk stops. Please also confirm with Karyn Ryg if asphalt path or concrete
sidewalk is desired. Although the A TP requires concrete walk, the path is similar in installation
cost.
45. Entrance (at its final location) shall account for the final plans and elevations from the City's road
project.
46. The grading plan shall account for the final top - back-of-curb elevation from the road project.
47. No earth disturbing activity may commence until a storm water management permit is obtained.
Please add a note to this effect to the plans.
48. The City requests milling and resurfacing of the entire width of Old Meridian across the frontage
from the right-of-way of U.S. 31 to the end of project of the City's road project. (Station
12+297.305 "A" and any incidental construction).
These comments represent the Department of Engineering's initial review of the construction plans for this
project. We request that all responses to our comments be provided in writing. Failure to provide written
responses may result in the delay of the review process.
It is critical that our office be made aware of all modifications made on the plans being resubmitted,
particularly if any such changes are considered ''new'' or fall outside of our previous reviews. Please
provide revised plans including all revisions. Please notify us of any changes and specifically state any
changes, including changes resulting from Plan Commission, BZA or other committee meetings.
All bonds and performance guarantees must be posted prior to Engineering Department approval of
construction plans. Also, Board of Public Works and Safety approvals and any other governing agency
approvals (if required) must be obtained prior to Engineering Department approval.
The Department reserves the right to provide additional comments based on subsequent reviews.

Engineering 'TAC comments are forthcoming; but I wanted to highlight some points that will be outlined
in the letter that your client needs to consider as soon as possible. The comments below are not
complete, as other comments will follow:
1. Engineering will support the proposed access from Old Meridian Street provided that it is aligned
with the existing entrance to Pinnacle Point and that it is understood that the City cannot
guarantee long term full access at this point given the potential for the IN DOT to restrict some
portion of the roadway to one-way. If other access points are desired that have a higher potential
for long tenn full access, we would be happy to have a meeting to discuss options.

Office on January 3, 2007. for this project and have the following comments:
1. The proposed project falls in the incorporated area and MS4 jurisdiction, of the
City of Carmel
2, The proposed project DOES NOT fall in a Carmel Wellhead Protection Zone.
3. The proposed project falls in the W.R. Fretig Regulated Drain Watershed. This
watershed has an allowable release rate ofO.2S cis per acre.
4. Please submit an indirect outlet permit for the discharge into the W.R. Fertig
Regulated Drain. The application is available on our website at

S. In reviewing the drainage calculations for this project, it is evident that this
project is NOT meeting that general countywide allowable release rates, let alone
