] understand that under the applicable rules and regulations, the Architectural CommitteelBoard of Directars
will act on this request and provide me with a written response of its decision. I further understand and agree to
the following provisions:
I, The covenants state this concerning an Architectural change or proposal: "in the event said Board, or its designated
Committee, jails to approve or disapprove such design and location within thirty (30) days after said written plans and
specifications have been submitted to it, or ifno suit to enjoin the making of such additions, alterations or changes or to
force the cessation thereofhas been commenced within sixty (60) days of such submission, such approval wiII be deemed to
have been given". The Board has decided an Architectural change proposal may be considered "submitted to it" when the
request has been submitted in writing AND is present at a Board meeting in which there is a quorum present to act upon
said request.
2. AU work will be done at my expense and all future upkeep will remain at my expense.
3. All work will be done expeditiously once commenced aDd will be done in a good workman-like manner by myself
and/or a licensed and insured contractor. Trash will be picked up or removed daily
4. All work will be perfonned at a time and in a manner to minimize interference and inconvenience to other home
owners.
5. 1 assume all liability and wi1l be responsiblefor all damage and/or injury which may result from perfonnance of this
work.
6. I will be responsible for the conduct of all persons, agents, contractors, and employees who are connected with this
work
7. . I will be responsible for complying with, and wiJI comply with, aU applicable federal, state and local laws, codes,
regulations and requirements in connection with this work, and I will obtain any necessary governmental pennits and
approvals for the work. I understand and agree that the Association, it's Board of Directors, its agent and/or the Committee
have no responsibility with respect to such compliance and that the Board ofDirector~s and/or i1:s designated committee's
approval of this request shall not be understood as the making of any representation or warranty that the plans.

Hi Kathy,
The architectural control committee has reviewed your project proposal for 10775 Bunker Hill and has approved it with the
following stipulations:
1. the exterior finish of both portions of the project (the porch addition end the room additions to the SE) must be constructed as
shown in the drawing package and related materials. Any deviation from the submitted drawings must be approved by the ACe.
2. all aspects of the finished project must match the ex1'St\ng structure Including colors and finishes, window finishes, and
exterior materials used (eg type of siding, type and style of windows, gutters, trim, stc).
3. we-would ask that once construction starts that the exterior ?spects ofthe project be completed within 90 days.
Unless you require the official notification this weekend (i.e. the architecural change request form with signature affIXed), I will be
sending it to you via email on Monday. If you need it earlier (this evening or tomorrow) please call me on my mobile number
(250-6502) and I will make arrangements to get the documents to you sooner.
Please advise IT you have any questions or concerns.
