Gentlemen,
The following requests for copies of certain parts of the approved plan come from
members of our Planning and Zoning office. Verification that copies have been received
and stamped as approved will be required for submittal for the building permit.
1. Please submit three (3) copies of the approved site plan to Scott Brewer, DOCS, One Civic Square,
Carmel, IN, 46032. You will be notified when your stamped copies are ready for pickup. Contact: Scott
Brewer, City Forester/Environmental Planner. One Civic Square; Carmel. IN 46032. Phone: (317) S71-247B. FAX (317) 571-

2. Please submit two (2) copies of the architectural elevation drawings for the structure to Angelina Conn, I
DOCS, One Civic Square, Carmel, IN, 46032. You will be notified when your stamped copies are ready for
pickup. Contact: Angie Conn, Planning Administrator. One Civic Square; Carmel. IN 46032. Phone: (317) 571-2417. .

Also, we will need to confirm with Dick Hill of the City Engineer's office, if there will be a
water connection and/or availability permit due for this building. If there is, you will neeCl

lateral connection to the utility line. That contractor must hold a current bond with the ,

Per our phone conversation Maple Crest Commons will be 22,000 sq.
figured at 9,000 gallons per 1000 sq. It per month.
So, the connection lee would be figured as follows:
22 x 9,000 = 198,000 gallons per month
9,000 gallons or 1 edu is charged at $1310 per 9,000 gallons-
$1,310 x 22 = $28,820 water connection fee

Again, this is based on what is within City ordinance. We also will review engineers extimates for water consumption as
well as any water usage data you have from other comparable retail stores. It is possible that this could lower the fee; but I
will need the information before determing that. Thanks
