I have a couple of questions. First of all, the street address listed on the application, plans, and State release is not for the building that it was our understanding this space was going to be in. (14390 Clay Terrace Blvd. is building 0-2. It was our understanding that this space was going to be in building E-1, which has a street address of 14300 Clay Terrace Blvd, and that suite #100 was going to be assigned to this space.) I am trying to determine for certain which building and which space you folks are moving into. Secondly, a copy of your Letter of Grant, which would have been issued by the Planning & Zoning office of the City of Carmel for approval of an ADLS Amendment for the design of your storefront, was not included with your submittal paperwork. We need to have a copy of that approval letter before we can process the permit application. Third, the name and Indiana State license number of the plumbing subcontractor that will be involved with this project was not listed on the application. We will need this information before you will be allowed to schedule the Rough-in inspection. Finally, there should be signed and dated green affidavit tags, one for each set of plans. The~e state that the plan copies you are submitting to us are exact copies of what was submitted to, and subsequently released by the State of Indiana. .
