Ramona Hancock no later than NOON, Friday, March 9, 2007. Failure to submit Informational Packets
by this time will result in the tabling of the petition to the April 17, 2007, agenda of the Plan Commission.

1. Allow plenty of time for review and approval process (approximately .two (2)
months).
2. Discuss proposed project with DOCS staff (please call for an appointment to discuss
review procedure and appropriate dates) first week of the month, works the best to
begin a project.
3. INFORMATION NEEDED for formal DOCS staff and Plan Commission review:
a. Two copies of formal application with required information
b. Two copies of legal description
c. Two location maps showing location of subject site, zoning and existing land
uses of all adjacent properties.
d. Two copies of a detailed site development plan showing:
1) Detailed drainage plan with drainage calculations. If project is in Clay
Township, take to Kent Ward, County Surveyor (776-9626) and John South,
U.S. Soil Conservation Service (773-1406). If project is in City of Carmel,

Lot 5 - Carmel Science & Tech, Block 11
Electric - Electric service is provided by Duke Energy in this area.
Service is available along the southern property line within an existing
utility easement.
Gas -Indiana Gas Company provides gas service to this site. Gas is
available along Carmel Drive and Guildford Rd. It is expected that gas
service will be extended to the proposed building from one of two mains
available which have sufficient capacity to serve the proposed projects
needs.
Telephone - Service is provided by SBC/Ameritech and is available
within the Carmel Drive ROW.
Cable - Brighthouse Networks provides cable service to this project
area. Cable should be available in sufficient capacity for the proposed
project from Carmel Drive.
Sanitary Sewer - The proposed project site falls within the Clay
Township Regional Waste Districts service area of Carmel. The
proposed subdivision of Block 11, Carmel Science & Tech Park includes
the sanitary main extension from Carmel Drive to the northern property
line of Lot 5.
Water Service - The proposed project site falls within the City of Carmel
Utilities service area. The proposed subdivision of Block 11, Carmel
Science & Tech Park includes a water main extension from Carmel Drive
across the northern property line of Lot 5.

For walkways. plazas or pedestrian areas.
Features
Housing - KB_6: Bottom housing is 0.125" ex-
truded aluminum. Top cover is 0.156" cast alumi-
num. KB_8: Bottom housing is 0.156" extruded
aluminum 42" overall height standard. Flush-fit-
ting lens is 112", clear, 100% virgin acrylic. All
exposed hardware is tamper-resistant stainless
steel. Four 1/2" x II" anchor bolts with double
nuts/washers and 4-1/2" diameter bolt circle
template provided for mounting (shipped sepa-
rately). Dark bronze (DDB) corrosion-resistant
polyester powder finish standard; other archi-
tectural colors available.
Optics - Reflector system incorporates an anod-
ized, spun aluminum, flared cone and an anod-
ized, hydroformed, fluted upper reflector pro-
viding a Type V (symmetric) cutoff distribution.
Optional cylindrical lower reflector or cast-alu-

Architectural Round Vertical
For streets, parking lots and surrounding areas.
Features
Housing - Cylindrically shaped, rugged, heavy-
gauge, spun aluminum housing. Fully gasketed
for weathertight integrity. Standard finish is
dark bronze (DDB) polyester powder. Other ar-
chitectural colors available. KVR2 standard door
frame is natural aluminum (DNA). Door frame
can be painted to match housing color (see DFP
option below).
Lens - Impact-resistant, clear, 3/16" thick, tem-
pered drop lens.
Mounting - Extruded-aluminum arm with inte-
gral splice compartment for wall or pole
mounting is shipped in fixture carton. Optional
mountings available.
Optics - KVR2 vertical-lamp reRectors are one
piece spun and formed around anodized alumi-
num. KVR2 horizontal-lamp hydroformed and
segemented reflectors also available. KVR2 re-
flectors are rotatable and interchangeable. KVR3
segmented, anodized aluminum optics also are
rotatable and interchangeable.
Electrical - Constant-wattage autotransformer,
high-power factor ballast. Ballast is copper
wound and 100% factory tested. Super ONA
pulse start ballasts required for 320M and 350M
(must order SCWA option).
Socket - Mogul-base porcelain socket with cop-
per alloy, nickel-plated screw shell and center
contact. UL Listed 1500W, 6OOV.
