RE: Penn-View Heights, Lots 7, 8, 9, 10, llpt, 17pt & 18 (#05070005 SP) - Replat Application
Dear Ms. Hartman:
This letter is in response to your Replat Plat Application for Penn-View Heights, Lots 7, 8, 9,10, llpt,
17pt & 18. Members at the T AC Meeting will make additional comments on July 20. Please complete
the following:
1. Change the plat title to read: Replat of Penn- View Heights, Lots 7, 8, 9, 10, 11 pt, 17pt & 18.
"Dr. Lynn Burford's Office" is not required in the title.
2. Please add docket number 05070005 SP to the plat.
3. Attached is an example of a plat, with location map and signature blocks. Please use this as a
guide. Also, see Chapter 9 of the Subdivision Control Ordinance."
4. Make sure all owners involved are on the signature page.
5. On the plat, make sure to include the date of submission or revision.
6. Make sure to include the owner, subdivider, and surveyor on the plat, along with each
address and phone number (see example).
7. Add the surveyor's signature, seal, and date to the signature page (see example).
8. Place the source of title of ownership on the plat.
9. Provide a statement ofland to be dedicated on the signature page (see example).
10. Please provide copies ofletters of transmittal to TAC members, so we know they have
received plans.
11. What will the new lot number be? Make sure to add the suffix A to the new lot number, so it
reads, such as lot 18A.
12. Place zoning classification on the plat: B-5/Business (pending).
13. Please show/label the existing and proposed road right of way for Pennsylvania St. (120-ft)
and Delaware St. Contact the Engineering Dept for exact dimensions at 571-2441.
14. Provide a subdivider agreement form - see attached.
Please submit a revised Secondary Plat after the T AC meeting with the above-mentioned changes. Once
these changes have been made and comments addressed, please produce a Mylar copy for signature by
this Department. Two paper copies of the plat must be filed with the Mylar prior to recording, and three
paper copies and a Mylar of the recorded plat must be submitted following recording. Our approval of
the secondary plat (signature) will be forthcoming upon revision of the plans according to the items listed
above.

Also, the staff has been advised that we cannot present a Secondary Plat to the Director for signature
from this point forward until Terry Krueskamp (tkrueskamp@carmel.in.gov) in Information Systems has
received an e-mail with an electronic file attached (MicroStation .dgn is preferred, or AutoCad .dwg of
the subdivision) and it (the e-mail) has been copied to Matt Griffin (mgriffin@carmel.in.gov) and myself
(aconn@carmeLin.gov). (Also, Matt Griffin would also like a .pdffile, as we move toward a paperless
office.)
