Also, on the Liberty Mutual permit (07030137) I don't show sign off of any other phases that might have already
been completed, by the Carmel Fire Department. It does look, however, iike our office did a final on Phase 1. Do
you know if someone over there completed your finai on that area? Last, is this it-or are there more
phases/areas?

I think the original calculation sheet I gave you was based upon the old fee. I did a new
calculation sheet when I thought they were going to pay the new fees. I think I brought the
revised sheet over one day you were gone. The calculation sheet you gave me this a.m. was '
the revised sheet with the new fees. If you have a sheet based on the old fee, then I will discard
the revised one.

Brockway Public House or does this ramp already exist? (2) Is there any existing walk to
the west of the north-south drive from Horseferry just west of the Brockway Public House
or will a future developer install this walk and a future ADA ramp. If so, would it be
best to depress the curb today for the future ADA ramp as the curb for this new drive lis
being constructed? rf so, please indicate on the plans submitted for approval. (3) Has the
r/E easement for the sidewalk to the east of the Regents Building already been dedicated
or is this forthcoming?
Prior to approval of the drawings, you will need to verify with Dick Hill that all BPW
approvals have been obtained; all certified estimates and bonding has been provided and
any fees have been paid.

The Hamilton County Surveyors Office has never received our required permits from our TAC
review of the project. I will make contact with the civil engineer, Brandon Burke o~
Schneider Corporation, to track down the permits. Please do not allow this project to
move forward until I notify you that we have received our permits. If you have any

The following requests for copies of certain parts of the approved plan come from members of
our Planning and Zoning office. Verification that copies have been received and stamped as
approved will be required for submittal for the building permit.
1. Please submit three (3) copies of the approved site plan to Scott Brewer, DOCS, One Civic Square, Carmel,
IN, 46032. You will be notified when your stamped copies are ready for pickup. Contact: Scott Brewer, City
Forester/Environmental Planner. One Civic Square; Carmel. IN 46032. Phone: (317) 571-2478. FAX (317) 571-2426
2. Please submit two (2) copies of the architectural elevation drawings for the structure to Angelina Conn, DOCS,
One Civic Square, Carmel, IN, 46032. You will be notified when your stamped copies are ready for pickup.

Also, we will need to confirm with Dick Hill of the City Engineer's office if there will be a water
connection and/or availability permit due for this building. If there is, you will need to either
pay for this permit ahead of time, or bring a check to the submittal meeting. We will need the
name of the contractor who will be doing the excavation of the exterior, lateral connection to'
the utility line. That contractor must hold a current bond with the City. Additionally, some of
