Note from 8/23/06 was for original
submittal for this permit, under #
06080132. That permit was never picked
up, after multiple messages to builder
that it was ready. That application
was eventually dosed out, and the
plans disposed of. Builder now wants
another permit, but this situation must
be resolved. CFD has inspected already,
and noted only 1 violation. They are
otherwise okay with the space.
Space was cited in letter to Reed
Carlson on 1/27/05 as having had needed
an interior tenant finish permit for the
space originally, which should have
included the HOOD release. This permit
was never applied for. Builder is now
coming in with tenant permit application
which includes HOOD release. (State #:
314178; Construction type Exst,
Occupancy classification A-2. HOOD.
Condition that release is for complete
installation of exhaust hood and hood
suppression system and shall be in
accordance with the required code
section.)
Letter to Carlson has stated that late
fees would be applied to the permit and
possibly to the inspections, as the
space was already occupied and open. It
also included a further note that there
were REAR EXIT HINDRANCE ISSUES which
needed to be addressed, as there was
(at that time) an illegal locking device
on the rear door.

This permit is valid only if construction commences within one (1) year of the date of issuance of the State Commercial Design Release. AI! constru~tion
must be completed (C/O issued) within two (2) years of the issuance date. :
