Procedure for Plan Commission
For Business Zones, U.S. 31 Meridian Street Corridor, U.S. 431 Keystone Avenue Corridor
and the U.S.421 Michigan Road Corridor
1. Allow plenty of time for review and approval process (approximately .two (2)
months).
2. Discuss proposed project with DOCS staff (please call for an appointment to discuss
review procedure and appropriate dates) fIrst week of the month, works the best to
begin a project.
3. INFORMATION NEEDED for formal DOCS staff and Plan Commission review:
a. Two copies of formal application with required information
b. Two copies of legal description
c. Two location maps showing location of subject site, zoning and existing land
uses of all adjacent properties.
d. Two copies of a detailed site development plan showing:
1) Detailed drainage plan with drainage calculations. If project is in Clay
Township, take to Kent Ward, County Surveyor (776-9626) and John South,
U.S. Soil Conservation Service (773-1406). If project is in City of Carmel,
discuss with Kate Boyle, City Engineer at 571-2441..

docket number will be released when the filing fee is submitted.
Petitioner or representative must appear at the CarmeVClay Plan Commission (public
hearing) and give a presentation. The presentation should include information relative
to the project and should be presented on a poster board so that it can be seen from 20
feet. In addition, reduced packets (8-1/2 It x 11 It) packets of the poster board project
information should be handed out for each Plan Commission member.

Petitioner or representative must attend the Technical Advisory Committee (T AC)
meeting on Wednesday following the Plan Commission Meeting.
Petitioner or representative must attend the Industrial and Commercial Subcommittee
to review all pertinent information.
Petitioner or representative must attend the second Plan Commission meeting for final
vote (questions may need to be answered).
Once Plan Commission has voted on the project, it is up to the petitioner to work with
the staff to obtain proper building permits.

from UL-recognized electrical components. Core and coil ballasts shall

ballasts shall be available for the 39W and 70W metal halide lamps in
120 and 277 volts and rated for _200P (-28.9"C). Ballast shall be housed
in a round die-cast aluminum ballast box finned for heat dissipation.
Luminaire lamp ,holder shall be of a mini-candelabra type for quartz
lamps and a G-12 bi-pin for UV Block T6 metal halide lamps. Yoke

FINISH
Finished parts shall be washed in a multi-stage phosphate
processing system and then finished with polyester urethane

10% up), WTU (wide throw, 10% up) and MDUS
(up/down, medium throw, 50% up, 50% down).

Electrical - HID: SOW MH-150W utilizes a high
reactance, high-power factor ballast. 35S and
50S utilizes a reactor normal-power factor bal-

Socket - HID is porcelain, medium-base copper
alloy, nickel-plated screw shell and center con-
tact. (UL Listed 660W, 600V 4KV pulse rated). Flu-
orescent socket is high-temperature thermo-
plastic with integral lamp retention clip.
Installation - Universal mounting mechanism
with integral mounting support allows fixture to
hinge down. Bubble level provides correct align-
ment with each installation.
