This evaluation is intended to assess the level of compliance with 327 lAC 15-5 (Rule 5). It is also intended to identifY areas
where additional measures may be required to control erosion and sedimentation. All practices recommended in this report
should be evaluated as to their feasibility by a qualified individual with structural practices designed by a qualified engineer.
All erosion and sediment control measures shall meet the design criteria, standards, and specifications outlined in the "Indiana
Handbookfor Erosion Control in Developing Areas" or similar guidance documents.

The technical review and comments are intended to evaluate the completeness of the erosion and sediment control plan for the
project, The erosion and sediment control plan submitted was not reviewed for the adequacy of the engineering design. All
practices included in the plan, as well as those recommended in the comments should be evaluated as to their feasibility by a
qualified individual with structural practices designed by a qualified engineer, The plan has not been reviewedfor local, state,
or federal permits that may be required to proceed with this project. Additional information, including design calculations may
be requested to further evaluate the erosion and sediment control plan.
The erosion and sediment control plan has been reviewed and it has been determined that the plan:
1. Will likely disturbed less than five acres. An NOI does not need to be submitted for projects less
than five acres. If the site work and home construction is estimated to disturb more than five
acres an NOI for Rule 5 must be submitted.
Proper implementation of the erosion and sediment control plan and inspections of the construction site by the developer or a
representative are necessary to minimize off-site sedimentation. The developer should be aware that unforeseen construction
activities and weather conditions may effect the performance of a practice or the erosion and sediment control plan. The plan
must be aflexible document, with provisions to modify or substitute practices as necessary.

Note: All erosion and sediment control measures shown on the plans and referenced in this review must meet the design
criteria, standards, and specifications outlined in the "Indiana Handbookfor Erosion Control in Developing Areas" from the
Indiana Department of Natural Resources, Division of Soil Conservation or similar Guidance Documents.
1. A construction sequence is recommended. The dry detention basin should be completed and seeded
early in the construction schedule.
2. The permanent seeding needs to be labeled. The seeding in the dry basin should be permanent.
3. Erosion control blanket is needed for the swale at the rear oflots 4 & 5. Blanket S75 is not adequate
for the swale at the rear of lots 4 & 5.
4. Water bars were recommended for the swale behind lots 4 & 5.
5. A temporary riser is recommended for the outlet of the dry basin. The riser could be five feet high.
6. The silt fence that crosses the front of the property should turn north at the east and west ends.
7. Existing wells need to be abandoned properly.

I am writing this letter to explain the action taken to resolve the items outlined in your review
comment letter dated May 28 2002. This letter will address the review comments for Secondary
Plat. I have addressed each item individually using item designation consistent with your letter.
1. Signature block for the Hamilton County Commissioners has been removed.
2. Signature block for the Carmel Board of Works and Safety has been added.
3. The Primary and Secondary Plat Docket numbers have been added.
4. Source of title and instrument numbers has been added to sheet 1, which was made the last
sheet (sheet 3) per comment 10.
5. Address note has been removed from the legend.
6. Legend has been revised and monuments have been added to each sheet.
7. See enclosed Landscape Plan.
8. See enclosed Covenants and Restrictions.
9. After all comments have been successfully addressed then we will send a signed mylar copy
for the directors signature.
10. Signature page has been made the last sheet (sheet 3).

Dear Mr. Mouser:
As stated in our Project Review Letter #1, dated May 13,2002, we reserved the right to make additional
comments after the City Engineer reviewed the drawings. She has now reviewed the plans. We offer the
following additional comments from the City Engineer:
DRAINAGE COMMENTS
. Need additional topo or spot shots shown (proposed) to ensure side/rear yard drainage works for
each lot. Show installation of rear yard swales (high and low points).
. Still need to show more detail for detention area, including:
Limits of detention area? Provide easement on both lots 5 and 6.
Contours are too close at scale shown (on Sheet 4). Show blow-up detail with proposed
tops of banks, emergency overflow (with elevations), easements, etc.
. The basic goal in detention is to hold the post -developed 100-year rate to at or below the existing
10-year rate. I don't believe this is shown in the report.
. Show the proposed grade on profile section of Sheet 5 (this will represent the flow line of the rear
yard swale behind lot 3, 4 and 5).
OTHER COMMENTS
. How is the sanitary sewer to connect in Valleybrook? Show property (lot #'s) and easements
across which the proposed line will be constructed.
. Is the common area an easement? It needs to be labeled as such.
. Eliminate concrete saddles. We utilize stone saddles.
. Notify this office prior to entrance work on 1 16th Street. This note should be placed on the plans.
. What is the spacing of barrels? The spacing scales 40' = or -, but it is not "noted" as legend
suggests.
. Is there a legend for the Erosion Control Plan Sheet? What are the crosshatched and
honeycombed areas, materials?
. The sidewalk ramp detail (Sheet 11) does not seem to match proposed ramp locations.
. If sidewalks are planned in the 1 16th Street right of way, would recommend all new sidewalks
along 1 16th Street as this construction may cause additional breaks/cracks in existing segments,
and the City may require new panels where condition is unacceptable (it is not desired to have
vastly different aged concrete walks as it creates maintenance problems.
If we remember correctly from T.A. C. comments, an asphalt path is required in the 11 (/h Street
right of way so the above comments would not be applicable.

This letter is in response to your Secondary Plat application for Williams Ridge subdivision. The following issues
need to be addressed and plans updated accordingly.
1. Remove signature block for the Hamilton County Commissioners.
2. Add signature block for the Carmel Board of Works and Safety.
3. Add Primary and Secondary Plat Docket numbers to sheet 1 (70-02 PP & 71-02 SP.)
4. Add a source of title and instrument number to sheet 1.
5. Remove street address note from the legend. Addresses are not required on a secondary plat.
6. Revise Legend and add to each sheet where appropriate: The monuments and markers should be
as required under Section 8.2 of the sca.
a. Add concrete monuments at all comers that form the boundary of the plat, as required
under sca 8.2.1.
b. Add monuments at the beginning and ending of all curves along street centerlines, as
required under SCO 8.2.2.
7. Provide construction grade landscape plan as approved by Scott Brewer, DOCS Urban Forester.
8. Provide a copy of the covenants and restrictions.
9. Have Mylar signed by property owner and surveyor (with R.L.S. seal) prior to signature by
director.
10. Please make the signature page the last sheet in the secondary plat set.
Please submit a revised Secondary Plat with the above-mentioned changes. Once these changes have been made and
comments addressed, please produce a Mylar copy for signature by this Department. Two paper copies of the plat
must be filed with the Mylar prior to recording, and three paper copies and a Mylar of the recorded plat must be
submitted following recording. Our approval of the secondary plat (signature) will be forthcoming upon revision of

The technical review and comments are intended to evaluate the completeness of the erosion and sediment control plan for the
project. The erosion and sediment control plan submitted was not reviewedfor the adequacy of the engineering design. All
practices included in the plan, as well as those recommended in the comments should be evaluated as to their feasibility by a
qualified individual with structural practices designed by a qualified engineer. The plan has not been reviewedfor local, state,
or federal permits that may be required to proceed with this project. Additional information, including design calculations may
be requested to further evaluate the erosion and sediment control plan.
The erosion and sediment control plan has been reviewed and it has been determined that the plan:
1. Will likely disturbed less than five acres. An NOI does not need to be submitted for projects less
than five acres. If the site work and home construction is estimated to disturb more than five
acres an NOI for Rule 5 must be submitted.
Proper implementation of the erosion and sediment control plan and inspections of the construction site by the developer or a
representative are necessary to minimize off-site sedimentation. The developer should be aware that uriforeseen construction
activities and weather conditions may effect the performance of a practice or the erosion and sediment control plan. The plan
must be aflexible document, with provisions to modify or substitute practices as necessary.

criteria, standards, and specifications outlined in the "Indiana Handbookfor Erosion Control in Developing Areas" from the

1. The upper pipe that goes through the dry dam should not discharge on the back side of the dam. The
pipe should be extended through the fill and discharge at the toe of the back slope.
2. One or two water bars are recommended to help stabilize the path of the storm sewer going down the
steep slope.

GENERAL INFORMATION

Owner and the City of Cannel.
I am also enclosing a schedule for Board of Public Works and Safety meeting dates and agenda
deadlines for your use. Please use the Engineering Department deadlines for submissions to the
Board.
Any submission to the Board requires prior approval by the Carmel Clay Plan Commission
and/or the Board of Zoning Appeals (if applicable) and completion of review by the Technical
Advisory Committee. AU written requests to be placed on the Board's agenda must include the
appropriate Docket Number and the date (or dates) of approval by the Plan Commission and/or
the Board of Zoning Appeals (if applicable).
4. T.A.C. Review/Drawings submitted for approval:
We request that all comments and comment letters generated by this office be answered in writing
and be accompanied by a drawing reflecting requested revisions. Final drawings will not be
approved for construction until all Engineering Department and Utility Department issues have
been resolved. The design engineer must certify all drawings submitted for final approval. This
office will require a minimum of five-sets of drawings for approval after all issues have been
resolved. The drawings will be stamped as approved and will be signed by the City Engineer and
Director of Cannel Utilities. The Owner will receive one-set which is to be maintained on the
construction site at all times. Carmel Utilities will receive two-sets, our Public Works Inspector
will receive one-set and one-set will be maintained in the Engineering Department

The amount of the Performance Guarantee is based upon a certified Engineer's Estimate for 100%
of the cost of labor and materials to construct the individual improvements, to be provided by the
design engineer. Upon completion and release of individual Performance Guarantees, a three-year
Maintenance Guarantee will be required. The Maintenance Guarantee amount is based upon 15%
of the Performance amount for Streets and Curbs and 10010 of the Performance amount for all other
improvements. Performance Guarantees may be Performance or Subdivision Bonds, Irrevocable
Letters of Credit or Cashier's1CertifiedlOfficial Checks. Please reference the enclosures for more
detailed explanation of our procedures.
AVAILABILITY AND CONNECTION FEES
10. Availability (acreage) Fees must be paid after all other Engineering Department requirements have
been satisfied and prior to start of construction activities (the exception is that you are allowed to
move dirt on site). Availability Fees are based upon total platted acreage for the development at
the rate of$I,OlO.OO per acre for Water Availability and $1,365.00 per acre for Sanitary Sewer
Availability. Connection Fees are $1,310.00 per single-family residence and $795.00 per single-
family residence. Connection Fees are paid when the infrastructure has been completed,
satisfactory test results obtained and the subdivision has been released for permits. Connection
Fees are paid on a lot-to-lot basis.

11. It appears that there will be open pavement cuts required in existing dedicated streets. One area is
located in the pavement area of Valleybrook Place in the Briar Cove subdivision. This cut is
required to provide sanitary sewer service to Williams Ridge. The other location is in the
pavement area of eastbound 1 16th Street, close to the curb line on the south side of the street. This
cut is required to provide water service to Williams Ridge. These pavement cuts will require prior
Board of Public Works and Safety approval. Please provide a detailed plan for both cuts,
including a traffic control plan for 116th Street. We request that the plans minimize the amount of

pavement that must be cut wherever possible. It is very doubtful that you will be allowed to close
more than one lane of 116th Street and you will be required to maintain two-way traffic at all times
on 116th Street. Be advised that the Board may require the Developer to resurface a larger area of
116th Street as a condition of approval of the open cut.
12. Please designate existing right of way lines as existing right of way on all sheets.
13. The pipe designation for the run of pipe from storm structure 711 to 712 is pointing to the wrong
run of pipe.
14. Please place the following statement on Sheet 3, Site Development Plan, close to the Storm Sewer
Structure Table. Also place this statement on the Storm Sewer Plan and Profile Sheets, Sheets 5
and 6.
THE STORM SEWER SYSTEM SHALL BE CONSTRUCTED PER DESIGN SPECIFIED
AND AS APPROVED BY THE CITY OF CARMEL ON THE FINAL APPROVED
CONSTRUCTION PLANS*. DEVIATIONS FROM THE APPROVED DESIGN SHALL
ONLY BE PERMITTED DUE TO SPECIAL CIRCUMSTANCES OR DIFFICULTY DURING
CONSTRUCTION AND WILL REQUIRE PRIOR FIELD APPROVAL FROM A
DESIGNATED REPRESENTATIVE OF THE CITY OF CARMEL IN ADDITION TO
SUPPLEMENTAL APPROVAL BY THE DESIGN ENGINEER. AN EXPLANATION OF
ANY SUCH DEVIATION SHALL BE INCLUDED AS A REQUIREMENT ON AS.
BUILT/RECORD DRAWINGS SUBMITTED FOR RELEASE OF PERFORMANCE
GUARANTEES. APPROVED DESIGN SLOPES IDENTIFIED AS GENERATING
VELOCITIES OF 2.5 FPS OR LESS SHALL REQUIRE AS-BUILT CERTIFICATION AT
THE TIME OF CONSTRUCTION.
*THE CONTRACTOR SHALL FIELD CHECK EACH SECTION OF STORM PIPE AS IT IS
BEING INSTALLED TO ENSURE COMPLIANCE WITH THE DESIGN PLANS.
15. Identify the structure numbers on the levee cross section.
16. Even though the structure tables are on this sheet, we request that the top of casting and invert
elevations be placed on the plan view sheets.
17. The detention area is confusing. Is it intended to be wet or dry detention? Perhaps a detail of this
area in larger scale would better illustrate how this is intended to function.

18. The run of pipe from storm structure 702 to 701 exceeds our maximum length requirement.
Please install another storm structure to assure that anyone length of pipe does not exceed 400
feet in length.
19. This office would prefer alignment of the tops of the inlet and outlet rather than inverts when
possible. If it is not possible to align the tops of pipes, we would request the inlet pipe invert to be
raised above the outlet pipe invert.

20. Please identify all sanitary sewer laterals on the plans including size, length and type of pipe and
the distance from the nearest downstream sanitary structure.

21. Please show and identify all water laterals on the plans including size, length and type of pipe and
the distance from the nearest lot property line.
The above comments are based upon the Engineering Department's initial review of this project. Please
provide our office with a follow up letter and revised plans indicating all revisions. Second and subsequent
reviews will focus only on revised items. It is critical that this office be made aware of all modifications
made on the plans being re-submitted, particularly if any such changes are considered "new" or fall outside
of our initial review.

The City Engineer is out of town this week and has not had the opportunity to review the plans or the
drainage calculations. Therefore, we reserve the right to provide additional comments after she has
reviewed the plans and the calculations.
