The erosion and sediment control plan submitted was not reviewed for the adequacy of the engineering design. All practices included in the plan, as well as those recommended in the comments should be evaluated as to their feasibility by a qualified individual with structural practices designed by a qualified engineer. The plan has not been reviewedfor local, state, or federal permits that may be required to proceed with this project. Additional iriformation, including design calculations may be requested to further evaluate the erosion and sediment control plan. The erosion and sediment control plan has been reviewed and it has been determined that the plan: X Does not satisfy the minimum requirements and intent of 327IAC 15-5 (Rule 5); deficiencies are noted in the checklist and in the comments section. Deficiencies constitute potential violations of the rule and must be adequately addressed for compliance. This site is being constructed prior to plan approval. The items that needed to be addressed on the plan are now past in the construction process. Proper implementation of the erosion and sediment control plan and inspections of the construction site by the developer or a representative are necessary to minimize off-site sedimentation. The developer should be aware that unforeseen construction activities and weather conditions may effect the performance of a practice or the erosion and sediment control plan. After reviewing the plans, the Highway Department has the following comments: 1. Before any approvals are granted for the Secondary Pfat, an engineers Estimate broken into the foRewing categories will need to be submitted, along with a Performance Bond or Letter of Credit In the amount of 100% of the estimate: · Stone Base · HAC Binder · HAC Surface * Concrete Curbs * Street Name and Regulatory Signs 2. An intersection detail is needed for the two intersections and cul-de-sac shown in this section. 3. Please show a Construction Drive on the Erosion Control Plan. The drive should be at the end of the pavement for section 1. 4. The radius to the back of the curb in the cul-de-sac of Reedy Court must be a minimum of 39'. 5. The water main valve shown in the pavement near Reedy Court must be relocated to the north side of Tuscany Boulevard. 6. Relocate the fire hydrant in Reedy Court to the south side of the cul-de-sac bulb and use the poly pipe to service lots 32-35 from behind the curb. 7. Please include HCHD Standard Detail C-5, and add a note to the site plan stating that chairback curb with reverse slope must be used around the islands. 8. Remove the road patch detail from sheet 14. Road cuts wUI not be permitted. 9. Pfease include flow lines for the street drainage. 10. Please note that additional comments may be necessary after the revisions have been made. The following issues must be resolved prior to signature of the plat by the director: 1. Revise the Key Map on page one of the Secondary Plat to clearly highlight and identify Section 2 of the subdivision. 2. Change the Secondary Plat Docket number to 68-02 SP on page 1, and add it to page 2. 3. Forward a copy of final review letters from the following agencies: a. County Highway b. County Surveyor c. Hamilton County Soil and Water 4. Have Mylar signed by property owner and surveyor (with R.L.S. seal) prior to signature by director. 5. Submit a construction grade landscape plan as approved by Scott Brewer, DOCS Urban Forester for primary plat. 6. On sheet 3 of the construction plans, show the 6' asphalt path extending into the common area across Tuscany Boulevard. It appears as drawn that the path may be in conflict with the median. Show curb depression and ramps (concrete in ROW.) 7. Note: where asphalt paths intersect streets and sidewalks the approach shall be concrete with depressed curbs the full width of the path. See Hamilton County Highway Department for detail. 8. Change common area description to 3A, not #1 as written on sheet 2. 9. Add cross-reference numbers for both survey plats on sheet 2. 10. Add cross-reference numbers for the covenants and restrictions on sheet 2. Please submit a revised Secondary Plat with the above-mentioned changes. Village of West Clay, section 3004, Block C, Townhomes The plans received by the District only show the mainline sewer for the development but there must be individual connections for each unit in each building in case these are sold as condominiums. Revised plans need to be submitted showing each building with its own individual connection to the District's sewer along with an address list for permits and inspections. Building permits should NOT be issued unless the District has issued connection permits. West Carmel Center Block D,Lot 2 - 5th 3rd Bank The plans show a questionable connection to sanitary sewers along us 421. The project developers will need to consult with the District as to the appropriate lateral connection point which may be within the West Carmel Center development, not along us 421. After reviewing the plans, the Highway Department has the following comments: Construction Plans 1. Has the additional right of way needed for the installation of the passing blister lane acquired? Construction plans will not be approved until the necessary right of way is acquired and the dedication forms fo~rded to the Highway Department. 2. Please verify that 131 Street will not be topped with storm water during the 100-year storm. If this is the case, revisions will need to be made to the plans to prevent water from topping the road. 3. Trees are not permitted within the 50' right of way. Please remove all trees on the Landscaping Plan that are shown within the right of way. If trees are to be placed within the 70' right of way, they must be behind the sidewalk. Please revise the Landscape Plan to show this. 4. The location of the signs must be shown on the Traffic Control Plan. 5.
