For Business Zones, U.S. 31 Meridian Street Corridor, U.S. 431 Keystone Avenue Corridor

1. Allow plenty of lime for review and approval process (approximately .two (2)
months).
2. Discuss proposed project with DOCS staff (please call for an appointment to discuss
review procedure and appropriate dates) first week of the month, works the best to
begin a project.
3. INFORMA TION NEEDED for formal DOCS staff and Plan Commission review:
a. Two copies of formal application with required information
b. Two copies of legal description
c. Two location maps showing location of subject site, zoning and existing land
uses of all adjacent properties.
d. Two copies of a detailed site development plan showing:
1) Detailed drainage plan with drainage calculations. If project is in Clay
Township, take to Kent Ward, County Surveyor (776-9626) and John South,
U.S. Soil Conservation Service (773-1406). If project is in City of Carmel,
discuss with Kate Boyle, City Engineer at 571-2441..

FGI is contracted to provide new seating & decor for this Fazoli's restaurant. In addition we plan to paint the exterior of the
building with their new Decor colors and replace 12 existing Goose Neck lights on the exterior of the building with
Up/Down combination Directional Flood lights.
We are not changing anyth ing with the Signage or Parking or any other part of the exterior and we had not bid to do any
landscaping but will plan to do whatever landscaping is necessary to comply with the city.

Angie, the photo of Lexington shows the new color scheme that we plan to paint all of the Central Indiana stores
including Carmel. The specs are: Benjamin Moore "Aztec Gold" with trim in Sherwin Williams "Frog Green" &
"Red Door".
We will be replacing exterior "Goose Neck" Lights with up down lights (same quantity {12} and location as
existing)
The spec for new light is: RUUo Lighting # DF1 P20S in Black and will be a combination of 1 u
fixture (as depicted on our elevation drawing- see photo to right).
We will not be changing any signage.
I have been unsuccessful in finding Parcel 10 # on-line. Can you provide and if you have all the info there for plot
size and zoning classification, could you send me that too. I have requested the legal description from the owner
but I want to be sure I provided you everything quickly. Should I fax application or do you require original
hardcopy only? .
