MATTHEW J. ZIMPFER, Senior Vice President of WASHINGTON NATIONAL
INSURANCE COMPANY, an Illinois domiciled insurance company ("Affiant''), states
in his capacity as Vice President the following:
1. Bankers National Life Insurance Company, a Texas domiciled insurance
company, ("Bankers National") owned of record several parcels of real estate in
Hamilton County, Indiana.
2. Effective October 1,2004, Bankers National merged with and into Washington
National Insurance Company ("Washington National") and Washington National
became the surviving company as a result of the merger and became the
successor by merger to Bankers National.
3. As a result ofthe merger, Washington National became the owner of a11 the real
estate previously owned of record by Bankers National, as a matter oflaw.
4. This Affidavit is made for the purpose of placing of record notification that all
real estate in Hamilton County, Indiana previously titled in the name of Bankers
National is now owned, as a matter oflaw, by Washington National.
Under penalties of perjury, Affiant swears that he has examined this Affidavit on behalf
of Washington National Insurance Company, in his capacity as Senior Vice President,
and to the best of his knowledge and belief, it is true, correct and complete, and Affiant
further declares that he has the authority to sign this docwnent on behalf of Washington
National Insurance Company as its Senior Vice President.
