Church has approved substituting 16 gray dogwoods (30") for the specified 1.6 pink cloud
beauty bushes (30") on the above referenced project. The bushes are located at the
southeast corner of the property.

The following comments are regarding this months TAC items. Additional information, such as the District

Clarian North Expansion - The existing sanitary sewer main extension must be dedicated to the District before
sanitary sewer permits may be issued in this area. Sanitary sewer construction plans need to be submitted for
review to the District. The District has not received sewer service plans. A District project application needs to be
completed.
Grandin Hall Section 2 - Outside service area.
College Park Baptist Church Parking Lot Expansion - Plans were received and reviewed. No action needed.
Stratford Assisted Living at the Village of West Clay - Plans and District application have been received and
reviewed. Comment letter to follow shortly. An IDEM construction permitfor sanitary sewer will be needed. All
IDEM submi1tal information should be provided following the District's review and approval of final plans.
Kendal Wood - The plans and application have been received last week. Ani OEM construction permit for
sanitary sewer will be needed. All IDEM submittal information should be provided following the District's review
and approval of final plans.
Sanders Plat -- Not received by District. Please submit for review.
Mayflower Storage Condos - Plans have been received and reviewed. Additional information will be requested.
A CTRWD Application is needed.
Old Meridian Place -- Out of service area.
Brownstone Homes at Guilford Reserve Section 2 - Please resubmit plans and application.
Laurel Ridge - Plans received.
Carmel Water Operations Facility - Plans received and reviewed. A District Application needed. Additional
information will be requested.

Dear Mr. Doster:
This letter is regarding a transmittal received November 1, 2005 containing the plans for
the above-mentioned project After reviewing the plans the Highway Department has the
following comments:
1. A Commercial Drive Permit will be required for each of the entrances.
Pleas,e contact Mr. Robb Chadwell, Highway Department Permit
Inspector, for the necessary forms and information. The permits can be
found on the Hamilton County website at www.co.hamilton.in.us. under
the Highway Department link.
2. Please prepare a right of way dedication for the parcel where the
improved drive is located. A 60' half is required.
3. Has the right of way necessary for the installation of the passing blister
and accel/decellanes been acquired? If so, please forward the dedication
documents to our offlce.
4. Please note that decorative signs, sprinkler systems, trees, landscaping
mounds, fences, light poles or other such amenities are not permitted in
the right of way.
5. Please provide a note that the pavement shall be saw cut to provide a
smooth transition.
6. Please prepare a detail of the entrances. The detail should include a
pavement cross section and layout of the accel/decellanes as well as the
passing blister on 96th Street. Other things which will need to be included
will be the pavement cross section for 96th Street (14" stone base, 5"
HMA Intermediate course and 1" HMA surface course), lengths of the
tapers and lanes and a striping plan for the passing blister plus any other
pertinent information. The through lanes need to be widened to 12'. The
pavement within your project limits should be milled and then resurfaced
along with the new, widened portions. '
7. The construction plans will need to include a sheet titled Majntenance of

Traffic. The notes should include but not be limited to the following:
Flagger Ahead signs to be covered or taken down when flaggers not
present, and Any drop off adjacent to the travel lane which exceeds 4"
must have type II drums with type 'A' flashers spaced every 100'.
8. Per County Ordinance, storm water is not pmmitted to discharge into the
right of way. Please revise the plans to correct the situation wherever this
situation occurs. .
9. ADA ramps will be required. All ramps must be constructed of concrete,
including those in the asphalt paths along the perimeter street.
10. All offsite utility work that is with in the right of way of the exterior streets
will need to be permitted through this office. Please contact Mr. Robb
Chadwell for the necessary permits and information.
11. Please be aware that further comments may be necessary at a later
time.

We have ~viewed the construction plans submitted to the Hamilton County Surveyorts
Office on November I, 2005 for this project and bave the following comments:
1. The proposed project falls in unincorporated Hamilton County.
2. Th,e proposed project falls in the Fitch & Jessup Regulated Drain Watershed.
3. The proposed project DOES NOT fall in a Carmel Wellliead Protection Zone.
4. The comments from the previous plans for this site, in regards to the
improvements to the Fitch & Jessup Drain., do not show up in these plans. please
reinsert the notes into this plan set.
5. Please show and label both the Fitch & Jessup regulated dnUn and corresponding
easement on the site utility plan (CI03).
6. The HeSO would reconunend you try to achieve H% cross slope on the dry
detention ba.s~ to help eliminate standing water issues. If the cross slopes are
unda- 1 %. HCSO would reoomtn.end SSD in the bottom of the dry detc;ntion basin
to help keep the area dry.

7. HeSO requires any storm pipe located within ourregula.ted drain easements to be

constructed with RCP to comply with this requirement. All other private pipe on
the side can be constructed with the HDPE pipe.
8. The landscape. plan show 2 new trees being planted within the Fitch & Jessup
Regulated Drain Ea5ement. HeSO is acceptable with thisan.d long as they are
not planted anyclosef to the drain then what is shown on the landscape plan.
9. Please note the additional comments may be warranted at a later date.
