The technical review and comments are intended to evaluate the completeness afthe erosion and sedIment control plan for the
project. The erosion and sediment control plan submitted was not reviewed for the adequacy o{the engineering design. All
practices included in the plan, as well as those recommended in the comments should be evaluated as to their feasibility by a
qualified individual with structural practices designed by a qualified engineer. The plan has not been reviewed for local, state,
or federal permits that may be required to proceed with this project. Additional information, including design calculations may
be requested to further evaluate the erosion and sediment control plan.
The erosion and sediment control plan bas been reviewed and it has been determined that the plan:
X Satisfies the minimum requirements and intent of 327IAC 15-5 (Rule 5). Notification will be forwarded to the
Indiana Department of Environmental Management.
X Refer to the comments section for additional infonnation.
Does not satisfy the minimum requirements and intent of 327IAC 15-5 (Rule 5); deficiencies are noted in the
checklist and in the comments section. Deficiencies constitute potential viola_ions of the rule and must be
adequately addressed for compliance. The information necessary to satisfy the deficiencies must be submitted:
Proper implementation of the erosion and sediment control plan and inspections o/the construction site by the developer or a
representative are necessary to minimize off-site sedimentation. The developer should be aware that unforeseen construction
activities and weather conditions may effect the performance of a practice or the erosion and sediment control plan. The plan
must be aflexible document, with provisions to modifY or substitute practices as necessary.

Dear Rich:
As we discussed on the phone, the current set of plans meet the requirements, but I would
ask for two adjustments:
1. The addition of so many Old Gold Junipers has created a mono-culture planting around
the edge of the rear yard parking lot. Using the same total number of shrubs, please
plant 4 different species instead of just one. This will increase the overall health of
the plantings and reduce the long term maintenance required.
2. Switch the 4 transplanted red oaks located in the parking lot islands to the south
of the building with 4 of the red maples in front of the building. This will provide a
better growing environment for the oaks, and lessen the potenial replacement costs for the
parking lot islands.

To all:
I met today with Rich Kelly of EMHT and Mark Monroe on the revised landscaping plan for the 103rd Street Medical
building submittal. Revised landscape plans were submitted and reviewed. These plans show increased plant material
(spruces) in the bufferyard along the north and west perimeters. Rich Kelly said this was in response to neighbors
concerned.
I gave Rich some suggestions for spacing, irrigation, mulching, and planting over sewer lines, including suggesting the use
of root barriers over the sewer lines to retard root growth into possible openings and cracks The plans met ordinance
requirements. The developers have gone to some effort satisfy all parties.

in the 30' landscape buffer to conform with ordinance
requirement for a Bufferyard D.

RB: Landscape plan for 103rd Street Medical Building expansion
Dear Rich:
My comments on the plans are as follows:
1. Since the 30-foot landscape buffer is to be extensively disturbed during the
construction of the storm sewer installation, the areas to be disturbed are required
to meet the bufferyard requirements found in the Carmel/Clay Zoning Ordinance
Chapter 26.4.5. Bufferyard D requires 5 shade trees, 5 orna111ental trees, and 27
shrubs (or 9 evergreen trees) per 100 linear foot section. It appears that
approximately 440 feet of greenbelt will be disturbed, requiring 22 shade trees, 22
ornamental trees and.ll9 shrubs (40 evergreen trees) to be planted in this area
after construction.
2. The existing plants can be transplanted back to the same location after
construction is completed and included in the number: of plants required, but it is
doubtful that if they are of any size over 4 inches in diameter, that they will
survive the process.
3. Street trees bordering Illinois Street are listed as Lamarcki Serviceberry. They are
a small growing omamental species. The other trees along this section of road are
all large growing shade trees (Ash and Sycamore species). Please choose a large
growing shade tree species for street trees. Red Oak (Quercus rubra) or Summit
Ash (Fraxinus pennsylvanica 'Summit'), both of which you are already using, are
acceptable alternatives.
4. Any transplanted plant material that does not survive will have to be replaced.
5. In the tree planting graphic detail, one note states "Ground line to be the same as
at the nursery." The should be changed to read "Plant matenal root flare needs to
be planted at grade level."

Per our conversation on April 26, 2002, I have advised Kite Construction of
the additional zoning-related items that we discussed. At this time, we
will delay our request for approval of the variance of development standards
for the building height until all zoning issues are identified and can be
presented under a single application. We trust that this can be
accomplished for presentation at the June 24, 2002 BZA meeting.
As I understand our conversation, there are additional items requiring
resolution as follows:
1. The property is presently zoned under three zoning classifications (B-1,
B-3, and B-6). It is your recommendation that the applicant petition to
rezone the property to a single classification (most likely B-3) .
2. Variances of Development Standards will be required for the building
height, as well as, to allow the existing parking spaces and existing sign
to encroach upon the required setback as established from the proposed
60-foot half right-of-way for Illinois Street as identified in the
Thouroughfare Plan.
After you have completed your review of the submitted plans, please confirm

Dear Board of Public Works and Safety:
Kevin Fosler, of CSO Architecls & Engineers and on behalf of Lilly lnduslries, has requested approval for
1\\0 curb culS for the proposed Lilly Industries Corporate Headquarters in the Meridian at 465 Business
Park. The sile is along tllC western side of the roundabout at 103rrl and Illinois Street.
The proposed 65,000 square foOL building is expected to generale traffic flows that would best be suited by
two curb cuts, one of which (south o[ roundabout) will allow [or southbound entry/exit only. Additionally,
the northem curb cut proposes to include a median cut, and the creation of a \cft lurn stacking lane to allow
for both north and southbound traffic entry/cxit. This office was informed of the likelihood that Lilly
lndustries would expand on this site, and has the additional space to the SOUtll of the building to do so. This
expansion would not include an additional curb cut, but would use these two curb cuts being requested at
this time.
Utility relocates (light poles within tllC median, ctc.) and signage ("No Left Turn", for exiting cars at the
southern entrance) for tllesc curb cuts will be the responsibility of the petitioner. Additionally, all curbs cut
are to be repaired andlor constnlcted according to standards accepted by the Department of Engineering.
Upon approval, the petitioner is required to submit a street cut/right-of-way pennit and post a $4000.00
street cuI bond for the (2) curb cut.

project includes a two-story, 65,000 sf. building with 95 proposed parking
spaces. There are two proposed curb cuts, one exiting onto Illinois St. and one
exiting onto 103rd S1. A higher intensity speculative office building was
proposed approximately one year ago on this site and received all necessary
approvals. This newer, less intense version of the site plan has left the curb
and median cuts in the same locations that were previously approved.
Attached you will find a reproducible 8 Yz" x 11" drawing of the proposed curb
cuts for use by the board. If you should have any questions or concerns, please

Sirs:
I would request that no further building be permitted unless effort is make to quiet the
unnecessary noise that is made by the existing air conditioning unit.
The noise has caused a significant reduction in property values in the residential housing
development (The Reserve) adjacent to the building. Previous feeble efforts to correct the
problem have been unsuccessful. I fear that a major modification is necessary.
I am sorry that I will be out of town and unable to attend the Plan CommissIon meeting.

New 126th Street Middle School
plans have been received by the District and reviewed by the District's engineering
consultant and will be ready for submittal to IDEM for state construction permits once the
District's construction specifications are added to the plans. The School Corporation also
needs to consider dedicating the sewers to the District for maintenance once they are
completed. The District is also negotiating with the School Corporation for easement
rights for a future force main through the property; if they remain private, no other
users besides the School Corporation will be allowed to connect. Building permits should
NOT be issued unless the District has issued connection permits.
Saddle Creek Sections 10 & 11
A comment letter was sent by the District's engineering consultant to Cripe in July 2001
but the District has not received any revised plans for review s.ince that time. Please
have the petitioner provide plans for review. Building permits should NOT be issued unless
the District has issued connection permits.
Cox Hall Park Maintenance Facility
The plans received by the District do not indicate how the facility proposes to receive
sanitary sewer service. District specifications need to be added to the plans. Building
permits should NOT be issued unless the District has issued connection permits.
Little Farms Lot 46 (Primary Plat Amendment)
The petitioner will need to contact the District to discuss the proposed sanitary sewer
connection; for an existing active line, connection via a manhole core drill is preferred.
