Docket No. 32;..02 OA, 31-02 CPA; Old Meridian District Amendment (ZO Chapter 20G) - Mixed Medical Zone, Old Meridian Comprehensive Plan Amendment - Mixed Medical Zone Petitioner seeks a favorable recommendation for the establislunent of the Old Meridian District - Mixed Medical Zone & Comprehensive Plan Policies. The area affected is generally located on the east and west sides of Old Meridian Avenue between Guilford Road and Main Street. Filed by the Depaliment of Community Services. 3i. Docket No. 33-02 Z; Rezone from Old Meridian District (OM)-Mixed Use (MU), Office (0), and Single Family (SF) to Old Meridian District (OM) - Mixed Medical (MM) Petitioner seeks a favorable recommendation on a rezone from Old Meridian District (OM)-Mixed Use (MU), Office (0), and Single Family (SF) to Old Meridian District (OM) - Mixed Medical (MM). The subject real estate is generally located on the east and west sides of Old Meridian A venue between Guilford Road and Main Street. Filed by St. Vincent Hospital and Health Care Center, Inc. Please refer to the attached information. Contained you will find the Text Amendment and Rezone in Ordinance format as well as the Comprehensive Plan Amendment in Resolution fonnat. In addition, a commitment has been included addressing dedication of right-of-way. The Special Studies Committee forwards a favorable recommendation to the full Plan Commission by a vote of 5-0 on all three requests. The Department recommends that the proposed text amendment and Comprehensive Plan Amendmeut be forwarded to the City Council with favorable recommendations. The Department recommends that the proposed rezone be forwarded to the City Council with favorable recommendation subject to the following conditions: 1. Identification of and an agreement to construct a trail that would extend from Main Street north along the future western entrance drive and across the current hospital site, across Guilford. This path is shown on the originally adopted Old Meridian plan and is an element the Department wants to maintain. 2. Dedication ofright of way pursuant to the Thoroughfare Plan (131 st St., Guilford, Old Meridian) and including a roundabout at Guilford/Old Meridian subject to approval of the City Engineer. Note: Three separate recommendations should be made.
