7. For a slab-on-grade floor, the perimeter insulation
shall extend downward from the top of the slab to the botton
of a trencl-? footinc in accordance with Section 502.2.1.4,
IECC (675 IAC 19-3-1),
8. The exterior wall assembly ahall have a combined thermal
transmittance value (Uo) of the gross area of exterior walla
not exceeding the value given in Table 502.3.1. Equation
No. 1 shall be used to determine the acceptable combinations
in accordance with Section 502.3.1.1., IECC (675 IAC 19-3-1).
9• The roof/coiling assembly shall have a combined thermal
transmittance value Wo-i for the gross area of the roof not
exceeding the value given in Table 502.3.1, in accordance
with Section 502.3.1 2, IECC (675 IAC 19-3-1).
10. Beginning April 1, 1995, new projects which are re-
quired to be certified by a design professional, in accord-
ance with 675 IAC 12-6-9(a)) shall include an affidavit, ex-
ecuted on the form provided by the Plan Review Division, by
each design professional certifying portions of the project.

design and corresponding inspection activities in accordance

the installation, building, or business, or by the owner's
legally authorized representative, in accordance with 675

The plans. specifications and application submitted for the above referenced project have been reviewed for compliance with the applicable rules of the
Fire Prevention and Building Safety Commission. The project is released for construction subject to, but not necessarily limited to, the conditions listed
below. THIS IS NOT A BUILDING PERMIT. All required local permits and licenses must tie obtained prior to beginning construction work.
All construction work must be in full compliance with all applicable state rules. Any changes in the released plans andior specifications must be filed

2. The complete installation shall comply with NFPA
Standard 13 (1375 IAC 13-1-8); Chapter 38, IBC 1675 TAC
13-2.2); and all other applicable codes and standards.
3. The sprinkler installation was reviewed for a Light
Hazard Occupancy. The method of design was "Pipe Schedule"
in accordance with Chapters 2 and 8, NFPA Standard #13 (1175
IAC 13-1-8)
4. The location of the fire department connection shall be
approved by the local Fire department in accordance with
Section 3801(b), IBC (675 IAC 13-2.2).
5. An inspector's test connection shall be provided in
accordance with Section 2-9, NFPA Standard #13 (675 IAC
13-1-8).
A set of documents released by this office shall be maintained on the construction site until the structure is occupied (675 IAC 12-6-19).

This letter provides notice of the action taken by the Fire Prevention
and Building Safety Commission on your application(s) for a variance(s) from
the Commission's rules under IC 22-13-2-11 and 675 IAC 12-5. The commission
considered the application, as part of its published agenda, at its regular
meeting on July 5, 1995. Information regarding action taken by the
Commission is provided on the attached sheet.
You are advised that if you desire an administrative review of this
action, you must file a written petition for review at the above address
with the Fire Prevention and Building Safety Commission. Your petition must
fully identify the matter for which you seek review no later than eighteen
days from the above-stated date, unless such date is a Saturday, Sunday,
legal holiday under state statute, or day that the Fire and Building
Services Department's offices are closed during the regular business hours;
in which case the deadline would be the first day thereafter that is not a
Saturday, Sunday, legal holiday under state statute, or day that the Fire
and Building Services Department's offices are closed during the regular
business hours. If you do so, your petition for review will be granted and
an administrative proceeding will be conducted by an administrative law
judge appointed by the Fire Prevention and Building Safety Commission. If
you do not file a petition for review, this action will be final.
The purpose of the administrative hearing is to allow you to present
information to the administrative law judge concerning why the decision
should not be upheld. This information can include a re-presentation of
your petition and the reasons already stated, and/or new reasons why the
decision should not be upheld. Please be advised that agency counsel will
be present at such a proceeding, and that you may be represented by counsel
if you wish. Again, if you do not file a petition for review, this action
will be final.
