This project includes the installation of 41 feet of 4-inch P.V.C. sanitary sewer, a 1,000-gallon septic tank, a 1,000-gallon dosing tank with a simplex 46 gpm submersible pump and a dosing volume of 132 gallons, and an elevated sand mound with at least 250 square feet of bed area and 868 feet of basal area encircled by a perimeter drain. This approval letter shall act as your construction permit. This Permit shall be void if construction is not begun before April 1, 1996. Approval of this project is conditional and subject to the following requirements: That if pollution, health hazards, or nuisance conditions develop or are created, immediate corrective action be taken by the owner. That no change in occupancy or use of the facility served be effected if it would result in wastewater flow an the peak day in excess of 300 Gallons Per Day, or if it would result in wastewater being generated of a type incompatible with absorption field disposal. Any such change in occupancy or use may be made only after the department has issued a construction permit for modifications to the subject wastewater disposal facility that will allow it to accommodate increased wastewater flows. That the permittee notify the department and the local health department at least 7 days before construction of the approved commercial on-site wastewater disposal facilities is to commence. D7 i raeh Robert Irsay -2- That all necessary local permits and approvals be obtained before construction is begun on this project. You are hereby notified that most county and local health departments, and several conservancy districts as well, require that a sewage disposal permit be obtained before construction may begin. The sanitary features of this project must also comply with any additional local health department requirements. That plans and specifications for any changes, alterations or additions to this project as herewith approved be submitted and approved prior to such construction. That Sanitary Engineering, AC 317/383-6177, State Department of Health, P.D. Box 1964, Indianapolis, Indiana 46206-1964, be notified at least 15 days prior to the date mound construction is to be undertaken so that a pre-construction conference with the contractor can be arranged. It is of vital importance to the success of your mound disposal system that the contractor has a complete understanding of mound construction techniques. These plans and specifications were prepared by Allan H. Weihe, P. Indianapolis, Indiana and submitted on January 26, and March 1, 1995. If you wish to request review of this Approval, you must petition for review in writing, demonstrating that: You are a person to whom the Approval is specifically directed; You are aggrieved or adversely affected by the Approval; or, You are entitled to review under any law. Your request for review must be filed in writing with the Manager, Sanitary Engineering, Indiana State Department of Health, P.0.
